Compressed Fixation Combined with Vacuum-Assisted Closure for Treating Acute Injury of the Heel Fat Pad.

BACKGROUND Treating acute injury of the heel fat pad is different from treating common soft tissue damage. Due to the paucity of literature on the topic, we described our initial experience treating acute injury of the heel fat pad to determine the ideal treatment method. MATERIAL AND METHODS A total of 53 patients with acute injury of the heel fat pad admitted to our hospital were selected for the study and were randomly divided into 2 groups: the compressed fixation combined with vacuum-assisted closure group and the only reimplanted and sewn group. Twenty-seven of the heel fat pads were com

Closure of the patent ductus arteriosus with the Amplatzer Duct Occluder II: a clinical experience.

BACKGROUND: The aim of our study was to share our clinical experience on cases with patent ductus arteriosus treated with the Amplatzer Duct Occluder II. METHODS: Between 2008 and 2012, 26 of 31 patients with patent ductus arteriosus underwent successful transcatheter closure of patent ductus arteriosus using the Amplatzer Duct Occluder II. Mean age was 3.3 years and mean weight was 15.7 kilograms. The presence of a residual shunt, left pulmonary artery or aortic obstruction was explored by administering contrast material during the procedure. The patients were discharged 24 hours after the p

Diagnosis and management of congenital umbilical arteriovenous malformation.

An 8-day-old neonate was presented with severe respiratory distress and diagnosed as primary pulmonary hypertension of the newborn on functional echocardiogram. Evaluation showed bounding pulse, enlarged umbilical cord, and bruit over the periumbilical region. Transthoracic echocardiography and CT angiogram showed a large fistulous communication between the umbilical vein and artery suggestive of congenital umbilical arteriovenous malformation leading to high-output cardiac failure and pulmonary artery hypertension. The patient was stabilised with medications and ventilation. Transcatheter clo

Evolving trends in interventional cardiology: endovascular options for congenital disease in adults.

As increasing numbers of patients with congenital heart disease enter adulthood, there is a growing need for minimally invasive percutaneous interventions, primarily to minimize the number of repeated surgeries required by these patients. The use of percutaneous devices is commonplace for the treatment of simple lesions, such as atrial septal defect, patent foramen ovale, patent duct arteriosus, and abnormal vascular connections. There is also substantial experience with device closure of membranous and muscular ventricular septal defects, as well as more complex shunts such as baffle leaks af

Rare adverse event monitoring of medical devices with the use of an automated surveillance tool.

Prospective outcomes surveillance using population level data allows for statistical methodologies and confounder adjustment not supported by the FDA's current monitoring system. We explored propensity score matching integrated into an automated surveillance tool as a method for confounder adjustment in an observational cohort. The application analyzed all patients undergoing PCI via femoral access route from 2002-2006. The rare outcome of interest was retroperitoneal hemorrhage (RPH) and the device was a vascular closure device (VCD). A propensity score model was developed to match VCD and no

Time trends and clinical outcomes of transcatheter patent ductus arteriosus closure in adults: insights from a Japanese nationwide registry.

Large-scale data on the current status, outcomes, and complications of transcatheter patent ductus arteriosus (PDA) closure in adults remain limited. This study aimed to elucidate the current status of transcatheter PDA closure in adults. Patient data (age ≥ 16 years) between January 2015 and December 2023 were extracted from the Japanese Structural Heart Disease (J-SHD) registry. We assessed the current practice of transcatheter PDA closure in Japan, focusing on the clinical characteristics, in-hospital mortality, intraoperative deaths, and adverse events. Procedural success was defined as ap

Optimizing Anticoagulation Therapy when Utilizing Mechanical Circulatory Support Devices.

Hemorrhagic complications are frequent and clinically significant in patients undergoing mechanical circulatory support (MCS), occurring in 10% to 40% of cardiogenic shock cases and 5% to 10% of high-risk percutaneous coronary interventions. These events prolong hospitalization, increase morbidity, and adversely impact outcomes. This review examines the prevention and management of bleeding during MCS implantation, prolonged support, and removal. Access-site complications-predominantly from large-bore sheaths-can be mitigated through pre-procedural vascular imaging, ultrasound-guided puncture,

Transapical left ventricular access for difficult to reach interventional targets in the left heart.

OBJECTIVE: Interventional targets may be virtually "excluded" due to vascular access problems or complex previous surgical procedures. This study reviews our experience using transapical ventricular puncture to gain direct access to the systemic ventricle. PATIENTS: Patient 1 (74 years, 2 previous sternotomies), patient 2 (66 years, 5 previous sternotomies), and patient 5 (69 years, 3 previous sternotomies) with prosthetic valves had paravalvular mitral valve leaks. Patient 3 (6.3 years, 2 previous sternotomies) with an extracardiac Fontan conduit, had a significant residual leak after two pr

Totally Extravascular Bioresorbable Closure Reduces Access Complications after Endovascular Peripheral Intervention.

BACKGROUND: Percutaneous closure of the access artery has become commonplace after endovascular intervention. In patients with peripheral vascular occlusive disease, however, control of the puncture site is more problematic as the access artery is frequently plaque-laden and stenotic. Ischemic complications in the access extremity are more common in these cases, particularly when using devices that depend on the deployment of prosthetic material within the compromised arterial lumen. The purpose of this retrospective clinical study was to assess the efficacy of totally extravascular bioresorba

Toward a minimalist strategy for PFO closure: outcomes and patient experience from 205 ICE-guided procedures performed under local anesthesia and early discharge.

Patent foramen ovale (PFO) closure is an established treatment for secondary prevention after PFO-related stroke. Traditionally guided by transesophageal echocardiography (TEE) under general anesthesia, the procedure can now be performed using intracardiac echocardiography (ICE) under local anesthesia, supporting a minimalist and potentially outpatient approach. We retrospectively analyzed 205 consecutive patients who underwent ICE-guided PFO closure at Dijon University Hospital between 2018 and 2023. All procedures were performed under local anesthesia, without TEE or anesthesiology support.

Balloon-Assisted Transaxillary Artery Closure (BATAC): A Novel Vascular Closure Approach in Patients Undergoing TAVI Via Percutaneous Transaxillary Access.

Transaxillary (TAx) access has shown comparable outcomes to transfemoral access in selected studies of transcatheter aortic valve implantation (TAVI). Although TAx-TAVI has mainly been performed via surgical cutdown under general anesthesia, a fully percutaneous approach under local anesthesia with mild sedation has recently emerged as a less invasive option. We present Balloon-Assisted Transaxillary Artery Closure (BATAC) approach and report our early clinical experience. This observational study included consecutive adult patients with severe aortic stenosis who underwent simplified TAx-TAVI

Enhanced Thromboresistance and Endothelialization of a Novel Fluoropolymer-Coated Left Atrial Appendage Closure Device.

BACKGROUND: Device-related thrombus (DRT) after left atrial appendage closure (LAAC) procedures is a rare but potentially serious event. Thrombogenicity and delayed endothelialization play a role in the development of DRT. Fluorinated polymers are known to have thromboresistant properties that may favorably modulate the healing response to an LAAC device. OBJECTIVES: The goal of this study was to compare the thrombogenicity and endothelial coverage (EC) after LAAC between the conventional uncoated WATCHMAN FLX (WM) and a novel fluoropolymer-coated WATCHMAN FLX (FP-WM). METHODS: Canines were

A Case of Atrial Septal Defect Unveiled by the Treatment for Pulmonary Arterial Hypertension.

We present a case of a 51-year-old woman with atrial septal defect (ASD) masked by pulmonary arterial hypertension (PAH). Three months after PAH treatment with a combination of endothelin receptor antagonist and phosphodiesterase five inhibitor, the transthoracic echocardiography revealed left-to-right shunting through a secundum ASD. The pulmonary vascular resistance decreased from 7.4 to 2.6 Wood units. Subsequent transcatheter closure of ASD using Occlutech Figulla Flex II device was successfully performed as a treat-and-repair strategy. Five months later, hemodynamics had normalized. Due t
