We evaluated soluble interleukin-2 receptors (sIL-2R), neopterin and adenosine deaminase (ADA) in pleural effusions from 93 patients with tuberculosis, malignancies, uremia, pneumonia and other kinds of pleurisy. There were significantly elevated ADA (102.7 +/- 47 U/l) and sIL-2R (8,238 +/- 4,117 U/ml) values in tuberculous (TB) pleural fluids as compared with other non-TB pleural fluids (p < 0.005). The neopterin levels in pleural fluid were significantly lower in the cancer group (17.3 +/- 7.8 nmol/l; p < 0.005) and most strikingly elevated (309.4 +/- 112.2 nmol/l; p < 0.0001) in patients wi

The relationships between pleural fluid pH and PO2 to pleural fluid PO2, amylase, protein, glucose, white cell count were examined in 110 cases of tuberculous and 140 cases of malignant pleural effusions. Pleural fluid amylase concentrations of 200 or more units/ml indicated malignant effusions rather than tuberculous effusions with a specificity of 92.7 per cent and sensitivity of 27.1 per cent. There was evidence which suggested that the increasing pleural fluid amylase concentrations were due to the excessive leakage of serum amylase into the pleural cavity secondary to the break-down of ca

BACKGROUND: Tuberculous pleural effusion (TPE) is a paucibacillary manifestation of tuberculosis, so isolation of Mycobacterium tuberculosis is difficult, biomarkers being an alternative for diagnosis. Adenosine deaminase (ADA) is the most cost-effective pleural fluid marker and is routinely used in high prevalence settings, whereas its value is questioned in areas with low prevalence. The lymphocyte proportion (LP) is known to increase the specificity of ADA for this diagnosis. We analyse the diagnostic usefulness of ADA alone and the combination of ADA ≥ 40 U/l (ADA(40)) and LP ≥ 50% (LP(50)

OBJECTIVE: To examine the role of naive and memory T cells (as determined by CD45 isoforms) in recent tuberculosis infection and in pulmonary and pleural forms to the disease. To relate such subpopulations to serum levels of soluble IL-2 receptor (sIL-2R) and interleukin-6 (IL-6).
METHODS: T cell subpopulations (CD3, CD4, CD8), naive (CD45RA), memory (CD45RO) and activated CD24+ T cells (IL-2 receptor) were measured in the peripheral blood and pleura of patients with recent tuberculosis infection (n = 7), pulmonary tuberculosis (n = 22) and tuberculosis pleurisy (n = 12). Serum levels of sIL-

BACKGROUND: Pulmonary tuberculosis is a major public health issue worldwide and is frequently overlooked, particularly in our part of the world. More caution with regard to symptoms could facilitate faster diagnosis, better treatment, and decreased morbidity and mortality.
CASE PRESENTATION: We describe the disease course in a middle-aged man presenting with non-productive cough, right-sided pleural effusion and fatigue. Approximately 3 months passed before he was diagnosed with pulmonary tuberculosis and pleuritis. Because of his mild symptoms and non-specific radiographic findings, he was i

How detection of patients with tuberculous pleurisy is organized in the Voronezh Region in the period of 1979 to 2003 was studied. Tuberculous pleurisy was diagnosed in 2681 (31.73%) of 8450 patients with pleural effusion of diverse etiology. A package of necessary studies was determined for the verification of the diagnosis. It is suggested that it is expedient to establish a pleural pathology unit at a tuberculosis hospital for enhancement of the efficiency of diagnosis and verification of tuberculous pleuritis.

Pleural tuberculosis (TB) is most often treated by a pneumologist. Some cases require operative interventions and may represent a challenge for the thoracic surgeon. There are two specific problems regarding TB pleural effusions: 15-25% of them remain undiagnosed using the conventional methods of diagnosis (imaging, thoracentesis, percutaneous pleural biopsies) and have a tendency for the rapid production of dense adherences and loculations. The authors present their experience in the diagnosis and the treatment of tuberculous pleurisy by means of mini-invasive surgical techniques. The period

223 cases with pulmonary tuberculosis sequelae underwent the examinations of arterial blood gases, pulmonary function or right cardiac catheterization. Especially 86 cases were investigated of pulmonary hemodynamics. We discussed about the findings on chest X-ray film--emphysematous change; fibrosis, bronchiectasis, cavity; atelectasis, pneumonectomy; pleural peel; thoracoplasty--in tuberculosis sequelae to influence pulmonary hypertension, about pulmonary hemodynamics, in the past history, with or without right heart failure, about the difference between tuberculosis sequelae and chronic obst

Secondary spontaneous pneumothorax (SSP), particularly when complicated by a persistent air leak, is an uncommon but serious manifestation of active pulmonary tuberculosis. Such cases often require pleural drainage or surgical intervention. Management becomes significantly more challenging when patients present with extensive pulmonary disease, compromised general condition, and severe comorbidities. We describe a 28-year-old male with bilateral extensive pulmonary tuberculosis, persistent right-sided pneumothorax, and a severe acute flare of chronic hepatitis B. Conventional management, inclu

The use of biomarkers on pleural fluid (PF) specimens may assist the decision-making process and enhance clinical diagnostic pathways. Three paradigmatic examples are heart failure, tuberculosis and, particularly, malignancy. An elevated PF concentration of the amino-terminal fragment of probrain natriuretic peptide (>1500 pg/ml) is a hallmark of acute decompensated heart failure. Adenosine deaminase, interferon-γ and interleukin-27 are three valuable biomarkers for diagnosing tuberculous pleurisy, yet only the first has been firmly established in clinical practice. Diagnostic PF biomarkers fo

Actinomycosis and nocardiosis are uncommon pulmonary infections that share a similar predisposition to involve the pleural space. After establishment of an initial pneumonitis, infection spreads across lung fissures causing pleural thickening, pleural effusions, and empyema. Although less common in the antibiotic era, associated sinus tracts or subcutaneous abscesses may develop adjacent to pleural abnormalities. Both disorders most commonly present as chronic, debilitating illnesses with radiographic manifestations simulating lung cancer or tuberculosis; however, immunocompromised hosts may d

An ELISA was established for the measurement of IgG antibody in human serum to the 30,000-Da native antigen of Mycobacterium tuberculosis and evaluated for its utility in the diagnosis of tuberculosis at the Instituto Nacional de Enfermedades Respiratorias in Mexico City. The test had a sensitivity of 70% in patients with sputum-positive active pulmonary tuberculosis and a specificity in control subjects of 100%. The accuracy of positive prediction was 100% and of negative prediction 93% for patients with pulmonary tuberculosis at this institute. Less favorable test characteristics were obtain

INTRODUCTION: Diagnosing tuberculous pleurisy is important in Japan because it currently has a moderate tuberculosis prevalence. However, physicians often have difficulty making a diagnosis. It was reported that thoracoscopy under local anesthesia is useful for the diagnosis of tuberculous pleurisy, but there are no reports focusing on elderly patients.
METHODS: In this study, the usefulness of thoracoscopy under local anesthesia was evaluated in elderly patients. Among 170 patients who underwent thoracoscopy under local anesthesia at our hospital during 11 years from January 2008 to December

A 62-year-old healthy man presented to our hospital due to a persistent fever of up to 38°C for one week. Thoracic computed tomography showed right pleural effusion with multiple large nodules up to 7 cm in diameter composed of numerous discrete small nodules like fireworks, the so-called "cluster" signs. Some of the large nodules had a hyper-dense portion centrally surrounded by partially discrete small nodules, not as densely assembled, suggestive of the "galaxy" sign. The repeated acid-fast sputum smears and both bronchial washings were all negative for Mycobacterium tuberculosis, but the a

Tuberculosis has made a comeback in recent years. This upsurge has been attributed to factors such as increased immigration and the human immunodeficiency virus epidemic. Primary pulmonary tuberculosis manifests radiologically with parenchymal involvement, lymph node involvement, pleural effusion, and/or miliary disease. In post-primary tuberculosis, the earliest radiological sign is small nodules and branching centrilobular lesions that increase in size and coalesce to form ill-defined patchy consolidations; cavitations are very characteristic of active disease. The aim of this article is to

Closed pleural biopsy is known to be diagnostic in approximately 75% of pleural effusion undiagnosed by thoracocentesis or pleural fluid evaluation. The purpose of this study was to determine the efficacy of closed pleural biopsy in a teaching hospital, Eastern India. The study was undertaken in all patients with exudative pleural effusion who were admitted in the department of chest medicine, Burdwan Medical College and Hospital, Burdwan, during the period from January 2001 to December 2005. Abram's needle was used for closed pleural biopsy in every case. One hundred and thirty-three pleural
