Examination of life-threatening injuries in 431 pediatric facial fractures at a level 1 trauma center.

PURPOSE: Pediatric facial fractures represent a challenge in management due to the unique nature of the growing facial skeleton. Oftentimes, more conservative measures are favored to avoid rigid internal fixation and disruption of blood supply to the bone and soft tissues. In addition, the great force required to fracture bones of the facial skeleton often produces concomitant injuries that present a management priority. The purpose of this study was to examine a level 1 trauma center's experience with pediatric facial trauma resulting in fractures of the underlying skeleton with regards to ep

Impact of catastrophic brain injury guidelines on donor management goals at a level I trauma center.

BACKGROUND: An organ procurement organization (OPO) and a level I trauma center developed catastrophic brain injury guidelines (CBIGs) to assist in the care of severely brain-injured adult patients before brain death. The CBIGs provided a set of clinical guidelines to maintain patient stability and optimize opportunity for organ donation. OBJECTIVE: The aim of this study was to determine if the use of the CBIGs affected the OPO's ability to achieve donor management goals (DMGs) before organ recovery. METHODS: We conducted a retrospective analysis comparing the number of DMGs met in the hospi

Increased consumption of hospital resources due to complications: an assessment of costs in a level I trauma center.

BACKGROUND: Hospital complications can pose a threat to patients, contribute to higher mortality and morbidity, and increase both the average length of hospital stay (LOS) and the use of other resources. The purpose of this study is to express the relationship between complications and the use of hospital resources in financial parameters. METHODS: All trauma patients admitted to the surgical ward in the period 2000 to 2008 were analyzed (n = 4,377). All activities registered during admission were obtained. The integral in-hospital cost prices of each activity were divided into various produc

Experience of using local flaps to cover open lower limb injuries at an Indian trauma center.

BACKGROUND: Optimal care of open, high-velocity, lower limb injury requires surgical skills in debridement, skeletal stabilization, and in providing appropriate soft tissue cover. Timely coordination between orthopedic and plastic surgeons, though ideal, is often difficult. In our center, orthopedic surgeons undertake comprehensive treatment of open fractures including soft tissue cover. We reviewed the results of the local flaps of lower limb, done by orthopedic surgeons. MATERIALS AND METHODS: We retrospectively reviewed the results of the lower limb flaps done between January 2005 and Dece

Trauma center brief interventions for alcohol disorders decrease subsequent driving under the influence arrests.

BACKGROUND: A substantial number of trauma center admissions are related to driving under the influence (DUI); however, there has been no prior report of brief intervention (BI) after injury reducing subsequent DUI arrests. The hypothesis of this study was that injured patients receiving BI would have a lower risk of DUI arrest within 3 years of discharge than those receiving standard care (SC). METHODS: This prospective, randomized clinical trial randomly allocated patients involved in motor vehicle collisions to receive SC or a BI regarding alcohol use. The primary outcome measure was DUI a

Antiplatelet Reversal Is Not Associated With Decreased Progression of Intracranial Hemorrhage in Near-Isolated Traumatic Brain Injury: A Retrospective Clustered Analysis From Two Trauma Centers.

INTRODUCTION: Antiplatelet agents (AAs) may increase the risk of intracranial hemorrhage (ICH). It is unclear whether reversal of antiplatelet effects (REV = desmopressin acetate [DDAVP] + Platelets) decreases ICH progression. The goal of the study was to determine whether REV was associated with decreased progression of ICH on repeat brain computed tomography (CT) scan. METHODS: This is a clustered study (November 2019 to March 2022) at two regionally distinct trauma centers (TCs) with differing standards of practice in patients with ICH, one reversal with DDAVP + Platelets (REV+) and the ot…

The relationships of skeletal injuries with trauma score, injury severity score, length of hospital stay, hospital charges, and mortality in children admitted to a regional pediatric trauma center.

Thirty-four-hundred and seventy-two children were consecutively admitted for acute traumatic injuries over a 34 month period to the Children's National Medical Center. The study comprised 805 patients who sustained 953 fractures and dislocations. The male to female ratio was 2:1. Age at the time of admission was evenly distributed over 16 years, with a mean of 8.3 years. Pedestrian accidents and falls each accounted for 34% of the musculoskeletal injuries, whereas motor vehicle accidents accounted for an additional 13%. The femur was the most commonly fractured bone, representing 22% of all fr
