The role of transcranial magnetic stimulation in evaluation of motor cortex excitability in Rett syndrome.

UNLABELLED: Rett syndrome (RTT) is a frequent neurodevelopmental disorder confirmed by clinical criteria and supported by the methyl-CpG-binding protein 2 gene (MECP2) mutation. A short central motor conduction time (CMCT) was reported in transcranial magnetic stimulation (TMS) studies performed in RTT. This was attributed to hyperexcitability of the motor cortex and/or spinal motor neurons, but was not studied further. AIM: We performed TMS in RTT to evaluate motor cortex excitability by determining the cortical motor threshold (CMT) and motor cortex inhibition by the cortical silent period …

A systematic review of the effects of low-frequency repetitive transcranial magnetic stimulation on cognition.

rTMS is increasingly used for a variety of neuropsychiatric conditions. There are data to support 'fast' rTMS (≥10 Hz) having some positive effects on cognitive functioning, but a dearth of research looking at any such effects of 'slow' rTMS. This question is important as cognitive dysfunction accompanies many neuropsychiatric conditions and neuromodulation that potentially enhances or hinders such functioning has important clinical consequences. To determine cognitive effects of slow (≤1 Hz) rTMS, a systematic review of randomized control trials assayed cognition in neurological, psychiatric,

Efficacy of Transcranial Magnetic Stimulation (TMS) in the Treatment of Schizophrenia: A Review of the Literature to Date.

We reviewed the literature on transcranial magnetic stimulation and its uses and efficacy in schizophrenia. Multiple sources were examined on transcranial magnetic stimulation efficacy in relieving positive and negative symptoms of schizophrenia. Literature review was conducted via Ovid Medline and PubMed databases. We found multiple published studies and metaanalyses that give evidence that repetitive transcranial magnetic stimulation can have benefit in relieving positive and negative symptoms of schizophrenia, particularly auditory hallucinations. These findings should encourage the psychia…

The precuneus and visuospatial attention in near and far space: a transcranial magnetic stimulation study.

BACKGROUND: There is a large body of evidence for the involvement of the parietal cortex in orientation and navigation in space. This has been supplemented by investigation of the contribution of a number of subregions using transcranial magnetic stimulation. OBJECTIVE: The role of the precuneus area, located in the medial plane of posterior parietal cortex (PPC), in visuospatial functions is not well understood. We investigated the contribution of this area using the landmark task. METHODS: Participants were asked to make forced-choice judgments of which side of prebisected line was longer …

Transcranial magnetic stimulation as an antidepressant alternative in a patient with Brugada syndrome and recurrent syncope.

Brugada syndrome (BrS) is a common occult cause of sudden cardiac arrest in otherwise healthy-appearing adults. The pathognomonic electrocardiographic pattern may be unmasked only by certain medications, many of which are unknown. We report a case of a depressed but otherwise healthy man with an asymptomatic right bundle branch block on electrocardiography who experienced antidepressant-induced BrS and ultimately recovered with transcranial magnetic stimulation (TMS). After an initial trial of nortriptyline, the patient's depressive symptoms improved; however, he experienced a syncopal event a…

The experience of patients with schizophrenia treated with repetitive transcranial magnetic stimulation for auditory hallucinations.

Introduction. Auditory hallucinations are a common symptom experience of individuals with psychotic disorders and are often experienced as persistent, distressing, and disruptive. This case series examined the lived experiences of four individuals treated (successfully or unsuccessfully) with low-frequency (1 Hz) rTMS for auditory hallucinations. Methods. A phenomenological approach was used and modified to involve some predetermined data structuring to accommodate for expected cognitive impairments of participants and the impact of rTMS on auditory hallucinations. Data on thoughts and feeling

The emerging role for repetitive transcranial magnetic stimulation in optimizing the treatment of adolescent depression.

Major depressive disorder (MDD) in adolescents is a common illness and significant public health problem. Treatment is challenging because of recurrences and limited modalities. Selective serotonin reuptake inhibitors and cognitive behavioral therapy are considered the standard of care in severe or treatment-resistant MDD in this age group. However, responses to these interventions are often suboptimal. A growing body of research supports the efficacy of repetitive transcranial magnetic stimulation (rTMS) for the treatment of MDD in adults. Induced seizures are a primary safety concern, althou…

A randomized clinical trial of repetitive transcranial magnetic stimulation in the treatment of major depression.

BACKGROUND: Multiple groups have reported on the use of repetitive transcranial magnetic stimulation (rTMS) in treatment-resistant major depression. The purpose of this study is to assess the efficacy of rTMS in unmedicated, treatment-resistant patients who meet criteria for major depression. METHODS: Depressed subjects, who had failed to respond to a median of four treatment trials, were assigned in a randomized double-blind manner to receive either active (n = 10; 20 2-sec trains of 20 Hz stimulation with 58-sec intervals; delivered at 80% motor threshold with the figure-of-eight coil posit…

Long-term effect of transcranial magnetic stimulation and transcranial electrical stimulation in primary progressive aphasia: study protocol for a randomised, double-blind clinical trial (RECONNECT-PLUS).

INTRODUCTION: Primary progressive aphasia (PPA) is a neurodegenerative syndrome associated with Alzheimer's disease and frontotemporal degeneration. Non-invasive brain stimulation (NIBS) is a promising treatment, especially associated with language therapy, but comparative efficacy and long-term effects between the different techniques (transcranial direct current stimulation (tDCS) and transcranial magnetic stimulation (TMS)) remain unknown. The present study aims to investigate the effects of non-invasive brain stimulation, alone or associated (tDCS/TMS/tDCS plus TMS) combined with language …

Comparative efficacy and safety of photobiomodulation, transcranial direct current stimulation, and repetitive transcranial magnetic stimulation in Alzheimer's disease: a network meta-analysis of randomized controlled trials.

Alzheimer's disease (AD) is a progressive neurodegenerative disorder for which current pharmacological therapies provide only modest and transient benefits, underscoring the need for effective non-pharmacological interventions. Non-invasive brain stimulation (NIBS), including photobiomodulation (PBM), transcranial direct current stimulation (tDCS), and repetitive transcranial magnetic stimulation (rTMS), is a promising approach. This study compared the efficacy, feasibility, and safety of PBM, tDCS, and rTMS in AD. We conducted a Bayesian network meta-analysis (NMA) of randomized controlled tr…

Controversy: Repetitive transcranial magnetic stimulation or transcranial direct current stimulation shows efficacy in treating psychiatric diseases (depression, mania, schizophrenia, obsessive-complusive disorder, panic, posttraumatic stress disorder).

Brain imaging studies performed over the past 20 years have generated new knowledge about the specific brain regions involved in the brain diseases that have been classically labeled as psychiatric. These include the mood and anxiety disorders, and the schizophrenias. As a natural next step, clinical researchers have investigated whether the minimally invasive brain stimulation technologies (transcranial magnetic stimulation [TMS] or transcranial direct current stimulation [tDCS]) might potentially treat these disorders. In this review, we critically review the research studies that have exami…

Repetitive transcranial magnetic stimulations of the rat. Effect of acute and chronic stimulations on pentylenetetrazole-induced clonic seizures.

Repetitive transcranial magnetic stimulation (RTMS) has been reported to induce epileptic seizures in normal control and in epileptic patients. Therefore we characterized the effect of acute or chronic stimulations with RTMS on the induction of pentylenetetrazole (PTZ)-induced clonic seizures in the rat. Male Wistar rats were stimulated with a 13-cm coil with a stimulus frequency of 50 Hz. The motor threshold (Tm) was determined by a single transcranial stimulus. Acute stimulation was performed with a stimulus intensity of 0.9 x Tm and 1.5 x Tm using a duration of the train of stimuli of five …

A coupling model of transcranial magnetic stimulation induced electric fields to neural state variables.

Transcranial magnetic stimulation (TMS) is a non-invasive brain stimulation technique used to modulate neural activity, with applications in clinical treatment, diagnostics, and neuroscientific research. TMS targeting the human primary motor cortex (M1) is well studied, aided by experimental readouts from the cerebral cortex, the spinal cord, and activated muscle targets. One key readout is a series of pulses that descend the spinal cord following TMS, called DI-waves. These reflect the output of M1 to the spinal cord and are influenced by TMS parameters such as orientation, strength, and wave…

Validating language localization by transcranial magnetic stimulation: A comparative study of adults and children.

Transcranial magnetic stimulation (TMS) is increasingly used for language mapping in epilepsy surgery. However, due to small sample sizes and inconsistent definitions of overlap, when compared to direct cortical stimulation (DCS), TMS has shown variable sensitivity and specificity. Furthermore, performance of TMS in pediatric cohort is not well understood. This study aimed to standardize the TMS-DCS overlap criteria using a Euclidean distance method and then document the efficacy of TMS in pediatric and adult patients. Using this approach, we evaluated the statistical performance of TMS langua…

Prospective open-label trial of personalised connectivity-guided transcranial magnetic stimulation therapy for migraine.

BACKGROUND: Migraine is a leading cause of disability worldwide, affecting hundreds of millions of individuals. Repetitive transcranial magnetic stimulation (rTMS) offers a non-invasive therapeutic option, yet long-term effects remain unclear. We investigated whether rTMS treatment effects could be extended by personalising stimulation targets based on connectivity between the pregenual cingulate cortex (PGC) and dorsolateral prefrontal cortex (DLPFC)-a neural pathway implicated in migraine pathophysiology. FINDINGS: Twenty-one patients completed all treatments and assessments. Clinical data w…

Repetitive Transcranial Magnetic Stimulation in Migraine: Clinical Outcomes and Neurobiological Mechanisms-A Systematic Review.

BACKGROUND: Migraine is a highly prevalent neurological disorder associated with substantial disability and socioeconomic burden. Although pharmacological therapies remain the mainstay of treatment, their effectiveness may be limited by incomplete response and adverse effects. Repetitive transcranial magnetic stimulation (rTMS) has emerged as a non-invasive neuromodulatory technique that may modulate cortical excitability and pain-processing networks involved in migraine pathophysiology. This systematic review aimed to evaluate the current evidence regarding the efficacy and safety of rTMS com…

The safety of ipsilateral high-frequency repetitive transcranial magnetic stimulation in brain tumour patients.

BACKGROUND: Motor deficits following resection of intrinsic brain tumours impair quality-of-life and survival. Repetitive transcranial magnetic stimulation (rTMS) has shown promise for motor rehabilitation. Only contralesional low-frequency stimulation (CL-LFS) has been tested in this setting. Ipsilesional high-frequency stimulation (IL-HFS), effective in stroke recovery, has not been explored in patients with brain tumours due to several safety concerns, namely: seizure risk, wound healing, and implant heating. METHODS: Four patients received IL-HFS and six received standard CL-LFS following…
