Whether cranial expansion is indicated in selected cases with microcephaly?

PURPOSE: Some children with microcephaly experienced severe social emotional deficits during their 1-2 years of age. Cranial expansion is generally not used in these cases. Our aim is to assess whether such operation is effective to improve their social emotional status, and thereby boosting their functional development in selected cases with such condition. METHODS: A retrospective cohort review in microcephalic cases who had undergone cranial expansion in Shanghai Children's Hospital since Jun. 2016 to Jun. 2017 with at least 12 months follow-up was conducted. Inclusion and exclusion criter

Huntington disease.

Huntington disease is devastating to patients and their families - with autosomal dominant inheritance, onset typically in the prime of adult life, progressive course, and a combination of motor, cognitive and behavioural features. The disease is caused by an expanded CAG trinucleotide repeat (of variable length) in HTT, the gene that encodes the protein huntingtin. In mutation carriers, huntingtin is produced with abnormally long polyglutamine sequences that confer toxic gains of function and predispose the protein to fragmentation, resulting in neuronal dysfunction and death. In this Primer,

Scrotal reconstruction in female-to-male transsexuals: a novel scrotoplasty.

BACKGROUND: One of the goals of genital construction in female-to-male transsexuals is the creation of an aesthetically acceptable result, both for phallus and scrotum, leaving minimal morbidity and recreating function. In the last 15 years, transsexuals have become more demanding, and scrotoplasty has received more attention than before. Traditional flaps for scrotal reconstruction in a biological male do not really apply in transsexuals: the labia majora seem to achieve the best results; still, they may not provide enough tissue and can be located much too posteriorly. METHODS: Since Novemb

Correction of anophthalmic enophthalmos with injectable calcium hydroxylapatite (Radiesse).

A 66-year-old man who presented with anophthalmic enophthalmos desired improvement in his appearance without invasive surgery. A nonincisional technique for volume augmentation in the anophthalmic socket with injectable calcium hydroxylapatite was performed. Clinical improvement was determined by comparison of preoperative and 6-month postoperative exophthalmometry values and photographs. Injectable calcium hydroxylapatite successfully increased orbital volume in this anophthalmic patient.

Spinocerebellar ataxia 7 (SCA7).

Spinocerebellar ataxia 7 (SCA7) is a progressive autosomal dominant neurodegenerative disorder characterized clinically by cerebellar ataxia associated with progressive macular dystrophy. The disease affects primarily the cerebellum and the retina, but also many other CNS structures as the disease progresses. SCA7 is caused by expansion of an unstable trinucleotide CAG repeat encoding a polyglutamine tract in the corresponding protein, ataxin-7. Normal SCA7 alleles contain 4-35 CAG repeats, whereas pathological alleles contain from 36-306 CAG repeats. SCA7 has a number of features in common wi

[Hard palate lengthening by palatal suture expansion: an experimental study in dogs].

OBJECTIVE: Scar contraction and lack of bony support for soft palate in traditional surgery of cleft palate are main causes of a shortening soft palate and its developmental deficiency. The wound and scar also lead to aberration of facial growth. In order to resolve these problems, lengthening of hard palate by transverse palatal suture expansion was studied. METHODS: Six mongrel dogs of 8-week age were used in the study. Amalgam markers were implanted in the hard palate of the animal. A NiTi-shaped memory alloy (NiTi-SMA) arch wire with a force level of 600 g was used to expand the palato-ma

Staged reconstruction of abdominal wall defects after intra-abdominal catastrophes.

Advances in surgical intensive care have improved survival in patients with major traumatic or infectious intra-abdominal insults. Patients who recover are often left with massive abdominal wall defects. Sufficient autogenous tissue may not be available for reconstruction and synthetic mesh followed by skin grafting can lead to unaesthetic results or complications. We report on four patients with abdominal wall defects and their reconstruction after intra-abdominal injury. Treatment involved local wound care to stimulate granulation tissue, which is eventually skin grafted to close the wound.

Expanded pectoralis major myocutaneous flaps in head and neck surgery.

The use of tissue expanders in head and neck surgery has been popularized since the early 1980s. Tissue expanders are currently used in nasal reconstruction, alopecia, and other head and neck defects when there is inadequate tissue or when transposition of a local flap would create a significant donor-site deformity. Occasionally, situations in head and neck reconstruction arise where local, regional, or free flaps are not available or sufficient for head and neck reconstruction. In these situations, the use of controlled tissue expansion can be employed for enlarging myocutaneous flaps for he…

The preoperative use of extra-tissue expander for syndactyly.

To our knowledge, this is the first report of a method of stretching the interdigital skin of syndactyly by means of a pincer. The pincer, a U-shaped metal spring plate with a sponge cushion, is applied before the operation for about two months; pressure is placed against both sides of the interdigital fusion to expand the skin and to develop hollows. Use of the pincer facilitates a syndactyly operation by allowing defect coverage with a local flap only. Four patients successfully treated by this method are reported. The method has the great advantage of not requiring skin grafts, which often

Correction of congenital microtia using the tissue expander.

We attempted auricular reconstruction using Radovan-type inflatable silicone expanders in six children and one adult, with the complete hypoplastic, the conchal remnant, and constricted type of microtia. Ear frameworks, including the helix, anthelix, concha, and tragus, were prepared using autologous rib cartilage. Based on the surface area of the normal adult auricle, the silicone expander was tentatively shaped and sized into a rotated semiellipse and expanded with 70 cc saline. Auricular reconstruction on the framework was completed at the time of insertion in four of the seven patients, re

Large-Volume Breast Autologous Fat Transfer Entirely Under Tumescent Anesthesia: Experience With 87 Patients Over 6 Years.

BACKGROUND: Autologous fat transfer (AFT) provides a natural alternative to implants for breast augmentation. While most large-volume AFT is performed under general anesthesia, experience with fully tumescent local anesthesia is limited. This study evaluates the safety, efficacy, and volumetric outcomes of Breast Autologous Fat Transfer Entirely Under Tumescent Anaesthesia (BAFTEUTA) and examines the role of pre-expansion versus enrichment in graft survival. PATIENTS AND METHODS: In this prospective single-center study, 87 patients (174 breasts) underwent large-volume AFT under local tumescen

Pre-Expanded Submental Island Flap for Resurfacing Middle and Lower Facial Defect.

Resurfacing large facial defect is a continuing challenge for plastic surgeons. Skin graft or free flap is hard to obtain satisfactory results or is beyond the skill of most surgeons. The authors performed 13 expended submental island flaps to resurface middle and lower facial defects and achieved satisfactory results. In the first stage operation, the authors implanted one soft tissue expander in the anterior neck region which was expanded over an average of 3 months. In the second stage operation, the authors elevated the expanded submental island flap to resurface facial defect. For the pat

The true meshing ratio of skin graft meshers.

Skin meshing is frequently used in the coverage of extensive burn injuries as well as other skin and soft tissue wounds. This technique allows coverage of more extensive areas with smaller donor sites and prevents fluid from collecting beneath the skin grafts. The devices used to achieve this expansion differ in their technology and the use of skin carriers. In addition, many of the devices permit meshing at single or multiple ratios depending upon the device chosen. Although commonly used, there have been few definitive studies analyzing the actual expansion ratios achieved by many of these d

Mucosal Perfusion Preservation by a Novel Shapeable Tissue Expander for Oral Reconstruction.

BACKGROUND: There are few methods for expanding oral mucosa, and these often cause complications such as tissue necrosis and expander eruption. This study examines mucosal blood perfusion following insertion of a novel shapeable hydrogel tissue expander (HTE). The canine model used subgingival insertion of HTE following tooth extraction and alveolar bone reduction. The primary goal of this study was to gain understanding of epithelial perfusion and reparative responses of gingival mucosa during HTE expansion. METHODS: Nine Beagle dogs underwent bilateral premolar maxillary and mandibular toot

Characterization of intestinal immune responses in generalized human and murine lipodystrophy.

Acquired generalized lipodystrophy (AGL) is a rare metabolic disorder frequently associated with autoimmunity. Its etiology is incompletely understood, and the effect of adipose tissue loss on intestinal inflammation in AGL remains unclear. Using mass cytometry and single-cell RNA-seq, we observed an oligoclonal expansion of T cells in the periphery and inflamed intestine in a patient with AGL and Crohn's disease (AGLCD). To explore if loss of adipose tissue triggers lymphoproliferation, we studied lipodystrophic mice as a model for AGL. Unexpectedly, lipodystrophic mice did not show T cell ex

Closure of wounds in the upper extremity using a skin stretching device.

A skin stretching device (SSD) harnessing the viscoelastic properties of skin using incremental traction has been used in 20 patients with skin deficits in the upper extremity. Complications were few considering the extensive damage to the skin and included slight partial dehiscence, necrosis of skin edges, local infection and hypertrophic scars. All wounds healed without the need for further surgical procedures. Application of the SSD is simple and it can even be used at the bedside under local anaesthesia. It reduces the need for more complicated surgical procedures like grafts or flaps.
