Women in Transition: Experiences of Health and Health Care for Recently Incarcerated Women Living in Community Corrections Facilities.

Health priorities of women after incarceration remain poorly understood, constraining development of interventions targeted at their health during that time. We explored the experience of health and health care after incarceration in a focused ethnography of 28 women who had been released from prison or jail within the past year and were living in community corrections facilities. The women's outlook on health was rooted in a newfound core optimism, but this was constrained by their pressing health-related issues; stress and uncertainty; and the pressures of the criminal justice system. These

Transition from child- to adult-orientated care for children with long-term health conditions: a process, not an event.

Long-term health conditions in childhood include both congenital conditions and acquired diseases. Children with long-term health conditions face issues and potential secondary problems that are different from those of adults with chronic diseases. Transition to adult-orientated care for such children and adolescents is a major challenge. Transition needs to be prepared for and planned. A variety of possible transition models exists, depending on circumstances.

Transition of care to adult congenital heart centres: what do patients know about their heart condition?

BACKGROUND: The population of adults with congenital heart disease is growing rapidly, and the transition of care from the pediatric to the adult setting is often not optimal. OBJECTIVES: To assess the level of knowledge that adults with congenital heart disease have of their condition. METHODS: All new patients referred to an adult congenital heart centre were asked to answer questions about their clinical diagnosis, their need and reason for anti-bioprophylaxis, and their risks of pregnancy when applicable. The patient's age at first visit, source of referral and time from last visit in ca

Transition of Rare and/or Complex Conditions From Paediatric to Adult Care: Use of the ITG Form and Outcome Analysis.

INTRODUCTION: Health care transition (HCT) is a coordinated pathway supporting the progression of young adults into adult-oriented health services. A structured pathway is essential to ensure continuity for patients with complex and chronic conditions. This study analyzes the transition model implemented at the City of Health and Science University Hospital of Turin (Italy), a tertiary hospital providing both paediatric and adult care. The study involves a cohort of patients with a wide spectrum of rare and/or complex chronic conditions, focussing on factors influencing successful transitions

Addressing organizational risk factors and interventions during transitions of care for older patients: a systematic umbrella review.

BACKGROUND: The global aging population presents significant challenges for healthcare systems, with projections indicating that by 2050, individuals aged 65 and above will total 1.6 billion. Older adults, often experience chronic conditions and face frequent transitions in care, which can result in fragmented care and increased risks. Communication issues, poor discharge planning, and lack of standardized procedures contribute to patient safety concerns during these transitions. Addressing such risks through effective organizational interventions is crucial for improving patient outcomes. Exi

Top 10 Tips Palliative Care Clinicians Should Know About Navigating Physician Job Transitions.

Most palliative care (PC) physicians will change jobs over the course of their career, yet few supports exist to guide them through this often unfamiliar process. Job transitions frequently elicit stress and uncertainty, as individuals balance personal and professional priorities, navigate complex negotiations, and assume the inherent risks of pursuing new opportunities. Yet, when approached intentionally, a job transition can also be a transformative process that allows for self-discovery, cultivation of new skills and connections, and reshaping of one's professional identity. Transitioning to adolescent and young adult or adult cancer survivorship care: A systematic review of contemporary guidelines and trials.

BACKGROUND: Health service transitions of child, adolescent, and young adult cancer survivors between paediatric and adult cancer care services are pervasively fragmented, resulting in poor coordination, reduced satisfaction, and adverse outcomes for cancer survivors and significant others (e.g., caregivers and family members). Numerous trials and clinical guidelines have been developed to support care transitions, but their convergence, divergence, and methodological quality remain unclear. To advance clinical practice and quality research about care transitions, a synthesis of trials and gui

Evaluation of goal setting and behavior change training for transition care case managers using Kirkpatrick's Model.

BACKGROUND: The transition care program (TCP) is a multidisciplinary, restorative care and rehabilitation program designed to support older adults in regaining functional independence. The integration of goal-setting and behavior change techniques in transition care can help support the uptake of health-related behaviors among older adults to optimize functional and cognitive health outcomes, although standardized education in TCP is not available. PURPOSE: We developed a goal-setting training module for case managers and therapists to address this need and evaluated its efficacy in improving

Clusters of out-of-home care trajectories and precarious school-to-work transitions: A Norwegian national register study.

BACKGROUND: Trajectories through child protection services involving out-of-home care (OHC) are diverse, yet studies of OHC and school-to-work transitions often overlook this heterogeneity. METHODS: Using monthly records for three complete Norwegian birth cohorts (1994-1996; N = 172,638), we identified five OHC trajectory clusters among individuals with OHC before age 18 (N = 4789). These clusters were based on placement timing, duration and type. Individuals without OHC were included as a non-OHC group. We examined how cluster membership, together with care instability, is associated with pr

"Okay, you're moving to the adult world": perspectives on transitioning users of home mechanical ventilation to adult care.

RATIONALE: Children requiring long-term home mechanical ventilation (HMV) increasingly survive into adulthood. However, structured processes to support their transition from pediatric to adult care are lacking. Understanding the perspectives of patients, caregivers, and health and social care providers (HSCP) is critical to improving transition experiences and outcomes. OBJECTIVES: To explore the experiences, perceptions, and priorities of patients, caregivers, and providers involved in the transition of young people using HMV from pediatric to adult care, and to identify practical targets fo

Navigating Transition to Adult Care in Youth-Onset Type 2 Diabetes: Facilitators, Attitudes, Barriers, and Behaviors.

CONTEXT: Transitioning from pediatric to adult-centered diabetes care may be challenging, especially for young individuals at risk for and with youth-onset type 2 diabetes (Y-T2D) who have a high disease burden and rapidly progressive disease. However, the scope of transition readiness, range of psychosocial factors, and perceived barriers among Y-T2D are understudied. OBJECTIVE: In Y-T2D attending an adult diabetes transition clinic, our objectives were to (1) characterize attitudes toward transition readiness, (2) examine relationships among depressive and anxiety-related symptoms and trans

Chinese physicians' experiences with end-of-life communication and hospice care transition: a qualitative study with content analysis.

BACKGROUND: Mainland China's end-of-life care system remained underdeveloped marked by underutilization of hospice care services. Physicians-initiated end-of-life communication is likely to result in immediate enrollment in hospice care. However, there is a lack of empirical evidence on how physicians in China undertake end-of-life communication in advanced cancer. This qualitative study aimed to explore physicians' experiences on discussing end-of-life matters and facilitating hospice care transition within families of patients with advanced cancer. METHODS: A descriptive qualitative study w

Evaluation of a Pediatric-Adult Dyad Care Model for Transitioning Youth With Childhood-Onset Systemic Lupus Erythematosus.

OBJECTIVE: Only 50% of pediatric rheumatology patients transition successfully to adult care. We developed a pediatric-adult provider dyad transition model for youth with childhood-onset systemic lupus erythematosus (cSLE). This model transitions patients from the SickKids Transition Clinic (STC) to the Young Adult SLE (YASLE) Clinic at Mount Sinai Hospital (MSH) at age 18, where they receive combined pediatric-adult care for 4 years before transitioning to adult care. We aimed to evaluate the success of this transition model. METHODS: A retrospective chart review was conducted for patients w
