Workplace-related abuse, threats and assault are globally recognised as a significant risk for healthcare workers. This paper briefly reviews systemic factors contributing to aggression and violence before focussing on techniques first to identify high-risk patients and then sufficiently defuse their behaviour to enable a clinical assessment to be undertaken. General guidance is offered to enable compliance with relevant guardianship and mental health acts. Emphasis is placed on the importance of post-incident debriefing and, where necessary, counselling and for there to be systems in place to

BACKGROUND: Workplace aggression constitutes a serious issue for healthcare workers and organizations. Aggression is tied to physical and mental health issues at an individual level, as well as to absenteeism, decreased productivity or quality of work, and high employee turnover rates at an organizational level. To counteract these negative impacts, organizations have used a variety of interventions, including education and training, to provide workers with the knowledge and skills needed to prevent aggression. OBJECTIVES: To assess the effectiveness of education and training interventions th

BACKGROUND: The causes and contexts of homicide of U.S. nurses have not been well studied, although recorded numbers have risen rapidly in recent years, drawing greater attention.
PURPOSE: This exploratory study aimed to describe nurse homicide trends and identify potentially remediable factors.
METHODS: Using data in the National Violent Death Reporting System (NVDRS) database, an analysis of records of reported homicides of all nurses and nursing assistants from 2003 through 2022 was conducted. Data analyses included descriptive statistics and examinations of paired characteristic variable

OBJECTIVE: Violence is a common hazard for those working in emergency departments (EDs), yet it remains understudied. We describe a prospectively derived estimate of the prevalence of violence against health care workers among agitated patients in an ED.
METHODS: This was a secondary analysis of two prospective, observational studies of patients receiving care in a dedicated portion of the ED meant primarily to observe patients with intoxication. We collected detailed data for patients with agitation, defined as a score of +1 or higher using the altered mental status scale, an ordinal agitati

An improved understanding of patient-related violent events toward healthcare workers (HCWs) is a critical step in mitigating patient violence in the pediatric medical hospital setting. Therefore, we sought to describe the timing/setting, potential antecedents to, and management of pediatric patient-related violence toward HCW. Using our electronic health record (EHR), we performed a retrospective study of patient-related physical violent events from 2017 to 2022 among youth hospitalized at our free-standing children's hospital. We identified 144 violent events associated with 75 patients. Mos

Workplace violence in healthcare settings is a complex topic with many different environments in which aggression is sometimes expressed by patients toward those entrusted with providing their healthcare. The assessment of violence risk in a nursing home containing many patients with organic brain syndrome is quite distinct from assessment in forensic psychiatric units, inner city emergency rooms, or outpatient pain clinics. Three cases are presented that are composite summaries of actual assaults which took place across different hospital settings, all within an urban Midwestern city in the U

BACKGROUND: There are many studies concerning the health consequences of intimate partner violence (IPV). However, little research has been done on the health consequences of other forms of violence against women (VAW) such as the violence perpetrated by male relatives, friends or strangers. The aims of this paper were: (i) to analyze the prevalence of different forms of VAW perpetrated by males at home, workplace and other social environments in Spain and (ii) to analyze whether IPV and other forms of VAW have a different or similar negative impact on women's health.
METHODS: A sample of 13

Background/Objectives: Healthcare workplace violence has evolved into a global crisis, significantly impacting high-risk specialties. While the "Second Victim Phenomenon" (SVP) is well-established for trauma following medical errors, the specific psychological trauma resulting from intentional external aggression remains conceptually under-defined. This study aims to introduce and define the "Violence Victim Phenomenon" (VVP) by exploring the lived experiences of anesthesiology and intensive care specialists, providing a theoretical framework for this distinct clinical state. Methods: A qualit

BACKGROUND: Home health aides (HHAs) are the frontline providers for vulnerable community-dwelling older adults. Despite their critical role, HHAs are often a understudied workforce. This study aimed to evaluate HHAs' workplace experiences and perceptions regarding caring for older adults, including data on dementia care needs.
METHODS: Following preliminary focus groups, an anonymous survey was distributed to HHAs from a large home health care agency in the greater New York metropolitan area. The survey collected demographic data, professional experience, training, perceived job-related chal

INTRODUCTION: Emergency department (ED) boarding, or holding admitted patients in the ED due to a lack of available inpatient beds, presents a major challenge to healthcare systems. This study examines the impacts of ED boarding on clinician wellness, burnout, moral injury, and workplace violence.
METHODS: We distributed a survey to members of the American Academy of Emergency Medicine over 4 weeks in early 2025. It included 9 questions on experiences with boarding, demographics, and practice setting, plus an optional free-response question. Descriptive statistics were performed, and free-res

INTRODUCTION: Ambulance personnel frequently encounter aggression in dynamic and unpredictable environments. Despite growing awareness of workplace violence in healthcare, few validated tools exist for systematic documentation in ambulance services.
OBJECTIVE: This study aimed to adapt and validate the Staff Observation Aggression Scale - Revised (SOAS-R) for use in ambulance services (SOAS-RA), and to examine the relationship between SOAS-RA severity scores and staff's subjective perceptions of incident severity using a Visual Analogue Scale (VAS).
METHODS: Using a modified Delphi method, a

BACKGROUND: Health care workers are 4 times more likely to suffer violence than workers in other industries.
PURPOSE: The aim was to examine types of patients' verbal/physical abuse against the nursing workforce observed through patient-engaged video surveillance (PEVS) and interventions initiated by monitor technicians.
METHODS: A descriptive study was conducted to analyze all types of patient-initiated abuse, physical and verbal, reported from 73 hospitals and patient response to PEVS.
RESULTS: Of 150 434 patients whom RNs enrolled into 24-hour PEVS, 5034 patients (3%) were identified by

Nurses and other emergency department (ED) staff often experience verbal and physical abuse as part of their everyday work. Body-worn cameras are one tool that ED staff can use to try to reduce workplace violence and abuse and have been shown to be effective in other healthcare environments, such as mental health units. This article describes a pilot service evaluation which used a survey to assess the effectiveness of using body-worn cameras for staff in the author's ED. Findings suggest that nurses believed the cameras provided support when they were confronted by abusive or aggressive patie

Background/Objectives: Patient- and family-initiated abuse of nurses is widely recognized as workplace violence, but it also raises a distinct professional-regulatory question: when may a nurse end or restrict an established therapeutic relationship without creating an allegation of abandonment or discriminatory denial of care? This perspective focuses on Alberta and examines whether the province's nursing regulator provides a comprehensive, nurse-specific, all-registrant standard comparable in clarity to Alberta's physician standard. The concern is not the absence of every form of protection

INTRODUCTION: The impact of racism on patient outcomes in Emergency Medicine has been examined but there have been few studies exploring the experiences of racism in health care workers. This survey aims to explore the experience of racism by interdisciplinary staff in a tertiary ED. By characterizing the staff experience of racism in the ED, we hope to inform the design of strategies to disrupt racism and ultimately improve the health and wellness of both staff and patients.
METHODS: We conducted a self-administered, cross-sectional survey to explore the reported experience of racism by heal

BACKGROUND: Youth violence affects thousands annually, with homicide being the third leading cause of death for those aged 10 to 24 years. This systematic review aims to evaluate the published evidence for the effects of health care-based violence intervention programs (VIPs), which focus on reducing recurrent presentations for injury due to youth violence ("recidivism").
METHODS: Health literature databases were searched. Studies were retained if peer reviewed and if programs were health care based, focused on intentional injury, addressed secondary or tertiary prevention (i.e., preventing r

OBJECTIVE: Clinical staff in EDs are subject to a range of stressors. The objective of this study was to describe and compare clinical staff perceptions of their ED's working environment across two different Australian EDs.
METHODS: This was a cross-sectional, descriptive, research design that included distribution of three survey tools to clinical staff in two Australian EDs in 2016. Descriptive statistics were reported to characterise workplace stressors, coping styles and the ED environment. These data were compared by hospital and the employee's clinical role (nurse or physician).

Violence against nurses is a systemic threat that compromises workforce safety and quality care, especially in HIV nursing. The death of Alex Jeffrey Pretti, an intensive care nurse, exemplifies broader patterns of violence affecting nurses both within and beyond clinical settings and represents inherent risks of militarization within democratic societies. This commentary reviews epidemiologic evidence on violence against nurses, situated within systems of oppression, and assesses legislative efforts to protect the nursing workforce. Effective protection requires structural interventions such
