Isoniazid-induced alopecia in isoniazid-monoresistant pulmonary tuberculosis.

INTRODUCTION: Alopecia is a rare adverse reaction during tuberculosis (TB) treatment. Both drug-resistant TB and isoniazid (INH)-induced alopecia have been reported. We present this case because INH monoresistance coexisting with INH-induced alopecia has not previously been described in the literature. CASE: A 22-year-old woman was diagnosed with pulmonary TB and initiated on antituberculosis treatment (ATT). After 1 month, she developed sudden, severe hair loss. INH was suspected as the causative agent and discontinued. Hair regrowth was observed within 1 month. Drug resistance testing reveal

Paradoxical intestinal perforation during treatment for pulmonary tuberculosis.

In managing the complication of tuberculosis (TB), the phenomenon of a paradoxical reaction must be considered when there are worsening existing or new TB lesions after initial positive response to TB medications. This case describes the sudden development of peritonitis in an adult male who was receiving anti-TB drugs for pulmonary TB. Emergent laparotomy revealed abdominal TB with perforated distal ileum and generalised tuberculous deposits over the peritoneum. The perforation site was trimmed and brought out as a loop ileostomy. Histology confirmed the presence of caseating granulomas in th

Invasive Pulmonary Aspergillosis and Tuberculosis after COVID-19.

BACKGROUND: Pulmonary tuberculosis (TB) and invasive aspergillosis are distinct infectious diseases that rarely coexist in the same patient. The concurrent occurrence of these infections presents diagnostic and therapeutic challenges, resulting in complex clinical presentations and potentially adverse outcomes. We present a case report highlighting the diagnostic process, management strategies, and pertinent literature regarding the simultaneous occurrence of pulmonary TB and aspergillosis in a patient following COVID-19 infection. CASE PRESENTATION: A 55-year-old male with a history of occup

Gestational trophoblastic neoplasia with pulmonary embolism mimicking tuberculosis.

Gestational trophoblastic neoplasia (GTN) comprises a group of human neoplastic diseases that derive from fetal trophoblastic tissues. They are proliferative as well as degenerative disorders of placental elements and include complete hydatidiform mole (CHM) or partial hydatidiform mole (PHM) (90%), invasive mole (IM) (5-8%), which could also be metastatic, villous, or villous choriocarcinoma (CC) (1-2%), and placental site trophoblastic tumor (PSTT) (1-2%). We present three cases of GTN, two mimicking tuberculosis radiologically, and all three are associated with pulmonary embolism.

Cluster analysis categorizes five phenotypes of pulmonary tuberculosis.

Tuberculosis (TB) has a heterogeneous phenotype, which makes it challenging to diagnose. Our study aimed to identify TB phenotypes through cluster analysis and compare their initial symptomatic, microbiological and radiographic characteristics. We systemically collected data of notified TB patients notified in Korea and constructed a prospective, observational cohort database. Cluster analysis was performed using K-means clustering, and the variables to be included were determined by correlation network. A total of 4,370 subjects with pulmonary TB were enrolled in the study. Based on the corre

Early dynamics of innate immunity during pulmonary tuberculosis.

Tuberculosis (TB) most frequently affects the lung, with Mycobacterium tuberculosis (Mtb), the etiologic agent of TB, promptly gaining access to lung-resident myeloid cells, notably alveolar macrophages. Historical observational case-contact surveys and recent epidemiological studies report on resistors. These individuals are likely protected against infection by defence mechanisms occurring promptly after bacterial exposure. The early events proceeding within the Mtb-infected lung are critical for the outcome of the infection. Despite the heightened relevance of the first contact between Mtb

Active pulmonary tuberculosis presenting with acute respiratory failure.

Four patients with active pulmonary tuberculosis (PTB) presenting with respiratory failure are reported here. Bronchogenic PTB, simulating an acute febrile illness or diffuse interstitial lung disease with short duration of symptoms, as a cause of acute respiratory failure is less recognized. If diagnosed and treated early, it has good prognosis. Three of the four patients presented here had an acute presentation with fever, dyspnoea, and hypoxemia with diffuse infiltrative lesions on radiography, and the other younger patient presented predominantly with lobar consolidation. These patients pr

Radiographic Features for Predicting Smear-Negative Pulmonary Tuberculosis.

BACKGROUND: Diagnosis of smear-negative pulmonary tuberculosis (TB) in some circumstances remains a challenge to physicians especially those working in limited-resource settings. OBJECTIVE: To investigate and examine radiographic characters as a predictor of true diagnosis of pulmonary tuberculosis in patients with negative sputum smears. MATERIAL AND METHOD: This retrospective cross-sectional study was conducted in patients with smear-negative pulmonary TB at Siriraj Hospital between January 2013 and June 2014. Patients with previous TB treatment, HIV co-infection, significant pleural effus

Oropharyngeal and miliary pulmonary tuberculosis without respiratory symptom.

Tuberculosis is an important cause of morbidity and mortality world-wide. We report the case of a 52-year-old man who presented with a two month history of sore throat without other general or respiratory symptoms. Oral ex- amination revealed ulcerative and granulomatous lesions on the soft palate and tonsils. Histological examination of the lesions showed granulomatous tissue with caseaous necrosis consistent with tuberculosis. A chest x-ray and computed to- mography of the chest showed miliary tuberculosis of both lungs. The oral lesions improved with antituberculous medication by one month.

Countrywide management of pulmonary tuberculosis reverses increasing incidence.

OBJECTIVE: To evaluate the impact of countrywide management of tuberculosis (TB) and the availability of second-line drugs (SLDs) on the notification rates of pulmonary TB (PTB) overall and of multidrug-resistant TB (MDR-TB), taking into account human immunodeficiency virus (HIV) co-infection and the national economy in Estonia. DESIGN: Retrospective analysis of notification rates and treatment outcomes of PTB and MDR-TB during 1998-2006. RESULTS: The annual notification rates of both PTB and MDR-TB decreased significantly, by on average 3.3 (P = 0.007) and 1.7 (P = 0.008) cases per 100,000

Extra pulmonary tuberculosis in children: two years study.

Tuberculosis (TB) is an important health problem in developing countries, with varying clinical presentations depending on the organs/systems involved. To study the spectrum of clinical and paraclinical aspects of extra pulmonary TB in children suffering from pulmonary TB. This study has been carried out on 65 children with tuberculosis, admitted in TB wards of National Research Institute of Tuberculosis and Lung Disease (N.R.I.T.L.D) during 2004-2006. All patients were investigated according to specific diagnostic criteria including; history of contact with TB patient, clinical manifestations

Modern morbidity following pulmonary resection for postprimary tuberculosis.

Between January 1991 and March 1996, a total of 28 patients with postprimary tuberculosis underwent resection for disease progression (n = 8), multidrug resistance or noncompliance to the medical treatment (n = 11), parenchymal sequelae (n = 3), suspected cancer (n = 5), and for the correction of postpneumonectomy bronchopleural fistula and empyema (n = 1). On admission, eight patients presented with sputum positivity (28.6%). Similar to previous series, tubercular predilection for upper lobes was confirmed (21/28, 75%); accordingly, upper lobectomy through an extrapleural approach was the mos

Pulmonary tuberculosis presenting as suspected foreign body aspiration.

Tuberculosis is a disease of protean manifestations, and despite a falling incidence is still prevalent in our society. We present a case of a child whose history and preoperative chest radiograph suggested the presence of a foreign body, but subsequent examination of the material removed at endoscopy revealed a diagnosis of tuberculosis. We suggest that all friable material removed from the lower respiratory tract be submitted for microscopy, culture and histopathology with this diagnosis in mind.
