Seventy cases of sickle-cell disease were identified in the London Borough of Brent from records dating back to 1962. All but three were still alive and, with one exception, were recalled for confirmation of the diagnosis and to provide personal and family histories. Communicating hydrocephalus secondary to cerebral venous outflow obstruction has been reported in a few patients several months following removal of a TPN catheter. We report the case of an infant who developed this sequal 4 mo after the catheter was removed. We illustrate the techniques used to delineate the type of hydrocephalus. A lumbo-peritoneal shunt was performed to alleviate the condition.

Lumbar thecoperitoneal shunting was carried out in patients with communicating hydrocephalus due to long-standing tuberculous meningitis. At the time of this surgical procedure, the filum terminale was excised to achieve filum terminostomy. The central canal of the excised filum terminale in seven hydrocephalic children and an equal number from control cases was studied histologically. These observations indicate that the central canal of the filum terminale dilates in communicating hydrocephalus, and the dilatation is proportionate to the lateral ventricular enlargement.

A 5-year retrospective case review and 6-month clinical observation, in a teaching and general hospital in Bangkok, Thailand, show that colonic diverticular disease is not so common as in the West and has many distinctive features. Nine patients with "limited" (thoracic) small-cell lung carcinoma were treated with combined chemotherapy and radiotherapy (involved field and prophylactic whole brain). The history of vacuum extraction and its use in a specific community hospital setting is reviewed. Basic principles for successful use of the extractor are cited. Two hundred twenty-eight consecutive vacuum extraction cases occurring in an 18-month period are evaluated for indications and outcome. Trachoma represents the largest single cause of preventable blindness in the Black population of the Northern Transvaal. Epidemiological and cultural considerations indicate the necessity of community involvement in trachoma control at the household level. Involvement of the community by means of groups of voluntary health workers (care groups) has proved to be effective and acceptable to the local inhabitants. These groups, who form part of the comprehensive health system, integrate trachoma control into the primary health care activities of the area.

A house-to-house survey of blindness in an Indian rural community covering a population of 20 134 in 12 villages revealed a prevalence rate of 35 blind and 144 partially blind persons per 10 000 population. Blindness was significantly associated with the age, sex, marital status, occupation, and socioeconomic status of the respondents. Caratact, glaucoma, smallpox and trachoma were the main causes of blindness. Preventive measures can reduce the toll of blindness in such a community.
