Genetics of recurrent vertigo and vestibular disorders.

We present recent advances in the genetics of recurrent vertigo, including familial episodic ataxias, migraneous vertigo, bilateral vestibular hypofunction and Meniere's disease.Although several vestibular disorders are more common within families, the genetics of vestibulopathies is largely not known. Genetic loci and clinical features of familial episodic ataxias have been defined in linkage disequilibrium studies with mutations in neuronal genes KCNA1 and CACNA1A. Migrainous vertigo is a clinical disorder with a high comorbidity within families much more common in females with overlapping f

Vestibular function is associated with immune inflammatory response.

Association between vestibular function and immune inflammatory response has garnered increasing interest. Immune responses can lead to anatomical or functional alterations of the vestibular system, and inflammatory reactions may impair hearing and balance. Vestibular disorders comprise a variety of conditions, such as vestibular neuritis, benign paroxysmal positional vertigo, Meniere's disease, vestibular migraine, posterior circulation ischemia, and bilateral vestibular disease. Moreover, some patients with autoimmune diseases develop vestibulocochlear symptom. This paper offers an overview

Factors influencing clinical outcome in vestibular neuritis - A focussed review and reanalysis of prospective data.

Following vestibular neuritis (VN), long term prognosis is not dependent on the magnitude of the residual peripheral function as measured with either caloric or the video head-impulse test. Rather, recovery is determined by a combination of visuo-vestibular (visual dependence), psychological (anxiety) and vestibular perceptual factors. Our recent research in healthy individuals has also revealed a strong association between the degree of lateralisation of vestibulo-cortical processing and gating of vestibular signals, anxiety and visual dependence. In the context of several functional brain ch

Direction-changing spontaneous nystagmus in patients with dizziness.

PURPOSE: The present study aimed to investigate the clinical features of patients with direction-changing spontaneous nystagmus (DCSN) and gain insight into its underlying mechanisms. METHODS: Medical records and vestibular function test results collected in our dizziness clinic between February 2013 and February 2020 were retrospectively reviewed. Spontaneous nystagmus was recorded while sitting upright using videonystagmography for 2 min to confirm the spontaneous changes in nystagmus direction. Causative disease diagnoses were based on the patients' clinical history, audiometry results, ve

[Vestibular neuritis - prevalence of inpatient cases in Germany].

INTRODUCTION: The vestibular neuropathy (VN) is a frequently diagnosed, acute peripheral vestibular form of vertigo. Typical symptoms are spinning vertigo, ipsilateral tendency to fall with standing and gait insecurity, and vegetative side effects. Epidemiological studies of VN in the general population are rare and the little existing literature varies in results. MATERIAL AND METHODS: All inpatient VN cases in Germany from 2000 to 2017 were evaluated. The data used was provided by the German Federal Statistical Office. In addition to the annual number of cases, information about age at diag

Vestibular neuritis.

Vestibular neuritis is the most common cause of acute spontaneous vertigo. Vestibular neuritis is ascribed to acute unilateral loss of vestibular function, probably due to reactivation of herpes simplex virus in the vestibular ganglia. The diagnostic hallmarks of vestibular neuritis are spontaneous horizontal-torsional nystagmus beating away from the lesion side, abnormal head impulse test for the involved semicircular canals, ipsilesional caloric paresis, decreased responses of vestibular-evoked myogenic potentials during stimulation of the affected ear, and unsteadiness with a falling tenden

Insufficient evidence for the effect of corticosteroid treatment on recovery of vestibular neuritis.

The authors studied the effect of corticosteroid treatment on clinical recovery and recovery of vestibular function in patients with vestibular neuritis. The comprehensive search (March 29, 2012) yielded 496 original papers, of which 5 (including 199 patients) during full-text screening satisfied our eligibility criteria. Methods assessment showed that 1 study (30 patients) provided direct evidence and carried low risk of bias. Two studies properly reported on their random and concealed allocation of treatment. In 1 study, patients were not randomly allocated to treatment. Blinding of outcomes

Impulsive testing of individual semicircular canal function.

In order to test the human angular vestibulo-ocular reflex in the dynamic range of normal head movements, we measured 3-dimensional compensatory eye-movement responses to low-amplitude (10-12 degrees), high-acceleration (3000-4000 degrees/s/s), passive, manually delivered head rotations (head "impulses") in the three planes of the semicircular canals in normal subjects, in subjects who had recovered from surgical unilateral vestibular deafferentation, and in patients after acute unilateral peripheral vestibulopathy, that is, from vestibular "neuritis." We found that canal-plane head impulses a

Eye movement features for assisting differentiation between posterior circulation infarction and vestibular neuritis in patients of acute vestibular syndrome.

INTRODUCTION: A significant portion of patients with acute vestibular syndrome (AVS) due to posterior circulation infarction (PCI) is free of non-vestibular signs. A differential diagnosis of this form from that due to vestibular neuritis (VN) can be challenging. We herein aimed to understand whether quantitative eye movement signature could be used to assist such a diagnosis. METHODS: A total of 56 patients with AVS due to PCI free of non-vestibular signs, 71 patients with AVS due to VN, and 127 controls were selected retrospectively into the study from our hospital database. Demographic, cl

Band limited chirp stimulation in vestibular evoked myogenic potentials.

Air conducted vestibular evoked myogenic potentials (VEMP) can be elicited by various low frequency and intense sound stimuli, mainly clicks or short tone bursts (STB). Chirp stimuli are increasingly used in diagnostic audiological evaluations as an effective means to obtain acoustically evoked responses in narrowed or extended frequency ranges. We hypothesized in this study that band limited chirp stimulation, which covers the main sensitivity range of sound sensitive otolithic afferents (around 500 Hz), might be useful for application in cervical and ocular VEMP to air conduction. For this p

[Malfunction of the inner ear is the most frequent cause of dizziness].

Half of all dizziness patients suffer from a malfunction of the inner ear and benign paroxysmal positional vertigo causes 25-30% of these malfunctions. During the latest two decades new vestibular test equipment has been developed and has made it possible to refine the vestibular diagnoses and consequently treat patients more efficiently. This brief overview describes the benefit of Epley's Omniax Chair, video head impulse tests and the Vestibular Autorotation Test. The need for vestibular rehabilitation programmes and for a closer collaboration between physicians and the social authorities is

Three-dimensional reconstruction of the cochlear nuclear complex in humans.

For accurate physical three-dimensional reconstruction of the cochlear nuclear complex (CN), we used light microscopy to analyze the CN boundaries, cell types, and myelinated nerve bundles and transferred traced contours from enlarged photographs of serial sections onto acrylic plates. We produced two models, one demonstrating the CN in relation to the brain stem and the other showing the CN completely dissected out of the brain stem. The models localized the CN boundaries on the brain-stem surface and revealed topographic characteristics of the CN and a part of the eighth nerve adjacent to th

Adult neurogenesis promotes balance recovery after vestibular loss.

A phenomenon called vestibular compensation occurs after peripheral vestibular loss. This process involves a mosaic of profound structural rearrangements within the vestibular nuclei. Among them, adult reactive neurogenesis is perhaps the most unexpected, as it occurs in a brain area that was never reported as neurogenic before. Both the survival and functionality of this newly generated neuronal network depend on its integration into preexisting networks in the deafferented structure. Far from being aberrant, this organization allows the brain to use inputs from other sensory modalities to fa

The endocannabinoid system: A new player in the neurochemical control of vestibular function?

The results of recent clinical trials of medicinal cannabinoid drugs show that dizziness and vertigo are commonly reported adverse side-effects. Cannabinoid CB1 receptors were initially thought to be expressed in very low densities in the vestibular nucleus complex (VNC). Recent immunohistochemical studies have challenged this idea and suggested that CB1 receptors may exist in numbers similar to the granule cell layer of the cerebellum. This, together with evidence that brainstem CB1 receptors have a higher efficacy than those in many other parts of the brain and that application of cannabinoi

Repetitive paroxysmal nystagmus and vertigo.

A 55-year-old woman had paroxysms of vertigo and visual blurring associated with complex combined torsional, horizontal, and vertical nystagmus. These episodes occurred regularly at 2-minute intervals, each attack lasting for 15 seconds. Between attacks, there was a much finer asymptomatic nystagmus whose components were in the opposite direction to those associated with the paroxysmal attacks. A brain MRI revealed an arteriovenous malformation in close proximity to the left vestibular nucleus, with evidence of previous bleeding. Caloric testing demonstrated a left-sided vestibular paresis. We
