OBJECTIVE: Temporal artery vasculitis (TAV) in patients younger than 50 years is extremely rare. A case of TAV in an 18-year-old man is described here, followed by a literature review regarding cases of all types of vasculitic involvement of the temporal arteries in the young.
METHODS: Review of the English literature on vasculitis involving the temporal arteries in young patients, based on a PubMed search.
RESULTS: Less than 40 cases of vasculitic involvement of temporal arteries in the young have been described. TAV in the young may be divided into 3 groups: juvenile temporal arteritis, a …

Vasculitis is inflammation of blood vessels, which may also affect the arteries of the kidney. The definition and classification of vasculitides derive from the Chapel Hill nomenclature criteria (CHCC) and American College of Rheumatology (ACR) criteria, respectively. These criteria are not perfect, and adult literature refers to problems with misclassification and a low concordance between the two criteria. The problems associated with defining a child patient according to these criteria are even more apparent, since both sets of criteria have been based on adult data alone and have never bee…

INTRODUCTION: Vasculitis are characterised by the inflammatory infiltrate, chiefly of mononuclear cells, in the walls of the blood vessels, which can lead to occlusion with necrosis and the subsequent infarction of the affected tissue.
AIMS: The Objective of this study was to determine the clinical, anatomicopathological and neurophysiological aspects of vasculitic neuropathies in infancy.
PATIENTS AND METHODS: Each patient was submitted to the following tests: a complete hemogram, systemic lupus erythematosus cells (LE cells), lupus anticoagulant, antinuclear antibodies, neutrophil anticyto…

OBJECTIVE: To describe clinical and pathological findings of a new case of juvenile gangrenous vasculitis of the scrotum, a peculiar and unusual (only 7 cases, approximately, have been previously reported) type of scrotal gangrene described by Piñol in 1973.
METHODS: A 24 year-old male presented at the emergency room with a right scrotal necrotising cutaneous lesion and fever. Physical examination, complete blood count, biochemical test, roentgenographic abdominal and thoracic findings, microbiological test and biopsy were performed.
RESULTS: Clinical aspects (symptoms and sings) and results…

Leucocytoclastic vasculitis is a small vessel inflammatory disease mediated mostly by deposition of immune complexes. Infections, medications, chemicals, bacteria, viruses, and diseases associated with immune complexes have been accused in the pathogenesis. Cutaneous leucocytoclastic vasculitis presents as palpable purpura most often localized in the lower extremities, often accompanied by abdominal pain, arthralgia and renal involvement. The clinical diagnosis of leucocytoclastic vasculitis is confirmed histopathologically by skin biopsy. In order to determine the cause of the disease, depend…

Important strides have been made in unraveling the pathophysiologic characteristics of some individual forms of vasculitis, but vasculitides continue to pose enormous challenges for clinicians. Over time, numerous myths and an occasional pearl have arisen from the care of patients with these disorders. In this collection of pearls and myths, we have attempted to pool our knowledge about the clinical care of vasculitis patients.

INTRODUCTION: We report an unusual case of a patient with clinical and histological features of cutaneous vasculitis. CASE PICTURE: A middle-aged Chinese male presented with livedo reticularis and digital gangrene without visceral involvement. Skin biopsy showed features suggestive of cutaneous vasculitis. Repeated testing for anticardiolipin antibody was negative.
TREATMENT: He was treated with two courses of intravenous prostacyclin and pulsed with one course of intravenous methylprednisolone. He was also put on oral prednisolone, pentoxifylline, aspirin, nifedipine and colchicine in additi…

The authors report a case of isolated spermatic cord vasculitis associated with testicular infarct in a 75-year-old man. No clinical symptoms or serological markers of a systemic disease were found. This is a rare disease which cannot be diagnosed without histological examination and which requires follow-up because of a risk of progression to autoimmune disease.

We present 2 cases of histologically identified necrotizing vasculitis found in the seminal vesicles of radical prostatectomy specimens containing prostatic adenocarcinoma. In neither case did the patient suffer from symptoms nor show signs of systemic vasculitis. In 1 case, the vasculitis correlated clinically with dense adhesions noted at surgery that simulated extraprostatic tumor extension. Isolated vasculitis of the seminal vesicle is similar to reports of isolated vasculitis limited to other organs, in that local clinical findings may or may not be present, but the patient is free of sys…

Central nervous system (CNS) involvement and management in primary vasculitis (giant cell arteritis, primary angiitis of the CNS, Takayasu's disease, periarteritis nodosa, Kawasaki disease, Churg-Strauss syndrome, Wegener's granulomatosis) and vasculitis secondary to collagen vascular diseases, Beçhet's disease and other systemic conditions (excluding those secondary to infections) and the use of illicit drugs are reviewed. Vasculitis is an infrequent cause of stroke even in the young age groups, and routine screening of stroke patients for vasculitis is not cost-effective. CNS vasculitis may …

Behçet's syndrome can involve all sizes and kinds of blood vessels. There is an association between arterial involvement and venous thrombosis. Pulmonary arterial aneurysms and neurological involvement have a definite influence on mortality. Male sex and young age are indicators of a more severe disease course. Immunosuppressive treatment early in the disease may affect the long term prognosis favourably. Patients with familial Mediterranean fever may develop manifestations of vasculitis. The most common associations are with Schönlein-Henoch purpura and polyarteritis nodosa. In some patients …

Angiitis of the CNS remains a poorly understood form of vascular inflammatory disease. Primary angiitis of the CNS or PACNS, a disease once considered extremely rare, recently has been reported more frequently. Traditional views of PACNS held it to be a relentless and uniformly fatal disease if untreated, but it is now viewed as more heterogeneous, with some relatively benign subsets. Its pathophysiology is poorly understood. Secondary vasculitis of the CNS can occur with a variety of other conditions and diseases; each requires a different diagnostic and therapeutic approach. Regardless of wh…

The kidneys are frequently affected by systemic vasculitides. This is not surprising given the numerous vessels within the renal parenchyma. The kidneys are most often involved by small vessel vasculitides, such as microscopic polyangiitis (microscopic polyarteritis), Wegener's granulomatosis, Henoch-Schönlein purpura, and cryoglobulinemic vasculitis. These vasculitides cause renal dysfunction predominantly by inducing glomerular inflammation with resultant nephritis and renal failure. Microscopic polyangiitis (microscopic polyarteritis) and Wegener's granulomatosis are associated with and may…
