Stereotactic brain biopsy for diagnosis of progressive multifocal leukoencephalopathy.

We describe three patients in whom the diagnosis of progressive multifocal leukoencephalopathy (PML) was made by stereotactic biopsy. All three patients had a rapidly progressive clinical course pathologically characterized by demyelination, Alzheimer I astrocytes, and basophilic intranuclear inclusions in oligodendrocytes. In each case, the diagnosis was confirmed with immunohistochemical staining of oligodendrocytes for the JC virus and in one case by electron microscopy. Determination of the adequacy of tissue received at the time of frozen section is critical and can direct the appropriate…

Pathological observations in HTLV-I associated myelopathy.

The following are the clinical and autopsy findings in a 63-year-old woman with myelopathy associated with the human T-cell lymphotropic virus Type I (HTLV-I). HTLV-I antibody was positive in both the serum and cerebrospinal fluid (CSF). In the lower thoracic region, demyelination and the loss of axons were accompanied by a proliferation of astrocytes, and gliosis was found in the lateral columns. Perivascular and parenchymal infiltrations of macrophages, lymphocytes, and plasma cells were also observed, but neither viral inclusion bodies nor atypical lymphocytes were found.

The growth of olfactory neurons in short-term cultures of rat olfactory epithelium.

We have found that purified astrocytes will support the growth of olfactory epithelial neurons ( OENs ) in vitro. Candidate OENs were visible by light microscopy within one day after plating of dissociated cells from neonatal rat ethmoturbinates . Electron microscopy showed that OENs expressed the unique ultrastructural features which characterize these cells in situ, showing that regulation of neuronal morphology and ultrastructure can function independently of contact with other cells of the appropriate tissue. Characteristics of the cultures are such that this in vitro system would be usefu…

Creutzfeld-Jakob disease.

The eyes of two patients with Creutzfeld-Jakob disease were received at autopsy and examined for any specific changes. Previous clinical examination of one of the patients had failed to reveal any specific ocular changes. No evidence for an infectious agent was found in any portion of the eye. The cornea and the retinal were structurally intact. The axons and the glial elements of the optic nerve head were intact. However, increased cellularity, consisting mainly of astrocytes, was noted in the optic nerves in front of and behind the lamina cribrosa.

Physiology and pathophysiology of the retinal neuroglia.

Neuroglia of the retina are represented by Müller glia, parenchymal astrocytes, microglia and oligodendrocytes mainly associated with the optic nerve. Müller glia are the most numerous glia, endowed with multiple homeostatic functions and indispensable for the retinal morphofunctional organization. Müller cells integrate retinal neurons into individual functional units (known as retinal columns) and act as a living light guide, transmitting photons to photoreceptors. In pathology, retinal neuroglia undergo complex changes, which include upregulation of neuroprotection, reactive gliosis, and fu…

Globoid cell leukodystrophy (Krabbe disease) in a Merino sheep.

We describe the clinicopathologic features of an ovine case of Krabbe disease (globoid cell leukodystrophy). Brain lesions, sometimes bilaterally distributed, were present in the cerebellar peduncles, cerebellar folia white matter, medulla, pons, and spinal cord and characterized by marked myelin loss and numerous large macrophages (globoid cells), which tended to aggregate perivascularly. Gemistocytic astrocytes were abundant, and their nuclei were frequently abnormal. The activity of the deficient enzyme, galactosylceramide β-galactosidase, was undetectable in this neurologic disorder compar…

Trema micrantha toxicity in horses in Brazil.

After ingesting green leaves of T. micrantha, 2 horses showed apathy, locomotor deficit, blindness, recumbency, paddling, coma and death. The main gross findings were scattered haemorrhages, enhanced lobular pattern of the liver, and cerebral oedema. Histological changes included disseminated haemorrhages, massive hepatocellular necrosis, neuronal degeneration, Alzheimer type II astrocytes and cerebral perivascular oedema. Clinicopathological findings which were comparable with those observed in Trema micrantha poisoned ruminants, associated with epidemiological evidence suggested the diagnosi…

Neonatal Alexander disease: MR imaging prenatal diagnosis.

Alexander disease (AD) is a rare neurodegenerative disorder characterized by megalencephaly, leukoencephalopathy, and Rosenthal fibers within astrocytes. This report describes the case of a female patient with sonography-detected ventriculomegaly at 32 weeks' gestation and distinctive MR imaging features at 33 and 36 weeks' gestation, at birth, and at 2 months of age, which led to the suggested diagnosis of Alexander disease. Molecular analysis confirmed a missense mutation in the GFAP gene. The literature contains little information on the fetal MR imaging findings that may allow prenatal dia…

Human brain derived cell culture models of HIV-1 infection.

During the clinical course of acquired immune deficiency syndrome, infection of the CNS by human immunodeficiency virus-1 (HIV-1) may ultimately result in the impairment of cognitive, behavioral and motor functions. Viral neuropathogenesis involves inflammatory molecules and neurotoxins produced from infected and immune-activated lymphocytes, microglial cells and astrocytes. Here, we discuss the current understanding of HIV-1 infection of the CNS and various cell culture systems from the developing human brain in order to study the neurobiology of HIV-1 infection, the mechanisms contributing t…

Dengue virus in the brain of a fatal case of hemorrhagic dengue fever.

Neurologic complications associated with dengue fever are in general unusual. However, recent reports evidence more frequent neurologic alterations. In Mexico, neurologic involvement has not been reported in dengue cases. This report demonstrates the detection of dengue virus in the brain of a fatal case of dengue hemorrhagic fever. Serotype 4 was detected by immunohistochemistry and by RT-PCR in the inferior olivary nucleus of medulla and in the granular layer of cerebellum. Immunoreactivity was observed in neurons, astrocytes, microglia and endothelial cells. Our results emphasize the import…

A role for ciliary neurotrophic factor as an inducer of reactive gliosis, the glial response to central nervous system injury.

Within the central nervous system (CNS) ciliary neurotrophic factor (CNTF) is expressed by astrocytes where it remains stored as an intracellular protein; its release and function as an extracellular ligand are thought to occur in the event of cellular injury. We find that overexpression of CNTF in transgenic mice recapitulates the glial response to CNS lesion, as does its injection into the uninjured brain. These results demonstrate that CNTF functions as an inducer of reactive gliosis, a condition associated with a number of neurological diseases of the CNS.

Trafficking of activated cytotoxic T lymphocytes into the central nervous system: use of a transgenic model.

We have used cell or tissue-specific promoters to express lymphocytic choriomeningitis virus (LCMV) proteins in selected cells in independent lines of transgenic mice. Upon adoptive transfers into these mice, MHC-restricted LCMV-specific cytotoxic T lymphocytes homed specifically to either the choroid plexus (SV40 promoter) or beta cells of the islets of Langerhans (rat insulin promoter). The availability of promoters specific for neurons, oligodendrocytes and astrocytes makes this approach compelling for evaluating T cell trafficking into the CNS and for analyzing antigen presentation in vivo…

Severe cerebral damage in ornithine transcarbamylase deficiency.

Two patients are described with hyperammonemia due to ornithine transcarbamylase (OTC) deficiency who suffered severe shrinkage and collapse of the brain. The cerebral cortex was spongy and cavitated, containing only a few residual neurons, and was markedly gliosed. In one patient the basal ganglia were affected and harbored Alzheimer type II astrocytes. These lesions resemble those of acquired hepatocerebral degeneration and occur especially in female children with the milder form of the disease, who have a potential to survive. Strict observance of dietary restrictions is mandatory to avoid …

Progressive multifocal leukoencephalopathy. A case report and review of the literature.

Progressive multifocal leukoencephalopathy developed in a 70-year-old man with hepatoma treated with oncostatica. The brain showed multiple demyelinated lesions in the upper half of the cerebral white matter. The lesions consisted of proliferation of hypertrophied astrocytes and glial cells with abnormally large nuclei. These nuclei were shown to be filled with DNA by DAPI staining and reacted specifically with anti-JC antibody by immunofluorescence. Papova-like virions were confirmed on both the sections and negatively-stained brain extract under electron microscopy. The sixteen cases so far …

The role of glymphatic system in brain injury after subarachnoid hemorrhage.

Subarachnoid hemorrhage (SAH) remains a devastating cerebrovascular emergency with high mortality and long-term neurological morbidity, yet the mechanisms driving secondary brain injury remain incompletely understood. The glymphatic system, a glial-dependent perivascular clearance pathway, promotes convective exchange of cerebrospinal fluid and interstitial fluid through aquaporin-4 water channels on astrocytic endfeet to remove neurotoxic solutes. Emerging evidence implicates glymphatic dysfunction as a key mediator of brain injury after SAH. This review synthesizes current knowledge on the a

Mission cholesterol: Uncovering its hidden role in ALS neurodegeneration.

Cholesterol is a central determinant of membrane architecture, signaling, and cellular homeostasis in the central nervous system (CNS). While historically viewed as a structural component, emerging evidence highlights its dynamic regulatory role in neuronal function, particularly through its compartmentalized synthesis, trafficking, and turnover. This review examines the complex landscape of cholesterol metabolism in the CNS, emphasizing the cooperative roles of astrocytes and neurons, the partitioning of biosynthetic pathways, and the barriers that distinguish brain cholesterol pools from per…

The Genetic Background of the Immunological and Inflammatory Aspects of Progressive Supranuclear Palsy.

Progressive supranuclear palsy (PSP) is a neurodegenerative disease, classified as an atypical Parkinsonian syndrome, that has been pathologically and clinically defined. The histopathological aspects of the disease include tufted astrocytes, while the clinical features involve oculomotor dysfunction, postural instability, akinesia, cognitive impairment, and language difficulties. Although PSP is generally considered a sporadic disease, interest is growing in its genetics, with contemporary research focusing on familial backgrounds and neuroinflammation. Indeed, microglial activation and other…
