Between 1973 and 1976, 153 patients (124 men, 29 women) with uncomplicated, chronic, duodenal ulcer were entered into a prospective randomized trial of highly selective vagotomy (HSV) or truncal vagotomy and pyloroplasty (TVP). Following the evaluation of the result of SPV in rats by determination of reduction of acid production the effect of the same operation on gastric motility was compared to that obtained with both SPV and a simultaneous pyloroplasty in a one year study. As demonstrated radiologically, with SPV alone gastric stasis resulted in a greatly enlarged stomach with a high percentage of peptic ulcers. Animals who had SPV together with pyloroplasty showed good emptying of the stomach with few ulcers. In this group the incidence of ulceration was reduced from 45% to 5,5%.

With the use of electromyography and intraluminal pressure recording of the pylorus, the effect of selective proximal and truncal vagotomy on the gastric motility was investigated in 10 dogs. The condition was studied of intermittent motility (IM) of the stomach and duodenal bulb in patients with duodenal ulcer in the preoperative period, six months later and 12 months after performing selective proximal vagotomy (n = 131, 103, and 83 respectively). All patients presented with disorders in their IM which were found to be dependent on their preoperative condition failing to reverse even 6 to 12 months after having the operation.

The pattern of gastric emptying after truncal vagotomy and drainage is usually biphasic. An early rapid phase is followed by a characteristically abrupt transition to slow emptying.
