Urological injuries following trauma.

Blunt renal trauma is the third most common injury in abdominal trauma following splenic and hepatic injuries, respectively. In the majority, such injuries are associated with other abdominal organ injuries. As urological injuries are not usually life-threatening, and clinical signs and symptoms are non-specific, diagnosis is often delayed. We present a practical approach to the diagnosis and management of these injuries based on our experience in a busy inner city trauma hospital with a review of the current evidence-based practice. Diagnostic imaging signs are illustrated.

A comparative study on the predictive value of digital subtraction angiography and B-mode ultrasonography in evaluating arterial injury in high-voltage electrical burn of the forearm.

To compare the predictive value of digital subtraction angiography (DSA) with B-mode ultrasonography in evaluating the arterial injury in high-voltage electrical burn of the forearm, 19 forearms sustaining high-voltage electric burn were examined with DSA and B-mode ultrasonography. During surgery, the involved arteries were examined closely to compare their gross pathology with the results of DSA and B-mode ultrasonography. The thrombotic or necrotic arteries found during surgery were excised and examined pathologically. Among 19 ulnar and radial arteries examined by DSA and B-mode ultrasonog

[Primary care and assessment of extended burn].

Extended burn is a severe trauma makes patients easily critical status without appropriate cure and assessment. In order to ride out hypovolemic shock, it is necessary to manage the internal water, the state of respiration and wound treatment and assess the depth of burn wound. Initialy we assess the burned area and it's depth, then we calculate the amount of water which will be necessary on acute phase for the patient We will resect the part of deep dermal burn immediately to reduce the causes of infection. The patient will need a graft operation as soon as we can to avoid the severe infectio

Systematic bias in traumatic brain injury outcome studies because of loss to follow-up.

OBJECTIVE: To identify potential sources of selection bias created by subjects lost to follow-up in studies of traumatic brain injury (TBI). DESIGN: Demographic, premorbid, injury-related, and hospital course characteristics were compared for subjects lost and found for 1- and 2-year postinjury follow-ups by using bivariate tests and logistic regression analysis. SETTING: Three prospective, longitudinal data sets-a single center, a multicenter, and a statewide incidence surveillance system and follow-up registry. PARTICIPANTS: Adolescents and adults hospitalized with a diagnosis of TBI. IN

The neurogenic bladder in spinal cord injury--pattern and management.

This study describes the various types of neurogenic bladder in spinal cord injury in relation to the level of lesion, defines the aims of bladder management, and discusses the importance of highly individualised management strategies and long-term follow-up. Urodynamic studies were done on 47 new patients with traumatic spinal cord injury when they had return of reflexic bladder activity. This study was conducted over a one-year period. Fifty-five per cent (n = 26) sustained cervical injuries (38.5% complete, 61.5% incomplete), 12.8% (n = 6) had thoracic injuries, 29.8% (n = 14) had lumbar in

Comparison between acute and delayed onset major depression after spinal cord injury.

Sixty patients with spinal cord injury were examined to assess major depression during the in-hospital period and at 3- and 6-month follow-up. Thirteen patients had depression during the initial in-hospital evaluation (acute onset depression) and eight had depression first diagnosed at either 3- or 6-month follow-up (delayed onset depression). Acute onset depression was related to the severity of impairment and premorbid history of psychiatric disorder, suggesting a psychological reaction to impairment or premorbid vulnerability as a possible mechanism for developing depression. Delayed onset

[Cardiorespiratory management of severe injured patients in intensive care units].

Late mortality of severely injured patients could be prevented by the quality of early cardiorespiratory management. Indeed traumatized patients with high risk of multiple organ failure (according to age, ISS, amount of blood transfused, and/or metabolic acidosis) need a pulmonary artery catheterization as soon as possible (postdefinitive phase: during the surgical period or at the admission in the ICU). Such a procedure allows the intensivist to determine therapeutic goals in term of O2 delivery (DO2) and O2 uptake (VO2) in front of a frequent increased peripheral O2 demand, These goals (usua

Measurement invariance of a Posttraumatic Stress Disorder symptoms measure (PCL-5) in college student and Amazon's mechanical TURK samples.

OBJECTIVE: College student and Amazon's Mechanical TURK (MTURK) samples are regularly utilized in trauma research. Recent literature, however, has criticized these samples for not being generalizable to the general U.S. POPULATION: The purpose of this study was to determine whether college student (n = 255) and MTURK (n = 316) samples are invariant on the Posttraumatic Stress Disorder Checklist for DSM-5. METHOD: Measurement invariance using confirmatory factor analyses was used to determine whether groups are invariant across factor structure, factor loadings, item intercepts, and residual

Structural Connectivity of the Posterior Cingulum Is Related to Reexperiencing Symptoms in Posttraumatic Stress Disorder.

Posttraumatic stress disorder (PTSD) is a heterogeneous disorder with disturbances in hyper-arousal or avoidance behaviors, and intrusive or re-experiencing thoughts. The uncinate fasciculus (UF) and cingulum bundle are white matter pathways implicated in stress and trauma pathophysiology, yet their structural integrity related to PTSD symptom domains is yet to be understood. Forty-four trauma-exposed young adults underwent structural and diffusion-weighted MRI. Stress and trauma exposure indices and severity of PTSD symptoms were collected and used to predict current integrity of the UF and c

International consensus statement: methods for recording and reporting of epidemiological data on injuries and illnesses in golf.

Epidemiological studies of injury in elite and recreational golfers have lacked consistency in methods and definitions employed and this limits comparison of results across studies. In their sports-generic statement, the Consensus Group recruited by the IOC (2020) called for sport-specific consensus statements. On invitation by International Golf Federation, a group of international experts in sport and exercise medicine, golf research and sports injury/illness epidemiology was selected to prepare a golf-specific consensus statement. Methodological stages included literature review and initial

Revision of the medicolegal assessment criteria in hypovolemic patients.

Trauma scoring systems are often used for the determination of the severity level of the lesion and the clinical status in medico-legal assessment of the trauma patient. Trauma scoring systems are used also for the determination of the life-threatening conditions. Blood loss of more than 20% was reported as the only criterion for life-threatening conditions in the acute hypovolemia. The objective of this study was to revise the medico-legal assessment criteria in the patients with acute hypovolemia and to discuss other parameters, which might be used in the determination of the severity level

Predictive energy equations are inaccurate for determining energy expenditure in adult burn injury: a retrospective observational study.

BACKGROUND: Severe burn injuries are associated with hypermetabolism. This study aimed to compare the measured energy expenditure (mEE) with predicted energy requirements (pERs), and to correlate energy expenditure (EE) with clinical parameters in adults with severe burn injury. METHODS: Data were retrospectively analysed on 29 burn patients (median (interquartile range) age: 46 (28-61) years, % total body surface area burn: 37% (18-46%)) admitted to an intensive care unit. Indirect calorimetry was performed on 1-4 occasions per patient to measure EE. mEE was compared with pER calculated usin

Novel transcatheter arterial embolization method for hemodynamically unstable pelvic fractures to prevent complications of gluteal necrosis.

PURPOSE: To validate our previously designed transcatheter arterial embolization (TAE) technique for bilateral iliac arteries in unstable pelvic fractures, which is designed to also prevent gluteal necrosis and avoid vasopressors. METHODS: We retrospectively analyzed the data of patients with pelvic fractures who underwent our new TAE procedure to determine the incidence of subsequent gluteal necrosis. We also compared certain variables between patients who underwent TAE before 2005 using a different technique and developed gluteal necrosis and patients who underwent TAE in 2005 and onward us
