The outcomes of 20 anthracite miners with coal workers' pneumoconiosis and culture-proved pulmonary tuberculosis treated with rifampin-containing chemotherapeutic regimens were determined by a retrospective review. Their mean age was 65 yr, and the duration of underground dust exposure averaged 27 yr. Nine miners had simple pneumoconiosis, 11 had progressive massive fibrosis, and 13 had cavitary disease; 3 also had extrapulmonary disease. Tobacco sensitivity is seen in atopic patients who are non-smokers but who have clinical symptoms on exposure to tobacco smoke. It is estimated that 8,000,000 persons with common allergies are also clinically sensitive to tobacco. Females outnumber males by about 4:1 and children are affected as well as adults. Allergic asthma is caused by antigen reaction with IgE fixed to mast cells of the bronchi. The reaction causes release of slow-reacting substance of anaphylaxis and histamine, which cause bronchial constriction. Prophylactic treatment includes avoidance of antigens, desensitization and use of cromolyn sodium. The latter appears to impede release of chemical mediators by the mast cell. Although of no value during an acute attack, this agent reduces attacks and reduces the need for other drugs in patients with severe perennial asthma.
