BACKGROUND: The traditional therapy for perforated sigmoid diverticulitis with peritonitis is emergency colectomy usually with colostomy. We report laparoscopic exploration with peritoneal lavage as an alternative in seven patients who required emergency surgery for diverticulitis.

Abdominal compartment syndrome is a serious and life-threatening condition that requires early recognition and urgent decompressive laparotomy. This case report describes an abdominal compartment syndrome due to a distended rectal stump. The patient had a previous sigmoid resection with colostomy performed for sigmoid volvulus. As far as we know, this is the first report of abdominal compartment syndrome due to rectal stump. In such cases, high index of suspicion and early intervention affect the clinical course.

Using alkaline phosphatase isozyme-specific immunocatalytical assays, the content of isozymes was determined in normal mucosas and adenocarcinomas from human colon or rectum. The authors report on three patients who had a perforated sigmoid diverticulum after unrelated surgery. If the preceding operation is remote in time from the perforation, detection is relatively easy, but if the perforation occurs shortly after major abdominal surgery, its presentation may be totally masked by the postoperative state. Awareness of this possibility may help alert the clinician to the presence of perforation of a sigmoid diverticulum in unexplained postoperative collapse. We report extensive colonic hemangiomatosis in two young adults. In the first, the hemangioma contiguously involved the entire colon. In the second, a large venous angioma affected the bladder, rectosigmoid, and descending colon. Venous angioma, a variant of hemangioma, is frequently confused with cavernous hemangioma because of their similar histologic appearance. The angiographic features, however, may enable a more accurate preoperative diagnosis and prevent unwarranted biopsy. We review and put into perspective pertinent previous reports of cavernous and venous angioma.

A review of patients with large-bowel cancer at a major New Zealand hospital showed an excess of right-sided colonic tumors in females compared with males. These observations stimulated analysis of the National Cancer Registry registration data for large-bowel cancer for the whole of New Zealand in the years 1972 to 1975 (4678 patients). Described in this paper is a large urinary fistula which developed 38 years after Wertheim's radical hysterectomy and percutaneous irradiation. The patient's condition was improved by supravesical urine discharge through anastomosis between ureter and sigma. Discussed are conditions required for adequate uretero-sigmoidostomy. Good therapeutic results may justify, in certain cases, uretero-sigma anastomosis, despite a history of irradiation and poor general condition.
