STUDY DESIGN: Case reports and survey of literature.
OBJECTIVE: Case reports of two women with tuberculosis (TB) of the spine (Pott's disease) presenting with severe back pain and diagnosed as compression fracture are described. Physicians should include Pott's disease in the differential diagnosis when patients present with severe back pain and evidence of vertebral collapse.
SETTING: Ohio, USA METHODS: A review of the literature on the pathogenesis, pathophysiology, clinical presentation, diagnostic methods, treatment and prognosis of spinal TB was conducted.
RESULTS: After initial delay,

Tuberculosis is the commonest cause of spinal infection worldwide (9-46%). Tuberculosis spondylodiscitis causes multifocal thoracic and lumbar spinal disease, and big paraspinal and psoas abscesses. It is more frequent in people under 40 who had previous tuberculosis infection and from countries where the illness is endemic. Clinic is non-specific and sub-acute. We report the clinical case of a 29-year-old patient who suffered from pericardic tuberculosis in her childhood and who presents a bilateral retroperitoneal abscess due to tuberculosis spondylodiscitis. Her clinical debut began with le

Scedosporium boydii spinal infection is rare and often mimics tuberculosis. We report a 54-year-old immunocompetent man with post-traumatic thoracic vertebral infection initially misdiagnosed as spinal tuberculosis. A multi-platform approach combining culture, matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS), and targeted next-generation sequencing (tNGS) identified S. boydii. Surgical debridement and voriconazole resulted in complete recovery. A high index of suspicion and integrated diagnostics are essential to avoid mismanagement.

BACKGROUND: Intramedullary spinal tuberculoma is a rare form of central nervous system tuberculosis. This article describes the successful management of intramedullary spinal tuberculoma in 3 patients who received treatment between 2000 and 2003.
CASE DESCRIPTION: The character of 3 cases was analyzed retrospectively, including clinical manifestation and magnetic resonance imaging findings. All masses were excised totally under microscope. Histopathologic examination revealed tuberculoma. Postoperatively, all patients received a 6 to 9 month course of ATT. The outcome was favorable.

INTRODUCTION: Cervical tuberculous spondylodiscitis is an uncommon but potentially devastating form of spinal tuberculosis. Its rarity, subtle clinical onset, and proximity to critical neurovascular structures make early diagnosis particularly challenging. Delayed recognition can lead to catastrophic outcomes, including spinal instability and neurological compromise. This case report aims to underscore the diagnostic pitfalls, highlight key imaging findings, and outline an effective management approach.
CASE PRESENTATION: A 36-year-old woman presented with a one-year history of progressive ne

Tuberculous spondylitis is a common spinal infection. If surgical intervention is necessary, anterior debridement and anterior fixation are typically performed. However, a minimally invasive surgical strategy under local anesthesia seems rarely implemented. A 68-year-old man presented with severe pain in the left flank. Whole spinal magnetic resonance imaging revealed abnormal intensity of vertebral bodies from T6-9. A bilateral paravertebral abscess extending from T4-10 was suspected. The T7/T8 intervertebral disc was destroyed, but severe vertebral deformity or spinal cord compression was no

Tuberculosis (TB) is a common disease worldwide that is caused by Mycobacterium tuberculosis. Tuberculosis of the spine, also called Pott disease, is the most common site of bony dissemination. Although children are disproportionately affected, spinal TB is nonetheless rare in very young children. Cases involving infants requiring surgical intervention have been previously reported, and they are often associated with greater management challenges given the technical difficulty with instrumentation in very young children. This case involved a 3-year-old girl with TB centered at T-6, who present

This case highlights a rare presentation of disseminated tuberculosis in a child, manifesting as tuberculous meningitis and flaccid paralysis due to spinal tuberculosis. Timely initiation of antitubercular therapy, corticosteroids, and ventriculoperitoneal shunt placement led to full recovery, emphasizing tuberculosis consideration in atypical neurological presentations in unvaccinated children with exposure.

PURPOSE: Spinal tuberculosis, if not promptly treated, can lead to kyphotic deformity, causing persistent neurological abnormalities and discomfort. Spinal cord compression can occur due to ossification of the ligamentum flavum (OLF) at the apex of kyphosis. Traditional surgical interventions, including osteotomy and fixation, pose challenges and risks. We present a case of thoracic myelopathy in a patient with post-tuberculosis kyphosis, successfully treated with biportal endoscopic spinal surgery (BESS).
METHOD: A 73-year-old female with a history of untreated kyphosis presented with walkin

Transverse myelitis typically extends two or less spinal segments, whereas longitudinal extensive transverse myelitis (LETM) extends three or more spinal segments in length and may occasionally span all the segments of the spinal cord. We present a case of spinal tuberculosis presenting with LETM with true lower motor neuron-type flaccid paraplegia.

INTRODUCTION AND IMPORTANCE: Tuberculosis (TB) primarily affects the lungs, but can cause extrapulmonary TB (EPTB), including spinal TB (Pott's disease), which presents with variable symptoms. Tuberculous psoas abscesses, a complication of Pott's disease, are increasingly reported due to improved diagnostics.
CASE PRESENTATION: A 32-year-old male from the Somalia region of Ethiopia presented with a history of chronic lower back pain that had worsened over the preceding month, with lower back swelling, unquantified weight loss, and a slight limp. He also reported intermittent, mild lower quadr

Central nervous system tuberculosis is a rare presentation of active tuberculosis and accounts for about 1% of cases (1). The three clinical categories include meningitis, intracranial tuberculomas, and spinal tuberculous arachnoiditis. We report a case of a young man who presented with active pulmonary tuberculosis in addition to tuberculous meningitis and the presence of numerous intracranial tuberculomas.

Paraplegia occurred in eight of 17 patients with central nervous system tuberculosis. In six of these paraplegia was the presenting feature. Paraplegia may complicate tuberculous meningitis, or vertebral tuberculosis, but it may also occur, as in three of our cases, as a primary localised spinal tuberculous radiculomyelitis. These cases are presented in relation to the concept that paraplegia complicating these forms of tuberculosis is caused by radiculomyelitis.

Tubercular spinal localization is very rare (5%) in paediatric age. We report the unusual case of a child with a history of bacillus Calmette-Guerin vaccination who presented with lymphadenitis in the absence of pulmonary involvement. Despite appropriate anti-tubercular therapy, the patient developed spinal tuberculosis with cord compression. Urgent surgical decompression was performed: laminectomy was done at D3-D5 levels and the higher abscess was then flushed using a catheter, decompressing the cauda equina. Our findings suggest that diagnosis of tuberculosis should be considered even in li

Tuberculosis is a contagious infectious disease caused by Mycobacterium tuberculosis, and is the leading cause of death from a single infectious agent worldwide. Imaging plays an important role in the early diagnosis of central nervous system tuberculosis and may prevent unnecessary morbidity and mortality. This article presents an extensive review of pathogenesis, clinical symptoms, typical and atypical imaging appearances of intracranial and spinal tuberculosis, and advanced imaging of intracranial tuberculosis. Furthermore, we explore central nervous system infection of nontuberculous mycob

Cervical tuberculous spondylitis is a rare and potentially life-threatening manifestation of spinal tuberculosis, accounting for only 3%-5% of all cases of spinal tuberculosis This report describes a case of a 17-year-old male patient who developed tetraparesis 2 weeks prior to hospital admission, after a 6-month record of progressive neck pain. Magnetic Resonance Imaging revealed tuberculous spondylodiscitis involving multiple vertebrae, with a severe spinal cord compression at C2-C3 due to an intraosseous abscess and paravertebral soft tissue mass. The patient underwent anterior cervical cor

BACKGROUND CONTEXT: Pott disease and tuberculosis have been with humans for countless millennia. Before the mid-twentieth century, the treatment of tuberculous spondylitis was primarily supportive and typically resulted in dismal neurological, functional and cosmetic outcomes. The contemporary development of effective antituberculous medications, imaging modalities, anesthesia, operative techniques and spinal instrumentation resulted in quantum improvements in the diagnosis, management and outcome of spinal tuberculosis. With the successful treatment of tuberculosis worldwide, interest in Pott

Immune responses to heat shock or stress proteins are observed in several chronic autoimmune diseases. Such proteins are major antigens of many bacteria, especially mycobacteria. To determine whether immune responses to stress proteins occur in chronic inflammatory diseases of the central nervous system such as multiple sclerosis (MS) we measured proliferative responses of lymphocytes from spinal fluids and bloods of patients with MS and other neurological diseases to a sonicate of M. tuberculosis, an acetone extract of M. tuberculosis, a recombinant 65-kd heat shock protein of M. leprae, and

STUDY DESIGN: This article evaluates how an immature spine responds to anterior débridement surgery (without bone grafting) for spinal tuberculosis during growth and development. Sixty-three patients were studied, 29 of whom were children aged 10 years or less at the time of surgery, whereas the remaining 34 subjects were adults. These patients were the subject of the Medical Research Council Working Party's prospective study, started in Hong Kong in the mid-1960s.
