SGLT2-inhibitors reduce the cardiac autonomic neuropathy dysfunction and vaso-vagal syncope recurrence in patients with type 2 diabetes mellitus: the SCAN study.

BACKGROUND: In patients with type 2 diabetes mellitus (T2DM) the vaso-vagal syncope (VVS) recurrence could be due to the alteration of auton … ce at 1 year of follow-up. CONCLUSIONS: Non-SGLT2-I users vs. SGLT2-I-users had alterations of the autonomic nervous system, with a higher rate of VVS recurrence at 1 year of follow-up. The indexes of cardiac denervation predicted the VVS recurrence, while the SGLT2-I reduced the risk of VVS recurrence. CLINICAL TRIAL REGISTRATION NUMBER: NCT03717207.

Case Report: Relief of long-standing severe motion sickness following stellate ganglion block.

BACKGROUND: Motion sickness is a common and often debilitating condition arising from sensory conflict within the vestibular-visual-proprioceptive system, with downstream activation of autonomic and emetic pathways. Despite its prevalence in travel, maritime operations, aviation, spaceflight, and virtual environments, effective prophylaxis remains limited by adverse side effects. Experimental and clinical evidence implicates autonomic imbalance and sympathetic nervous system (SNS) hyperactivity in motion sickness susceptibility and symptom severity. CASE PRESENTATION: We report the incidental

Autonomic and hormonal biomarkers in individuals with autism spectrum disorders and developmental or intellectual delay: A systematic PRISMA review.

BACKGROUND: Individuals with autism spectrum disorder (ASD) and co-occurring global developmental delay or intellectual disability (GDD-ID) are highly vulnerable to stress, but a very small amount of research is being conducted at the physiological level. This review aims to identify and synthesize studies examining autonomic nervous system (ANS) and hypothalamic-pituitary-adrenal (HPA) axis biomarkers in such populations. METHODS: The review followed PRISMA 2020 guidelines (Pages et al., 2021) and the inclusion criteria encompassed peer-reviewed studies published in English that reported dir…

Does ambulatory blood pressure monitoring improve the diagnosis of secondary hypertension?

Ambulatory blood pressure monitoring can determine the average blood pressure level and the short- and long-term blood pressure variability (circadian rhythm). The circadian blood pressure rhythm appears to be mediated mainly by the circadian rhythm of the sympathetic tone which is linked to changes in physical and mental activity, e.g. the waking-sleeping cycle. A statistically significant circadian blood pressure rhythm was observed in approximately 80% of mild to moderate essential hypertensive patients as well as in normal subjects. However, in patients with Cushing's syndrome, under gluco

Towards a nonlinear "neuroimmune gate-model": beyond stellate ganglion block and vagus nerve stimulation.

We propose a three-level neuroimmune gate model to characterize the communication between the nervous system and the immune system. The first gate, Gate 1, is the peripheral neuroimmune gate. Here, immune cells, inflammatory mediators, sensory neurons, autonomic nervous system neurons, blood vessels, and specific tissue components interact. The second gate, Gate 2, is the spinal neuroimmune gate and includes spinal afferents, spinal neurons and circuits, preganglionic sympathetic neurons, and microglia. The third gate, Gate 3, comprises supraspinal structures, including the nucleus tractus sol…

Brain-gut interactions in inflammatory bowel disease.

Psycho-neuro-endocrine-immune modulation through the brain-gut axis likely has a key role in the pathogenesis of inflammatory bowel disease (IBD). The brain-gut axis involves interactions among the neural components, including (1) the autonomic nervous system, (2) the central nervous system, (3) the stress system (hypothalamic-pituitary-adrenal axis), (4) the (gastrointestinal) corticotropin-releasing factor system, and (5) the intestinal response (including the intestinal barrier, the luminal microbiota, and the intestinal immune response). Animal models suggest that the cholinergic anti-infl…

Workshop report: The effects of psychological variables on the progression of HIV-1 disease.

The reciprocal interactions between the neuroendocrine, immune, and autonomic nervous systems are complicated, yet worthy of examination. A body of literature suggests that psychological factors such as stress, or psychiatric conditions such as major depression, may influence the immune system thereby altering host susceptibility to viral, or other types of infection. Alternately, in an attempt to limit infection and replication, the anti-viral host response, via innate and acquired immunity and subsequent release of pro-inflammatory cytokines and additional anti-viral mediators, may affect mo…

Interference resistant contactless heart rate variability monitoring via millimeter wave radar.

Heart rate variability (HRV) plays a critical role in the prediction of diseases related to cardiovascular and autonomic nervous system. HRV monitoring using millimeter-wave radar has been exploited as a promising research direction for daily health monitoring and supervision of patients with skin diseases. A critical issue with these solutions includes that good monitoring performance could only be achieved with unmixed heartbeat signal, while random body movements (RBM) and interference from respiration harmonics severely hinder the separation of heartbeat signal. In this paper, we propose a…

Sympathetic Nerve Hyperactivity in the Spleen: Causal for Nonpathogenic-Driven Chronic Immune-Mediated Inflammatory Diseases (IMIDs)?

Immune-Mediated Inflammatory Diseases (IMIDs) is a descriptive term coined for an eclectic group of diseases or conditions that share common … une-mediated or suspected to be autoimmune diseases. Another shared feature is dysregulation of the autonomic nervous system and hypothalamic pituitary adrenal (HPA) axis. Here, we focus on dysautonomia. In many IMIDs, dysautonomia manifests as an imbalance in activity/reactivity of the sympathetic and parasympathetic divisions of the autonomic nervous system (ANS). These …

Progression and prognosis in pure autonomic failure (PAF): comparison with multiple system atrophy.

OBJECTIVE: To clarify the progression of autonomic symptoms and functional deterioration in pure autonomic failure (PAF), particularly in comparison with multiple system atrophy (MSA). METHODS: The investigation involved eight patients with PAF (M/F = 7/1; mean age at onset, 57 years) and 22 with probable MSA matched for age at onset (M/F = 14/8; onset 56 years). Subjects were followed up for neurological symptoms, activities of daily living, and autonomic function for more than seven years. Autonomic functional tests were carried out. RESULTS: In PAF, fainting or sudomotor dysfunction occur

[Neuroplegia: results of an interdisciplinary approach to its physiopathology].

Neuroplegia was born from a physiopathological study of states of shock and research on inhibition of the autonomic reaction to aggression. Over the last 25 years, the experimental facts have become accumulated in favour of the first theory of H. Laborit, i.e. that this so-called defence reaction defended our lives only through conservation of motor activity in the environment. When this motor activity is inefficacious or useless, the neuroendocrine reaction may lead, during the acute phase, to states of shock and to chronic psychosomatic pathology. On this general theme, inhibition of this re

Haemodynamic responses during head-up tilt and tilt reversal in two groups with chronic autonomic failure: pure autonomic failure and multiple system atrophy.

Continuous haemodynamic responses to head-up tilt (HUT) and its reversal were studied in 21 subjects with sympathetic denervation due to primary chronic autonomic failure; 10 had pure autonomic failure (PAF; peripheral failure) and 11 had multiple system atrophy (MSA; central failure); 8 healthy subjects (controls) also were studied. Supine systolic, diastolic and mean arterial pressure (MAP) and total peripheral resistance (TPR) were highest in PAF. The MAP response to HUT and tilt reversal were different between groups. After HUT, MAP increased in controls (12+/-4 mmHg), but decreased in PAF

Impact of atrial fibrillation ablation on cardiac sympathetic nervous system in patients with and without heart failure.

BACKGROUND/OBJECTIVES: Catheter ablation of atrial fibrillation (AF) might influence the cardiac autonomic nervous system. To investigate the impact of catheter ablation on the sympathetic nervous function in AF patients with and without heart failure (HF) using cardiac iodine-123-metaiodobenzylguanidine ((123)I-mIBG) scintigraphy, and the association of this effect with AF recurrence. METHODS: Forty consecutive patients (median age, 65 (54-69) years; male, 29) with paroxysmal (n=22) and persistent (n=18) AF who were scheduled for ablation were enrolled. Twelve (30%) of these patients also ex…

Efficacy and Safety of Ultrasound-Guided Right-Sided Stellate Ganglion Block for Reducing the Severity and Duration of Acute Paroxysmal Sympathetic Hyperactivity in Patients with Acquired Brain Injury: A Randomized Controlled Clinical Trial.

Paroxysmal sympathetic hyperactivity (PSH) is a severe autonomic disorder following acquired brain injury for which there are no effective targeted therapies. The aim of this study was to evaluate whether ultrasound-guided right-sided stellate ganglion block (RSGB) can safely and effectively attenuate acute episodes of PSH. A prospective, single-blind randomized controlled trial was conducted. Eighty patients with PSH were randomized to receive either conventional care or RSGB combined with conventional care. Primary outcomes included PSH episode intensity and duration as assessed by the Clini

Long-lasting hyperalgesia and sympathetic dysregulation after formalin injection into the rat hind paw.

Subcutaneous formalin injection has been used extensively to evaluate acute effects (over several hours) of chemical nociceptive stimulation on nociceptive reflexes. Also, a persistent hyperreflexia for mechanical and thermal stimulation, lasting 3 weeks after formalin injection, has been revealed and related to microglial activation in the spinal dorsal horn. The present study demonstrates more prolonged effects of formalin injection, lasting 6 weeks, on operant escape from nociceptive thermal stimulation. Operant escape requires cerebral processing of nociceptive input and can detect effects

Chagas' heart disease as an experimental model for studies of cardiac autonomic function in man.

Patients presented here include apparently healthy persons who had diagnostic evidence of chronic cardiac Chagas' disease in the form of a positive complement-fixation test and an abnormal electrocardiogram. They had never been in heart failure. All were examined for autonomic cardiac function. Normal persons served as controls. Patients with Chagas' disease with sole involvement of hollow viscera were also included. In patients with cardiac Chagas' disease, failure of the heart rate to increase after administration of atropine and greatly reduced reflex changes in cardiac rate are believed to
