The use of tranexamic acid in external dacryocystorhinostomy: a systematic review and meta-analysis of randomized controlled trials.

PURPOSE: Tranexamic acid (TXA) is an antifibrinolytic agent effective in reducing extensive blood loss and mortality. However, it lacks significant clinical evidence in peri-ocular procedures, including dacryocystorhinostomy (DCR). This study aims to assess the effectiveness of TXA in reducing intraoperative hemorrhage and improving surgical outcomes. METHODS: We searched PubMed, Medline, ClinicalTrials.gov, and the Cochrane Library, along with grey literature sources from the year 2000 onwards for randomized controlled trials (RCTs). We used the revised Cochrane Risk of Bias tool (RoB 2.0) t…

Evaluation of Tranexamic Acid for the Reduction of Blood Loss During Burn Surgery in Adults (Tranburn Study).

The objective of this study is the evaluation of the use of tranexamic acid (TXA) for the reduction of blood loss and the use of blood products. We conducted a prospective, multicenter, interventional, randomized, blinded study in 4 burn intensive care units. Patients over 18 years of age with at least 5% body surface area burns scheduled for surgery were included. The treated group received 1g TXA intravenously, then 1g TXA over 8h. The placebo group received saline. Blood loss was calculated using the Mercuriali formula. 121 patients were enrolled. The groups were similar in terms of sex, ag…

The statistical fragility of tranexamic acid dosage and route of administration in total hip arthroplasty: A systematic review.

BACKGROUND: Tranexamic acid (TXA) is an established method of reducing blood loss during total hip arthroplasty (THA). Despite many randomized controlled trials (RCTs) examining its use, the optimal dose and route of administration of TXA in THA remain up for debate. Therefore, this study aimed to determine the robustness of RCT findings in this field by applying statistical fragility methodology, including fragility index (FI) and fragility quotient (FQ) calculations. METHODS: Embase, MEDLINE, and Pubmed were searched for RCTs examining the dose, administration, or the pairing of TXA with an…

Impact of Tranexamic Acid on Traumatic Hemorrhage Outcomes in Emergency Medicine: A Systematic Review and Meta-Analysis.

Hemorrhage is the predominant cause of death among trauma patients. Therefore, pharmacological treatments such as tranexamic acid (TXA) have been explored to supplement the conventional measures of controlling bleeding in this population. TXA has been found beneficial among patients undergoing elective surgery. However, its use to mitigate bleeding among trauma patients in different settings remains controversial. Therefore, we carried out this study to examine the efficacy of prehospital and in-hospital TXA among trauma patients in military and civilian settings. PubMed, MEDLINE, Cochrane Cen…

The Effectiveness and Safety of Tranexamic Acid in Treating Aneurysmal Subarachnoid Hemorrhage: A Systematic Review and Meta-Analysis.

Background/Objectives: Tranexamic acid (TXA) is a synthetic antifibrinolytic agent that inhibits plasminogen activation, thereby reducing bleeding. The aim of this systematic review was to investigate its role in aneurysmal subarachnoid hemorrhage (SAH)-a condition indicated by bleeding between two layers of brain tissue-to stop rebleeding and improve patient outcomes. Methods: We conducted a systematic review and meta-analysis of randomized controlled trials from 1981 to 2024, focusing on the efficacy and safety of TXA in treating aneurysmal SAH (PROSPERO registration: CRD42024504834). Our co…

Pharmacokinetics of Tranexamic Acid (TXA) Delivered by Expeditious Routes in a Swine Model of Polytrauma and Hemorrhagic Shock.

OBJECTIVE: Hemorrhage is the leading cause of preventable death in civilian trauma centers and on the battlefield. One of the emerging treatment options for hemorrhage in austere environments is tranexamic acid (TXA). However, the landscape is not amenable to the current delivery standard. This study compared the pharmacokinetics of TXA via a standard 10-minute intravenous infusion (IV infusion), intravenous rapid push over 10 s (IV push), and intramuscular injection (IM) in a swine polytrauma and hemorrhagic shock model (trauma group) compared to uninjured controls (control group). METHODS: …

Impact of time to start of tranexamic acid treatment on rebleed risk and outcome in aneurysmal subarachnoid hemorrhage.

INTRODUCTION: The ULTRA-trial investigated effectiveness of ultra-early administration of tranexamic acid (TXA) in subarachnoid hemorrhage (SAH) and showed that TXA reduces the risk of rebleeding without concurrent improvement in clinical outcome. Previous trials in bleeding conditions, distinct from SAH, have shown that time to start of antifibrinolytic treatment influences outcome. This post-hoc analysis of the ULTRA-trial investigates whether the interval between hemorrhage and start of TXA impacts the effect of TXA on rebleeding and functional outcome following aneurysmal SAH. PATIENTS AN…

The effect of tranexamic acid on synovium of patients undergoing arthroplasty and anterior cruciate ligament reconstruction surgery.

Preoperative hemorrhage can be reduced using anti-fibrinolytic medicine tranexamic acid (TXA). During surgical procedures, local administration is being used more and more frequently, either as an intra-articular infusion or as a perioperative rinse. Serious harm to adult soft tissues can be detrimental to the individual since they possess a weak ability for regeneration. Synovial tissues and primary fibroblast-like synoviocytes (FLS) isolated from patients were examined using TXA treatment in this investigation. FLS is obtained from rheumatoid arthritis (RA), osteoarthritis (OA), and anterior…

Removal of airway obstruction due to presence of blood clots and the role of tranexamic acid - case report.

During invasive mechanical ventilation, the physiological mechanisms of clearing secretions from the bronchial tree are impaired. Factors affecting this process include inhibition of the cough reflex and ciliary movement in the airways and the use of sedating drugs. The standard clinical practice is the suctioning of the residual secretions in the bronchial tree performed blindly with a suction catheter. Repeated introduction of the catheter may lead to mechanical injury of the respiratory tract mucosa [1].

Tranexamic acid suppresses the release of mitochondrial DAMPs and reduces lung inflammation in a murine burn model.

BACKGROUND: Severe burn injuries are known to initiate a profound systemic inflammatory response (SIRS) that may lead to burn shock and other SIRS-related complications. Damage-associated molecular patterns (DAMPs) are important early signaling molecules that initiate SIRS after burn injury. Previous work in a rodent model has shown that application of a topical immune modulator (p38MAPK inhibitor) applied directly to the burn wound decreases cytokine expression, reduces pulmonary inflammation and edema. Our group has demonstrated that tranexamic acid (TXA)-in addition to its use as an antifib…

Role of topical tranexamic acid in the management of idiopathic anterior epistaxis in adult patients in the emergency department.

PURPOSE: The role of topical tranexamic acid in the management of anterior epistaxis in adult patients in the emergency department (ED) is examined. SUMMARY: The use of alternative agents for the treatment of epistaxis before the use of nasal packing may be reasonable due to patient discomfort, potential complications, and the need for follow-up with a healthcare provider for packing removal. One such agent is tranexamic acid. Two published studies evaluated the off-label use of topical tranexamic acid for the treatment of epistaxis. The first trial compared the efficacy of a topical gel cont…
