Safety of monovalent and bivalent BNT162b2 mRNA COVID-19 vaccine boosters in at-risk populations in Israel: a large-scale, retrospective, self-controlled case series study.

BACKGROUND: COVID-19 continues to be a major health threat, particularly among at-risk groups, including individuals aged 60 years or older and people with particular medical conditions. Nevertheless, the absence of sufficient vaccine safety information is one of the key contributors to vaccine refusal. We aimed to assess the short-term safety profile of the BNT162b2 mRNA COVID-19 vaccine booster doses. METHODS: In this self-controlled case series study, we used a database of members of the largest health-care organisation in Israel. We analysed the medical records of individuals at risk of C

Haemophilus influenzae type B (Hib) necrotizing soft tissue infection (NSTI) in a vaccinated 16-month-old Inuit boy: a case report.

Necrotizing soft tissue infection (NSTI) caused by Haemophilus influenzae type B (Hib) is rare and life-threatening. We report severe monomicrobial Hib NSTI in a 16-month-old Inuit child who had received 3 doses of the Hib vaccine, highlighting vaccine failure and host susceptibility. The child sustained a minor fall to the left thigh without skin breakdown and developed fever the same day, initially treated with amoxicillin for presumed acute otitis media. He re-presented with persistent fever, progressive leg pain, swelling, ecchymosis, and refusal to weight-bear, prompting air transfer to t

Altruism, Scepticism, and collective decision-making in foreign-born U.S. residents in a tuberculosis vaccine trial.

BACKGROUND: The current vaccine against tuberculosis, BCG, is effective when given in most TB-endemic countries at birth but has diminished efficacy against pulmonary TB after 15-20 years. As a result, new booster vaccines for adolescents and adults are being developed to realize the World Health Organization target of global elimination of TB by 2035. Multiple TB candidates thus are in active clinical development. METHODS: One of these, DAR-901, is advancing in human clinical trials. These clinical trials are conducted in BCG immunized adults with and without HIV infection in order to assess

Mandatory vaccinations in European countries, undocumented information, false news and the impact on vaccination uptake: the position of the Italian pediatric society.

BACKGROUND: High rates of vaccination coverage are important in preventing infectious diseases. Enforcing mandatory vaccinations is one of the strategies that some Countries adopted to protect the community when vaccination coverage is not satisfactory. In Italy, in 2017 vaccination against diphtheria, tetanus, pertussis, hepatitis B, poliovirus, Haemophilus influenzae type b, measles, mumps, rubella and varicella became compulsory in childhood. In order to contrast vaccination policies, anti-vaccination campaigns contribute to the spread of fake news. Among them, there is the false informatio

Measles challenges: global resurgence and factors affecting eradication.

Measles is one of the most highly contagious and inflictive human viral infections, and approximately 90% of exposed individuals become infected following the exposure. The infection is communicable from one day before the prodromal symptom onset until four days after rash onset. Up to 40% of people who attract measles will have other health problems arising during the course of infection. There is no specific treatment for measles. Recent reports highlight that measles infection erases the already existing immune memory of various pathogens. The latest measles outbreaks in sub-Saharan Africa

Major motives in non-acceptance of A/H1N1 flu vaccination: the weight of rational assessment.

Recent efforts of health authorities to promote vaccination against influenza A/H1N1 were met with low compliance rates in most industrialized countries. Here we analyzed the attitudes of the Israeli public towards A/H1N1 vaccination based on a telephone survey conducted several months after the peak of the outbreak. The findings attest to the low uptake of the A/H1N1 vaccine (17%) in Israel, and identify the socio-demographic characteristics associated with non-compliance. In addition, the survey reveals passiveness, fear and distrust as motives leading to non-compliance. Most importantly, th

Disrespectful care in family planning services among youth and adult simulated clients in public sector facilities in Malawi.

BACKGROUND: Provision of high-quality family planning (FP) services improves access to contraceptives. Negative experiences in maternal health have been documented worldwide and likely occur in other services including FP. This study aims to quantify disrespectful care for adult and adolescent women accessing FP in Malawi. METHODS: We used simulated clients (SCs) to measure disrespectful care in a census of public facilities in six districts of Malawi in 2018. SCs visited one provider in each of the 112 facilities: two SCs visits (one adult and one adolescent case scenario) or 224 SC visits t

High-Stakes Treatment Negotiations Gone Awry: The Importance of Interactions for Understanding Treatment Advocacy and Patient Resistance.

Doctors (and sociologists) have a long history of struggling to understand why patients seek medical help yet resist treatment recommendations. Explanations for resistance have pointed to macrostructural changes, such as the rise of the engaged patient or decline of physician authority. Rather than assuming that concepts such as resistance, authority, or engagement are exogenous phenomena transmitted via conversational conduits, we examine how they are dynamically co-constituted interactionally. Using conversation analysis to analyze a videotaped interaction of an oncology patient resisting th

Transmission potential of smallpox: estimates based on detailed data from an outbreak.

Recent discussions on the use of variola virus by bioterrorists have rekindled interest in the parameters that govern the transmissibility of smallpox. Here, the authors estimate by maximum likelihood the parameters of the spread of smallpox from historical data on an epidemic in 1967 in the town of Abakaliki, Nigeria, afflicting a religious group that refused vaccination. According to the authors' estimates, 79.9% (95% confidence interval (CI): 63.6, 87.9) of the infectious contacts occurred within the compounds of the cases and 93.3% (95% CI: 80.6, 98.8) among compound members and other clos

Immunization history of children with inflammatory bowel disease.

BACKGROUND: Protection against vaccine-preventable diseases is important in children with inflammatory bowel disease (IBD) due to frequent immunosuppressive therapy use. The chronic relapsing nature and treatment regimen of IBD may necessitate modified timing of immunizations. OBJECTIVE: To evaluate the completeness of immunizations in children with IBD. METHODS: Immunization records of all children with IBD followed at the Alberta Children's Hospital (Calgary, Alberta) were reviewed. For children with incomplete immunization according to the province of Alberta schedule, the reasons for suc

Exploring communication strategies to use with parents on childhood immunisation.

While childhood immunisations are voluntary in the UK, healthcare staff strongly encourage uptake; this is endorsed by the Department of Health. While a few parents refuse immunisation outright, many more are uncertain about the risks and benefits. This uncertainty was exacerbated during the controversy over the mumps, measles and rubella vaccine. This prompted many studies exploring parents' views, which reported considerable criticism of the approach of healthcare professionals. The National Institute for Health and Clinical Excellence (2009) published guidance on reducing differences in upt
