Factors associated with sexually transmitted diseases amongst female prison inmates in Peru.

OBTECTIVES: The number of women in prison is rapidly increasing every year and it is important to establish what factors are related to the probability of presenting STDs in this population to enable health control policies to be established. MATERIAL AND METHOD: An analysis was carried out on the open database of the national prison population survey conducted in 2016 in all Peruvian prisons. The study was based on the census and all female and male adults in prison were surveyed using a validated questionnaire. RESULTS: The sample consisted of 4,574 inmates in 67 institutions, covering 98.

Drug treatment for tuberculosis during pregnancy: safety considerations.

Untreated tuberculosis in pregnancy poses a significant threat to the mother, fetus and family. Adherence to treatment is especially difficult in pregnancy because of the general fear of any medication and pregnancy-related nausea. Supervised treatment is especially helpful in encouraging adherence. All 4 first line drugs [isoniazid, rifampicin (rifampin), ethambutol and pyrazinamide] have an excellent safety record in pregnancy and are not associated with human fetal malformations. Drug-induced hepatitis, especially with isoniazid, is a significant problem in treating tuberculosis, not peculi

A clinicopathological cohort study of liver pathology in 301 patients with human immunodeficiency virus/acquired immune deficiency syndrome.

UNLABELLED: Liver disease complicates human immunodeficiency virus (HIV)/acquired immune deficiency syndrome; however, liver pathology data are limited, particularly from high HIV prevalence countries. We investigated the spectrum and clinicopathological correlates of liver pathology in a high HIV burden setting. In a single-center study, all HIV/acquired immune deficiency syndrome patients with complete clinical and demographic data who underwent liver biopsy were analyzed and clinicopathologically assessed by hepatologists and one of two experienced liver pathologists. We evaluated 301 patie

More than Skin Deep: Primary Inoculation with Mycobacterium tuberculosis as an Occupational Hazard.

BACKGROUND: Sharps injuries pose a significant occupational risk for healthcare and laboratory workers, with hundreds of thousands of incidents reported annually. While protocols for blood-borne pathogens like HIV and hepatitis viruses are well-established, the risk of traumatic inoculation with Mycobacterium tuberculosis (M. tuberculosis) is under-recognized. METHODS: This case series reviews six separate instances of healthcare and laboratory personnel across multiple academic health systems in a northeastern U.S. metropolitan region who developed cutaneous tuberculosis following sharps inj

Para-aminosalicylic acid-induced hypoglycaemia in a patient with diabetic nephropathy.

A 62-year-old Indian with diabetic nephropathy controlled with metformin, developed miliary tuberculosis for which he was treated with rifampicin, isoniazid and ethambutol. Soon afterwards he developed cholestatic hepatitis and visual disturbance. Rifampicin and ethambutol were stopped. Streptomycin caused vertigo and had to be stopped. The introduction of para-aminosalicylic acid (PAS) led to hypoglycaemic coma. Metformin was stopped. Hypoglycaemic coma recurred. PAS was stopped and the patient's blood glucose concentrations became normal. Treatment with isoniazid and ethambutol led to total

Diagnostic imaging of abdominal tuberculosis: gastrointestinal tract, peritoneum, lymph nodes.

Abdominal tuberculosis is usually associated to pulmonary tuberculosis. The most frequent gastrointestinal form is ulcerative illeocecal tuberculosis. Esophageal, gastric and duodenal locations are rare; jejunal and proximal ileal tuberculosis are rare during tuberculous peritonitis. The most common form with liver and spleen involvement is miliary tuberculosis, the macronodular form being rare. Tuberculous peritonitis is a common manifestation of abdominal tuberculosis with various aspects according to the presence of ascitis and the predominant fibrotic reaction. Most common abdominal manife

Tuberculosis on two fronts: A case report of liver and stomach involvement in a patient.

INTRODUCTION: Tuberculosis (TB) is a chronic disease caused by Mycobacterium tuberculosis, primarily affecting the lungs but can also affect other organs. In 2021, 10.6 million people became ill with TB, and 1.6 million died. Abdominal TB is rare, accounting for 1-3 % of cases, and often involves the ileocecal region. Stomach and duodenal TB are uncommon and challenging to diagnose as they mimic other conditions. Hepatic TB is rarer, accounting for < 1 % of TB cases, and has non-specific symptoms such as fever, hepatomegaly, and weight loss. CASE PRESENTATION: A 26-year-old female healthcare

Evaluation and Management of Occupational Exposures and Postexposure Prophylaxis.

Health care personnel (HCP) are at risk of exposure to infectious agents depending on their job duties and other factors. Risks include percutaneous exposure to blood-borne pathogens via sharp injuries (eg, human immunodeficiency virus, hepatitis B virus, hepatitis C virus); exposure by direct skin contact (eg, syphilis); droplet and airborne transmission of pathogens through direct patient care (eg, pertussis, measles, invasive meningococcus infections, tuberculosis); and through indirect contact transmission related to the contaminated health care environment (eg, norovirus).

Sarcoidosis with Hepatic Involvement: A Case Report.

Sarcoidosis is a systemic, noncaseating granulomatous disease with an unknown etiology. The liver is one of the most frequently affected organs. This case is presented to emphasize that hepatic granulomatous foci can lead to a determination of etiology in a diagnosis of sarcoidosis. A 53-year-old-female patient with complaints of fatigue and abdominal pain was admitted to the clinic of gastroenterology. The blood levels of alanine transaminase, aspartate transaminase, alkaline phosphatase and gamma-glutamyl transferase were markedly increased. The autoimmune parameters were negative. A Tru-Cut

[Fever of unknown origin with granulomatous hepatitis, uveitis and acute renal failure].

A 46-year-old man was referred to our hospital because of fever of unknown origin (FUO). Two months before admission, he noted fever, weight loss, and asthenia. Physical examination revealed only mechanical pains of the right shoulder. In the past, patient's father had a tuberculosis. At the age of 29 years, Brucella granulomatous hepatitis and B hepatitis were diagnosed. The past three years, he developed two episodes of uveitis which resolved with local treatment. The hepatic biopsy revealed only a non specific granulomatous hepatitis. Few days after admission, the patient developed acute re

Integration of hepatitis B immunization in the Expanded Program on Immunization of the Child Survival Project.

The Expanded Program on Immunization (EPI) is a component of the Child Survival Project (CSP) whose objectives are to reduce the incidence rates of six childhood diseases (Measles, Diphtheria, Pertussis, Tetanus, Tuberculosis, Poliomyelitis) and to reduce the number of infant deaths from those diseases by increasing effective vaccination coverage. In 1991, the CSP/EPI developed a national plan to introduce national immunization of infants against hepatitis B in an attempt to control the magnitude and seriousness of the damage which viral hepatitis causes in terms of morbidity, mortality and se

Lesions located at the liver periphery: A stepwise cross-sectional imaging approach toward diagnosis.

Lesions at the liver periphery may originate from intrahepatic or extrahepatic tissues due to the unique anatomical, peritoneal, and hemodynamic characteristics of the hepatic capsular and subcapsular regions. In this article, the anatomical and physiological factors predisposing the hepatic periphery to pathology are examined, including Laennec's capsule, perihepatic ligamentous connections, and alternative blood supply pathways. A stepwise imaging approach is outlined, emphasizing the role of computed tomography (CT) and magnetic resonance imaging (MRI) in lesion differentiation based on key

High prevalence of latent tuberculosis and bloodborne virus infection in a homeless population.

INTRODUCTION: Urban homeless populations in the UK have been shown to have high rates of active tuberculosis, but less is known about the prevalence of latent tuberculosis infection (LTBI). This study aimed to estimate the prevalence of LTBI among individuals using homeless hostels in London. METHODS: We performed a cross-sectional survey with outcome follow-up in homeless hostels in London. Our primary outcome was prevalence of LTBI. Recruitment for the study took place between May 2011 and June 2013. To estimate an LTBI prevalence of 10% with 95% CIs between 8% and 13%, we required 500 part

Is metformin poised for a second career as an antimicrobial?

Metformin, a widely used antihyperglycaemic, has a good safety profile, reasonably manageable side-effects, is inexpensive, and causes a desirable amount of weight loss. In 4 studies of patients with tuberculosis (1 prospective and 3 retrospective), metformin administration resulted in better outcomes. In mice with several models of endotoxemia, metformin diminished levels of proinflammatory cytokines and improved survival. Laboratory studies showed effectiveness of the drug on multiple pathogens, including Trichinella spiralis, Staphylococcus aureus, Pseudomonas aeruginosa, hepatitis B virus,
