[Magnetic resonance imaging of the inner ear and its application to cochlear implant candidates].

Magnetic resonance (MR) images of normal and abnormal inner ears obtained with a 1.5T unit without the use of a surface coil were analyzed to evaluate the effectiveness of this method in the screening of cochlear implant candidates. In a series with sixteen normal adult volunteers, 22 to 45 years in age, 100% of the cochleae, vestibules, and lateral and posterior semicircular canals were clearly demonstrated in T2 weighted images. Satisfactory images were not obtained for 22% of the superior semicircular canals, 22% of the cochlear aqueduct, and 81% of the vestibular aqueduct in axial images,

Poststapedectomy dizziness. A histopathologic report.

Long-term disabling dizziness is a rare but serious postoperative complication of stapedectomy. The temporal bone collection of the Ear Research Institute in Los Angeles includes 117 bones from patients who underwent unilateral or bilateral stapes operations. Of them, nine patients (8 percent) complained of severe long-term postoperative dizziness. Histologic examination of the temporal bones revealed a pathologic condition in the peripheral vestibular system of seven patients; the remaining two patients showed normal maculae, cristae, and vestibular nerve. Pathologic conditions found were sup

The vestibular aqueduct and endolymphatic sac and duct in endolymphatic hydrops.

The histologic features of the endolymphatic sac and duct in 23 serially sectioned temporal bones with idiopathic or secondary endolymphatic hydrops were blindly compared with 22 randomly selected, normal temporal bones. In idiopathic hydrops, the pars rugosa of the endolymphatic sac extended out of the vestibular aqueduct into the dura in 29% of bones, compared with none of normal bones (P less than .01). In the other 71%, the pars rugosa in the vestibular aqueduct was surrounded by dura more commonly than normal. Functional studies are required to assess the relationship of these findings to

Post-traumatic pneumolabyrinth: a rare cause of hearing loss.

Pneumolabyrinth (PL) is the presence of air within the vestibule, semicircular canals, or cochlea. It represents an abnormal connection between the inner ear and middle ear spaces. PL most commonly occurs after blunt head trauma, followed by penetrating injuries. Temporal fractures may or may not accompany. Prognosis of hearing loss is poor, while prognosis of vestibular symptoms is good. Herein we present a 45-year-old female with unilateral pneumolabyrinth, who presented with significant dizziness and unilateral total hearing loss after a car accident.

Regional differences of brain glucose metabolic compensation after unilateral labyrinthectomy in rats: a [14C]2-deoxyglucose study.

A unilateral labyrinthectomy was performed on anesthetized adult albino rats. Brain [14C]2-deoxyglucose (2DG) uptake was measured autoradiographically 3.5 h to 20 days later and compared to sham-operated controls. In the vestibular nuclei (nn.) of labyrinthectomized subjects, large left-right differences of 2DG uptake occurred, which decreased over time. The equalization of vestibular nuclear 2DG uptake paralleled behavioral compensation of body, neck and head postural abnormalities, and known equalization of vestibular nuclear cell firing rates during compensation. There was a small differenc

Medical management of vestibular dysfunction.

An overview of the medical management of vestibular dysfunction is presented. The lack of firm etiologic and pathophysiologic data on which to base rational treatments is emphasised. Medical management of Meniere's disease is reviewed in depth while that of other vestibular disorders is touched on briefly.

Quantification of slow compensatory eye movements in patients with bilateral vestibular loss. A study with a broad frequency-band rotatory test.

Seven patients with bilateral vestibular dysfunction were investigated with a new broad frequency oscillatory test. The vestibular stimulation was of two types: a pseudo-random oscillation in the frequency range 0.5-4 Hz and a sinusoidal frequency sweep from 0.5 to 3.5 Hz. Compensatory eye movements were recorded by EOG. Gain and phase values between eye and head movements were extracted using power spectral analysis technique. Two different visual conditions were used: 1) fixating a stationary target and /) fixating a moving target (visual suppression test). At high frequencies all patients s

[Multiple affections of the cranial nerves as a pathognomonic sign of periarteritis nodosa. Relationship with Cogna's syndrome (author's transl)].

A 43-year-old woman was found to have multiple cranial neuritis affections, present as an isolated disorder over a long period, and characterized by bilateral deafness and associated lesions of the VII and V cranial nerves. On investigation, visual disturbances were discovered which were of a transitory nature and made up a total clinical picture suggesting Cogan's syndrome. Neuropathological examination revealed the presence of typical periarteritis nodosa lesions in the cranial nerves. The authors suggest, therefore, that certain cases described as Cogan's syndrome are in fact particular for

[Significance of associated causes in the evaluation of post-traumatic vestibular damage].

Any evaluation of vestibular diseases following head and neck injuries must take into consideration the existence of other diseases which may act with the trauma causing lesions which might not have taken place otherwise or which would have had less import. In the present work the authors examine what medico-legal doctrine considers in regard to pre-existing, simultaneous and cropped up aggravations of lesions or of invalidity. Finally, an explanation is given of personal experience with 1500 patients with head and neck injury. It is pointed out how in 130 cases the presence of pathological co
