The purpose of this research is to study the causes of vertigo in the patients at Otoneurology Clinic, Thammasat Hospital. The data of these patients, collected from the medical records between January 2010 and January 2011, were reviewed and analyzed. From one hundred and forty-nine cases, 49 cases (33%) were men and 100 cases (67%) were women, which yielded the male-to-female ratio of 1:2. The average age of patients was 55 year olds. The most common diagnostic category was peripheral vestibular disorders (80.5%). Other causes were central vestibular disorders (4.7%) and non-vestibular relat…

A retrospective study to investigate the clinical epidemiological characteristics of vertigo was carried out on 187 patients with vertigo. A clinical history for each patient was recorded precisely about the attack, frequency, and development of vertigo, its duration, intensity, and the accompanied symptoms including the risk factors for cerebrovascular disease, etc. All the patients were subjected to physical examination with special attention to neurologic systems and Dix-Hallpike maneuver, computed tomography/computed tomography-angiography (CT/CTA) and MRI scan were performed when necessar…

Vertigo and balance disorders are not uncommon in children. The prevalence of vestibular vertigo in 10-year-Dolds is estimated to be 5.7%. The most common cause is vestibular migraine which accounts for almost 40% of the diagnoses. In adolescents, the incidence of somatoform vertigo syndromes increases. Vestibular function can be reliably evaluated at the bedside by the head-impulse test for vestibulo-ocular reflex function, ocular motor testing of the central vestibular system, and balance tests for vestibulo-spinal function. Vestibular migraine is treated by behavioural and drug therapies. S…

Vertigo can be defined as an illusion or hallucination of movement. The control of balance is complicated. Vertigo can be caused by many different pathologies, some of which are potentially life threatening. An important differentiation is whether the symptoms of vertigo originate from a central or peripheral origin. Clues to a central origin are other brainstem symptoms or signs of acute onset such as headache, deafness and other neurological findings. These patients warrant urgent referral and investigation. Red flags in patients with vertigo include: headache; neurological symptoms; and neu…

Anxiety and depression greatly affect the prognosis of and burden on subjects seen for dizziness or vertigo, who usually report multiple somatic symptoms. We studied the prevalence of these symptoms in 145 subjects hospitalized for dizziness or vertigo and taking part in 4-day group vestibular rehabilitation. Questionnaires given to determine the prevalence of somatic symptoms assessed headache, insomnia, diarrhea, constipation, stomachache, chest pain, palpitations, dyspnea, general fatigue, and stress. Quantitation used aerical rating scale (NRS). Anxiety and depression were assessed using t…

Otology & neurotology : official publication of the American Otological Society, American Neurotology Society [and] European Academy of Otology and Neurotology2009eng
OBJECTIVE: To evaluate the neurotologic findings in patient suffering from migraine with and without vestibular symptoms.
STUDY DESIGN: Comparative cross-sectional and observational study.
SETTING: Tertiary referral center.
BACKGROUND: Migraine headache is often associated with other symptoms, including dizziness, head motion intolerance, or rotational-type vertigo. The neuro-otologic examination in migrainous patients often gives unremarkable results. To date, it is not fully understood why some migraine patients complain of vestibular symptoms and some do not, and it is not yet clear whet…

OBJECTIVE: Causes of benign positional vertigo (BPV) are mostly unknown. The aim of this study was to elucidate an association of osteoporosis with idiopathic BPV.
METHODS: Two hundred nine consecutive patients with a confirmed diagnosis of idiopathic BPV underwent bone mineral densitometry of anterior-posterior lumbar spine and femur. The T scores were compared with those of 202 controls without a history of dizziness. Recurrence was defined when the patients reported two or more previous episodes of positional vertigo similar to those experienced at the time of diagnosis.
RESULTS: In both …

The diagnosis of vertigo syndromes in childhood is considered a difficult challenge. A correct diagnosis, however, makes successful therapy possible, prevents unnecessary investigations and alleviates the worries of parents. Migraine-associated forms of vertigo are very common in children (benign paroxysmal vertigo of childhood, vestibular migraine) and account for about 50% of diagnoses. Acute unilateral vestibular failure in the course of infectious and para-infectious labyrinthitis is also more common in children than in adults. Motion sickness is a frequent and relevant problem in children…

After the failure of medical treatment of dizziness or vertigo, surgical treatment is chosen according to functional diagnosis and duration and severity of symptoms. Surgery is contraindicated in patients with incomplete vestibular compensation, central nervous disorders and bilateral vestibular deficits. Surgery is obligatory in cases of sudden loss of labyrinthine function with traumatic perilymph fistula, middle ear and temporal bone processes. Endolymphatic hydrops and pathological endolymph pressure of other etiology as impaired middle ear ventilation, EVAS or superior semicircular canal …

We report a 37-year-old woman with acute disseminated encephalomyelitis (ADEM) who presented with progressive vertigo. A central nervous system disorder was initially suspected because gaze-evoked torsional nystagmus was observed and electrical nystagmography (ENG) revealed abnormal eye movements with saccadic ocular pursuit and pathological optokinetic nystagmus. Finally ADEM was diagnosed by the clinical symptoms and the characteristic patterns of brain MRI. It is rare for otolaryngologists to examine patients with ADEM. ADEM takes an acute course and can present a grave state and therefore …

Although our understanding of the mechanisms of vertigo and pathophysiology of vertiginous disorders has increased, diagnosis and treatment of various vertiginous diseases is challenging. The objective for the treatment of a vertiginous disease is to eliminate the underlying pathology either with maneuvers or drugs. In vertiginous diseases, surgery is performed either to eliminate the underlying pathologic event or to create stability in the incoming vestibular signals. It is not always possible to treat the underlying disease. Therefore, surgery is usually performed for the relief of vertigo.…

Subjects were 626 patients reporting vertigo or dizziness seen at the University Hospital Department of Otolaryngology from April 2001 to September 2003. Patients were diagnosed based on diagnostic criteria prescribed by the Japan Society for Equilibrium Research. The most common peripheral vestibular disorder was benign paroxysmal positional vertigo (32%), followed by Meniere's disease (12%). All peripheral vestibular disorders accounted for 65%. Central vestibular disorder accounted for 7%, of which space-occupying lesions in the posterior fossa accounted for 1.0% and cerebral infarction 1.9…

Investigation of vestibular system using a standard method of electro-nystagmography in complex otoneurologic examination does not provide enough information on the etiology of vestibular analyzer lesion, location and diffusion of the pathological process. Neurovisual methods, in particular MRI and CT, have demonstrated high diagnostic validity in such pathological states as tumors of pons cerebellum and posterior focca; infarctions in the stem and cerebellum regions. The study revealed a role of MRI and CT in etiology of vertigo and lesions of cochleovestibular analyzer. One hundred thirty pa…

Vertigo in children is less frequent than in adults, and examiners of patients showing these symptoms must rely on parents' or relatives' observations and details. Besides the equilibrium disorders caused by hereditary malabsorption syndromes or lesions in the peripheral and central vestibular structures, we know of typical diseases that are associated with vertigo and hearing problems and develop during childhood. One of them is Ménière's disease. Careful examinations are necessary to differentiate these illnesses from other vestibular disturbances accompanied by vertigo. Neurootological exam…
