Hand-arm symptoms related to impact and nonimpact hand-held power tools.

Hand and arm symptoms among workers using impact and non-impact hand-held power tools were investigated in a cross-sectional study and a 5-y … rkers using low-frequency impact tools than workers using non-impact tools reported symptoms in the elbows and shoulders. Elbow symptoms were accentuated in the cross-sectional study, while shoulder symptoms were accentuated in the follow-up study. Wrist symptoms were reported by more of those working with high-frequency impact tools than of those using …

Radiological features in Brachmann-de Lange syndrome.

Brachmann-de Lange syndrome (BDLS) is a well-delineated disorder consisting variably of pre- and postnatal growth deficiency, microbrachycephaly, characteristic face, hypertrichosis, visceral anomalies, and limb defects consisting primarily of variable limb reduction defects, micromelia, and elbow abnormalities. The course is usually marked by initial hypertonicity, low-pitched weak cry, feeding problems, and behavioral problems with marked mental deficiency. In classical cases there is rarely any difficulty in making the diagnosis, but for mildly affected cases, it may be difficult to feel se…

[Evaluation of synovectomy in hemophilic arthropathy follow-up of 26 cases].

Thirty-five joint synovectomies performed between 1973 and 1986 in 26 patients with hemophilic arthropathy at the Department of orthopaedics, University of Colorado Health Sciences Center (UCHSC), USA, were reviewed. There were 15 knees, 15 elbows, 3 ankles and 2 shoulders. Arthroscopic synovectomy was performed on 8 knees. The remaining 27 synovectomies were performed by open surgical synovectomy. Twenty-one (60%) of the 35 joints have had no recurrent hemarthrosis reported, in 14 (40%) there were occasional episodes of bleeding. Among 9 (60%) of the 15 knees and 9 (60%) of the 15 elbows, no …

Medial Ulnar Collateral Ligament Tears: Reconstruction.

The medial ulnar collateral ligament (MUCL) complex is integral for valgus elbow stability, especially in individuals engaged in repetitive overhead activities such as throwing. MUCL injuries often necessitate surgical intervention to restore elbow stability. Early studies reporting outcomes after MUCL repair demonstrated suboptimal return to play compared with ulnar collateral ligament reconstruction, prompting a shift toward reconstruction techniques. The first widely popular MUCL reconstruction technique was the Jobe technique. Many reconstruction techniques have since been described. Despi…

Plating of Proximal Ulna Fractures Using Posterolateral Distal Humerus Plates: Surgical Technique and Case Series.

Precontoured olecranon plates are frequently used in the management of proximal ulna fractures. Occasionally, in comminuted proximal ulna fractures or segmental ulna fractures, available precontoured olecranon plates are too short for the management of these fractures. The authors have utilized posterolateral distal humerus plates in these instances. The coronal bend in some posterolateral distal humerus plates anecdotally fits well to the proximal ulna, despite being designed for the distal humerus. We sought to measure the coronal angulation of precontoured posterolateral distal humerus plat

Accuracy of MRI and X-Ray Measurement of Displacement Distance of Humeral Lateral Condyle Fractures.

OBJECTIVE: To investigate the accuracy of X-ray and magnetic resonance imaging (MRI) measurements in evaluating the displacement of humeral lateral condyle fracture (HLCF) in different positions of the forearm based on human cadaveric HLCF models. METHODS: Three human cadaveric elbow HLCF fracture models were successfully established. The wrist joint was fixed, and the forearm was rotated forward along the mid-axis. The maximum distance between the two segments of the lateral fracture gap was defined as LFS (lateral fracture space) distance, and the maximum distance between the two segments o…

Open reduction of radial neck fractures in children: injury severity predicts the radiographic and clinical outcomes.

BACKGROUND: Radial neck fractures are the third most common elbow fracture in children. Open reduction may be required if closed or mini-open techniques are not successful in reducing the fracture. Previous reports on open reduction have noted poor outcomes and complications with this treatment approach. However, it is unknown whether it is the open procedure itself or the severity of the initial injury that leads to the poor results. The purpose of this study was to evaluate the correlation between intraoperative findings at the time of open reduction of radial neck fractures and the clinical…

Pyrocarbon unipolar radial head prosthesis: clinical and radiologic outcomes at long-term follow-up.

BACKGROUND: Several studies have already reported good short-term results with a pyrocarbon unipolar radial head prosthesis (Pyc-uRHP). The aim was to evaluate the evolution from mid- to long-term clinical and radiographic outcomes of a Pyc-uRHP. METHODS: This was a retrospective, single-center study. We followed up all the patients who underwent Pyc-uRHP surgery in our original study at 2 years of follow-up (52 patients), reaching a minimum of 7 years of clinical and radiologic follow-up. This study included 26 patients who underwent a clinical examination assessing mobility, the Mayo Elbow …

The pediatric proximal ulna: a radiographic study.

INTRODUCTION: The proximal ulna has been comprehensively described in the anatomic literature and imaging studies. However, to the best of our knowledge, the anatomy of the proximal ulna in children has not been fully described in the literature. METHODS: The present study was conducted on 189 children aged between 0 and 12 years (4.7 ± 2.7) by analysis of lateral X-rays of the forearm. Proximal ulna dorsal angulation (PUDA), tip-to-apex (TTA), and total ulnar length (TUL) were measured. The correlation between age and the various X-ray measurements was recorded and then compared with gender

Triple modified French osteotomy: a possible answer to cubitus varus deformity. A technical note.

BACKGROUND: Cubitus varus is the most common delayed complication of pediatric supracondylar humerus fractures. We developed a new technique, the triple modified French osteotomy, that we believe may be the answer to this common but yet unsolved deformity. MATERIALS AND METHODS: Ten patients aged between 6 and 12 years with post-traumatic cubitus varus deformity were operated on with the triple modified French technique. A varus angle of more than 10° measured on the radiograph was an indication for surgery. RESULTS: The radiologic union at the osteotomy site took place in a mean period of 5

Proximal forearm fractures.

Unfortunately, the literature has little guidance for revision elbow surgery. This article attempts to supplement what is known in the literature with the author's anecdotal experience. With this article, it is the author's hope that the reader may learn from his or her successes and his or her failures without having to discover them first hand. There is good reason for angst to overcome surgeons looking at radiographs depicting a traumatized proximal ulna or radius. Surgeons know that there is a good chance they will be seeing these patients for a long time.

Pediatric posterior monteggia lesion: a greenstick fracture of the proximal ulnar metaphysis with radial neck fracture. A case report.

We report a case of posterior Monteggia lesion in a 12-year-old girl consisting of a greenstick fracture of the proximal ulnar metaphysis and a radial neck fracture with a proximal and posterior 90-degree rotational displacement of the radial head. Closed reduction of the radial head resulted in complete reversal of the radial head, which was corrected by open reduction and internal fixation. Cast immobilization without correction of the plastic deformation of the proximal ulna caused posterior bowing of the ulna and posterior displacement of the fractured radial neck. Corrective osteotomy of

Epiphysiolysis of the capitellum humeri.

A four-year-old girl presents at the emergency department with left elbow pain after a fall. X-ray demonstrated an epiphysiolysis of the capitellum humeri, without a bony fragment, the so-called Salter Harris Type 1 fracture. The fracture was treated by open reduction and internal fixation. This case is unusual because avulsions of the capitellum are usually associated with a bony fracture.

Arthroscopic-assisted biceps tenodesis for ruptures of the long head of biceps brachii: The cobra procedure.

A number of open procedures have been presented in the literature that described the repair of the ruptured long head of biceps brachii (LHBB). Although arthroscopic biceps tenodesis techniques have been used to address partial tears or subluxation of the biceps, no arthroscopic technique to assist in the treatment of complete retracted ruptures of the LHBB has been described. This article describes an arthroscopic-assisted biceps tenodesis, using interference screw fixation, in the treatment of acute or chronic LHBB ruptures. An arthroscopic-assisted biceps tenodesis with interference screw f

Arthroscopic treatment of capitellum fracture of the humerus.

This case report presents a type I Hahn-Steinthal capitellum fracture treated by screw fixation under arthroscopic control. It is the first published case of the use of this technique. The 3 types of capitellum fractures and different methods for treatment are described: functional treatment, open surgery, and arthroscopic surgery. The arthroscopic approach allows a precise reduction and fixation of the articular fragment with a better evaluation of associated lesions compared with the open surgery. Arthroscopic fixation minimizes the damage to periarticular soft tissues and has a lower morbid

Cubitus varus--correction by distraction osteogenesis.

Cubitus varus deformity in thirty children was corrected by gradual medial opening wedge osteotomy. The osteotomy was stabilized and distracted by simple external fixator-cum-distractor. The patients of both sexes and age from 6 to 14 years, had varus deformity from 17 degrees to 43 degrees. We achieved good to excellent results in 28 cases as regards to correction of deformity and range of motion at elbow joint. None of the cases had neurological deficit or permanent stiffness. One case had fair result due to under-correction and another case had a poor result as osteotomy united before corre…

Radial neck osteolysis after annular ligament reconstruction. A case report.

The treatment of the dislocated radial head in a late Monteggia lesion remains a matter of controversy. Annular ligament reconstruction using a strip of triceps tendon is a commonly used technique. Although most published series report good and excellent results, the overall complication rate of this procedure is not known. This study reports a 9-year-old boy who had avascular necrosis and bone resorption of the radial neck after a late annular ligament reconstruction using a sling of triceps tendon for a radial head dislocation that was not detected previously.
