Rectus Urinoma Leading to Abscess Following Urethral Perforation From Self-Catheterization: A Case Report.

Urinomas are an accumulation of urine in the perirenal or paraureteral space due to urinary tract leakage. Stimulation of an inflammatory response results in the formation of a thick wall that encapsulates the urine. Etiologies of urinomas include trauma, surgery, or spontaneous occurrence. Complications when untreated vary and include peritonitis, fibrosis, abscess formation, and septic shock. We present a 52-year-old male with a neurogenic bladder who developed a rectus urinoma from the thorax to the scrotum. This likely developed from urethral trauma from intermittent self-catheterization.

Onabotulinum toxin A for the treatment of neurogenic detrusor overactivity due to spinal cord injury or multiple sclerosis.

Evidence for the efficacy and safety of intravesical onabotulinum toxin A (onabotA) injections has led to it being licensed in many countries in patients with urinary incontinence due to neurogenic detrusor overactivity resulting from spinal cord injury and multiple sclerosis, who are refractory or intolerant to the gold-standard treatment, anticholinergics. Owing to its mechanism of action, an inhibitory effect on acetylcholine release is obtained for up to 10 months, with a recommended dose of 200 units. Urinary tract infections and postvoid residual volume are the most prevalent side effect

[Current strategies in patients with urinary diversion, I: the lower urinary tract].

Urethral catheterization, intermittent self-catheterization and suprapubic catheters are reliable methods to drain the urine in the lower urinary tract. Only a strict indication for indwelling catheters can minimize the complications and negative side effects. All physicians, especially pediatricians, surgeons, neurologists and internists should be familiar with the problems of catheter drainage by using a strictly aseptic technique.

A comparison of suprapubic and transurethral drainage for postoperative urinary retention in general surgical patients.

Percutaneous suprapubic bladder drainage, using an indwelling Silastic catheter, was compared with transurethral catheterization for the management of postoperative urinary retention in general surgical patients. In a prospective controlled trail, the overall incidence of postoperative retention was 7%. The urinary infection rate was 70% with transurethral catheterization and 8% with suprapubic drainage. Patients accepted suprapubic damage much more readily than transurethral cateterization and both medical and nursing personnel found the former method easier to manage effectively. Also, the p

An Infection-Responsive Approach To Reduce Bacterial Adhesion in Urinary Biomaterials.

Infection is an inevitable consequence of chronic urinary catheterization with associated problems of recurrent catheter encrustation and blockage experienced by approximately 50% of all long-term catheterized patients. In this work, we have exploited, for the first time, the reported pathogen-induced elevation of urine pH as a trigger for "intelligent" antimicrobial release from novel hydrogel drug delivery systems of 2-hydroxyethyl methacrylate and vinyl-functionalized nalidixic acid derivatives, developed as candidate infection-resistant urinary catheter coatings. Demonstrating up to 20-fol…

Lack of effect of methenamine in suppression of, or prophylaxis against, chronic urinary infection.

Methenamine is frequently prescribed for patients who have chronic urinary infection to suppress bacterial growth during active infection or to prevent recurrence once an infection has been brought under control. We have examined the effect of methenamine mandelate and ascorbic acid on bacteriuria in para- and quadriplegics from a spinal cord unit. Patients with indwelling urinary catheters and those on a program of intermittent catheterization were included. No suppressive or prophylactic effect of this regimen was observed in any of our patients. Methenamine does not appear to be an effectiv

Sacral neuromodulation for the treatment of idiopathic non-obstructive urinary retention: A long-term follow-up case report.

BACKGROUND: Sacral neuromodulation (SNM) is a treatment used to manage urinary symptoms of both overactive bladder and non-obstructive urinary retention. The use of SNM in managing these symptoms has yielded positive results, contributing to a reduction in symptoms and an improvement in quality of life (QoL) for patients. This article aims to report on the electrical neurostimulation of sacral nerves to treat idiopathic non-obstructive urinary retention. CASE DESCRIPTION: A 27-year-old female presented with chronic non-obstructive idiopathic urinary retention, using intermittent bladder cathe

Transurethral bladder catheterization of male rhesus macaques: a refinement of approach.

BACKGROUND: Successful transurethral bladder catheterization in male non-human primates can be challenging. An optimized approach for consistent and reproducible catheterization using a refined technique is described. METHODS: Under sedated and non-sedated conditions, transurethral bladder catheterization was performed on 25 male rhesus macaques of varying ages and body weights over time. A refined technique ensuring optimal lubrication of the urethral canal prior to catheter insertion was utilized along with various single and multiple lumen catheters. RESULTS: All animals were successfully

Combinatorial small-molecule therapy prevents uropathogenic Escherichia coli catheter-associated urinary tract infections in mice.

Catheter-associated urinary tract infections (CAUTIs) constitute the majority of nosocomial urinary tract infections (UTIs) and pose significant clinical challenges. These infections are polymicrobial in nature and are often associated with multidrug-resistant pathogens, including uropathogenic Escherichia coli (UPEC). Urinary catheterization elicits major histological and immunological alterations in the bladder that can favor microbial colonization and dissemination in the urinary tract. We report that these biological perturbations impact UPEC pathogenesis and that bacterial reservoirs esta…

Urinary catheters: selection, management and prevention of infection.

Urinary catheterization is a common procedure for both hospital and community patients. Nurses make many of the decisions in regard to both catheter selection and subsequent catheter care. These decisions, for example, in the type of catheterization (intermittent, indwelling, urethral or suprapubic) should be made on an informed basis. Choosing the optimum catheter material and size can benefit catheter care. Encrustation can be a problem for some catheterized patients and nurses are often involved in trying to prevent or treat it. This and other aspects of catheter management including infect…

Postoperative urinary leakage after bilateral totally extraperitoneal herniorrhaphy in a patient with a healed cystostomy and appendectomy: A case report.

We report a case of postoperative urinary leakage after bilateral laparoscopic totally extraperitoneal (TEP) herniorrhaphy. A man in his upper 80s with a healed cystostomy and appendectomy underwent bilateral TEP herniorrhaphy. Urinary leakage was noted by ultrasound examination 4 days after bilateral TEP. Cystography and computed tomography conclusively confirmed a 6-mm extraperitoneal fistula at the site of the previous cystostomy. The fistula involved the anterior bladder wall and was associated with an extended urinoma. The patient was treated by indwelling catheterization using a Foley ca

[Appendicovesicostomy for controlled bypass of urine in children].

Catheterization through appendicovesicostomy (AVS) proposed Mitrofanoff in 1980 is an alternative of periodic catheterization of the urinary bladder via urethra. AVS was used in 30 children (21 boys and 9 girls). Appendix with intact mesentery was cut off the caecum and placed between the urinary bladder or artificial urinary bladder and the skin in the umbilical area. The immediate postoperative period was uneventful--complications were absent. 3 to 6 month follow-up registered obstruction in the created appendicostomas in 7 children. Urinary leakage from the "stoma" occurred in 3 patients. T

Modifications of and extended indications for the Pippi Salle procedure.

Total urinary incontinence is a difficult problem faced by the urologist. Several techniques to increase ureteral resistance have been described. The majority of them rely on intermittent catheterization for bladder emptying, especially in neurogenic incontinence. We have developed a new procedure in which a bladder flap is used to create a neourethra. This urethral extension acts as a flap valve to provide continence. Bladder emptying is accomplished by clean intermittent catheterization. Urethral lengthening with an anterior bladder-wall flap was performed in 18 patients aged a mean of 8.9 y
