RATIONALE: Hemangioma is a common benign disease in clinical practice, but it is rare to find a giant hemangioma in the fetal period.
PATIENT CONCERNS: Here, we report a case of a giant hemangioma of the fetal head and neck measuring approximately 10.1 × 6.5 cm.
DIAGNOSES: At first, only ultrasonography was used to diagnose the suspected hemangioma. The pregnant woman refused to undergo further testing and requested induction of labor, after which the tumor was finally sent for pathological examination to confirm hemangioma.

The Psychiatric Consultation Service at Massachusetts General Hospital sees medical and surgical inpatients with comorbid psychiatric symptoms and conditions. During their twice-weekly rounds, Dr Stern and other members of the Consultation Service discuss diagnosis and management of hospitalized patients with complex medical or surgical problems who also demonstrate psychiatric symptoms or conditions. These discussions have given rise to rounds reports that will prove useful for clinicians practicing at the interface of medicine and psychiatry.

Impairment of medical decision-making capacity is common in acutely ill patients especially those with solid malignancies. CONCLUSION: Percutaneous steroid injection (PSI) may be a useful alternative modality for treatment of benign vocal fold lesions.
OBJECTIVES: When patients refuse general anesthesia or voice therapy for benign vocal fold diseases (Reinke's edema, vocal polyp, nodule, and scarring), there are no other options available. We conducted an analysis of the effects of PSI as an alternative treatment for benign vocal fold diseases.

OBJECTIVE: To evaluate the reasons why patients reject digital rectal examination (DRE) when screening for prostate cancer.
METHODS: Four hundred and fifty men were prospectively evaluated in a prostate cancer educational program consisting of lectures, PSA testing, and DRE. Patients rejecting DRE were compared with those accepting DRE in regard to epidemic, social and cultural variables.

The transplant patient's therapeutic regimen consists of a lifelong drug therapy, including immunosuppressive drugs, prophylactic antimicrobials and often medications for the treatment of hypertension, diabetes mellitus and other comorbid diseases. The ventilatory support of patients may involve difficult decisions and complex discussions, to allow patients and their families and carers to make truly autonomous decisions. A case is presented of a patient who asked for invasive ventilation to be withdrawn and the problems for all involved--patient, family and professionals--are explored. The need for careful forward planning and discussion is emphasised so that decisions can be made after careful consideration and not in an emergency situation.
