This article encompasses care of the resident and family and dealing with the staff's concerns regarding this rare, infectious dementia. Comparison is made between Alzheimer's disease and Creutzfeldt-Jakob disease, the symptoms and causative agent, precautions that need to be followed, nursing care and medications used in maintaining the resident's comfort, and the maximum quality of life possible during the course of this terminal disease.

Though patients usually die peacefully, problems may arise in the last period of a terminal illness. In the final days new symptoms may arise or there may be exacerbation or recurrence of symptoms previously well controlled. Two hundred consecutive hospice patients were studied. The incidence was noted of pain, dyspnea, moist breathing, nausea and vomiting, confusion, restlessness, jerking and twitching, difficulty in swallowing, incontinence and retention of urine, sweating, moaning and groaning, and loss of consciousness. Each symptom is considered and the results of the management employed

AIM: To evaluate the impact of a palliative care educational intervention on nursing students' attitudes toward end-of-life (EOL) care, using the Frommelt Attitude Toward Care of the Dying Scale (FATCOD-B).
METHODS: A pre-post design was employed and 52 third-year nursing students enrolled in the study and 39 completed the post-intervention assessment. Attitudinal changes of nursing students were measured before and after a palliative care module was taken. Data were analysed using descriptive statistics and paired student's t-tests. Additional subgroup analyses were conducted based on age, g

CONTEXT: Whether differences in program composition and provider staffing translate into meaningful variation in end-of-life (EOL) outcomes remains poorly understood, underscoring the need to examine how palliative care (PC) is delivered within hospitals.
OBJECTIVES: This study applies a data-driven approach to understand longitudinal PC delivery of service across different types of providers ("provider sequence") and explore whether these patterns of PC delivery correlate with EOL quality measures for patients with poor-prognosis advanced cancer.
METHODS: We conducted a retrospective cohort

Dealing with dying and death is an integral part of working in an intensive care unit, yet time pressure and workload often leave little room for emotional processing. A shared moment of silence offers intensive care staff the opportunity to pause after a patient's death, to uphold dignity, and to foster team reflection. This article explores the origin and significance of the ritual, summarizes the current evidence, and provides practical recommendations for its implementation-including appropriate language, involvement of relatives, framework conditions, and evaluation. The moment of silence

Advanced lung disease, including chronic obstructive pulmonary disease (COPD) and various types of interstitial lung disease (ILD), are common disabling respiratory conditions with increasing prevalence in older adults. In this chapter, we provide an overview of COPD and ILD along with a summary of palliative care issues relevant to these populations. While the diseases tend to be gradually progressive, the time course and prognostication is highly variable with long periods of stability and occasional severe exacerbations. The symptom burden in advanced lung disease is high, with significant

CONTEXT: Provision of quality care may diminish expression of wish to die (WTD), but this relationship has not been empirically assessed.
OBJECTIVES: To assess the association between institutional quality of care and expression of WTD among patients with advanced cancer.
METHODS: We surveyed patients with advanced cancer from 9 hospitals in 8 low- and middle- income countries (Bangladesh, China, India, Thailand, the Philippines, Myanmar, Sri Lanka and Vietnam). We estimated mixed-effects logistic regression model to assess the relationship between patient and provider reported quality of ca

There is very little documented evidence on the experiences of healthcare professionals caring for their own family outside of their role. This article explores the differences in the care received as the author shares her perspectives of caring for her mother, who was diagnosed with dementia. The author reflects on the care received and the impact of communication, as well as the ethical limitations of caring for a member of her own family. Significant references to communication barriers already exist within the literature, relating to end-of-life conversations. The presence of a relative wh

The primary goal of intensive care medicine is to overcome a critical phase of illness using all available means and to enable patients to survive and return to an independent life without intensive care. However, this therapeutic goal cannot always be achieved. The possibility of death, the need to alleviate suffering, and respect for the end of life make it necessary to integrate palliative approaches into intensive care medicine. Supporting seriously ill and dying patients and their relatives in intensive care units requires an attitude that holistically recognizes and respects their indivi

BACKGROUND: Geographic variations in end-of-life healthcare utilisation may highlight disparities in access to care, including palliative care. Studies demonstrate that place of death and palliative care utilisation are influenced by geographic exposures such as sociodemographic and socioeconomic characteristics, rurality, and health service availability. No systematic review has synthesised the evidence across multiple geographic exposures. This is important to provide conclusions from a broader evidence base and inform equitable commissioning of palliative care services. AIM: To investigate

Similar to other institutions, the Swiss prison system faces a growing number of elderly prisoners, trends toward securitization, and, in consequence, more prisoners who will spend the end-of-life (EOL) period of time in prison. By law, prisoners should have the same access to care as the rest of the population. However, custody makes meeting the demands of medical and palliative care difficult. This article focuses on the organizational challenges related to EOL care. Based on ethnographic and documentary research, it examines the institutional logic of the prison and the competing "new" logi

BACKGROUND: We describe the experience of managing the dying moments of a nursing home patient via telemedicine.
CASE PRESENTATION: Ms. C was a 92-year-old frail woman with multiple medical problems, living in a nursing home. She spent her final days in the nursing home, choosing not to be transferred to an acute hospital should she turn ill. On the last day of her life, she complained of acute-onset breathlessness and agreed to a teleconsultation with the hospital physicians involved in acute care.
CASE MANAGEMENT: During the telemedicine consultations (tele-consultation) process, Ms. C's c

BACKGROUND: The impact of palliative care consultation on the management and outcomes of patients hospitalized with out-of-hospital cardiac arrest (OHCA) remains poorly understood. This study examined associations between palliative care consultation and in-hospital outcomes of patients hospitalized with OHCA, stratified by survival status.
METHOD: This cross-sectional study used data from the National Inpatient Sample (2016-2021). Adult patients hospitalized with OHCA who received cardiopulmonary resuscitation were included. Multivariable analyses assessed associations between palliative car

BACKGROUND: Clinicians observe that advanced cancer patients with dependent children agonize over the impact their death will have on their children. The objective of this study was to determine empirically whether advanced cancer patients with and without dependent children differ in treatment preferences, mental health, and end-of-life (EOL) outcomes.
METHODS: Coping with Cancer is a National Cancer Institute/National Institute of Mental Health-funded, multi-institutional, prospective cohort study of 668 patients with advanced cancer. Patients with and without dependent children were compar

INTRODUCTION: Critical care patients face varying degrees of physical and psychological stress; in recent decades, the increasing focus on technologies and clinical processes has led to dehumanisation. In contrast, humanistic care has been introduced as an essential care paradigm, emphasising comprehensive care and considering all stakeholders in the care process (patient, family and staff). In addition to improving clinical outcomes, this approach focuses on family satisfaction, preserving the human dignity of patients and reducing the incidence of acute cognitive disorders such as delirium;
