Cox regression analysis of multivariate failure time data: the marginal approach.

Multivariate failure time data are commonly encountered in scientific investigations because each study subject may experience multiple events or because there exists clustering of subjects such that failure times within the same cluster are correlated. In this paper, I present a general methodology for analysing such data, which is analogous to that of Liang and Zeger for longitudinal data analysis. This approach formulates the marginal distributions of multivariate failure times with the familiar Cox proportional hazards models while leaving the nature of dependence among related failure tim

An exploratory analysis of survival with AIDS using a nonparametric tree-structured approach.

We illustrate an analysis with classification and regression trees applied to survival data. Through this application, we provide a description of the opportunistic diseases and sociodemographic factors that contribute to survival among people with human immunodeficiency virus disease. The analyses are based on 43,795 cases reported to the Centers for Disease Control between January 1, 1984, and December 31, 1987. We used vital status as of December 31, 1989, to estimate mortality rates. We identified Kaposi's sarcoma and opportunistic diseases causing central nervous system damage (cryptococc

House modifications using insecticide-treated screening of eave and window as a vector control tool: evidence from a semi-field system in Tanzania and simulated epidemiological impact.

BACKGROUND: Gaps in unimproved house structures, especially in eaves and windows, allow mosquito entry, increasing indoor vector-borne disease transmission. Simple modifications to such houses may reduce human exposure, and insecticide treatment may kill mosquitoes, benefiting all community members. This study evaluated insecticide-treated screening (ITS) for eaves and windows, incorporated with deltamethrin and piperonyl butoxide (PBO), compared to a permethrin and PBO-treated bednet in Tanzania. METHOD: A randomised Latin-square design (4 × 4) was used in four experimental huts within a lar

A LASSO-based nomogram for predicting breast cancer-specific survival in elderly patients after surgery: a large retrospective cohort study.

BACKGROUND: Accurate prognostic assessment for elderly patients with breast cancer remains challenging, as the conventional tumor-node-metastasis (TNM) staging system is limited in capturing individual heterogeneity. This study aimed to develop and internally validate a least absolute shrinkage and selection operator (LASSO)-Cox-based nomogram for predicting breast cancer-specific survival (BCSS) in elderly patients. METHODS: A total of 69,137 eligible patients diagnosed between 2010 and 2015 were identified from the Surveillance, Epidemiology, and End Results (SEER) database and randomly div

Predicting high lymph node positivity risk factors in nasopharyngeal carcinoma patients: A multi-model approach.

Identifying patients at high risk of an elevated lymph node ratio (LNR) is critical for optimizing the management of nasopharyngeal carcinoma (NPC), as LNR, defined as the ratio of metastatic to examined lymph nodes, serves as a key prognostic indicator. This retrospective observational study aimed to investigate the epidemiology and influencing factors associated with high LNR in NPC patients. Various machine learning algorithms were employed to select independent predictive variables, and both univariate and multivariate Cox regression analyses were conducted to develop predictive models. PEStimate: predicting offspring disease risk after polygenic embryo screening.

MOTIVATION: Polygenic embryo screening (PES) is a new, controversial technology, whereby human in vitro fertilization embryos are screened for their genetic risk of complex, polygenic diseases. PES aims to reduce the disease burden in offspring by prioritizing the selection of low-risk embryos. However, given that polygenic diseases are usually late-onset, PES outcomes must be estimated by epidemiological modeling. The liability threshold model has been previously used to predict outcomes. However, predictions rely on complex sets of equations, some of which require numerical integration or si

The Role of Host Immunity and the Environment in Seasonal Disease Dynamics.

In both human and wildlife disease systems, temporal shifts in host immunity may shape the timing and severity of epidemics. Yet, immune responses, as well as seasonal patterns in their expression, are difficult to measure. Rather, field studies collect phenomenological data on infection outcomes. Pairing epidemic data of multiple outbreaks with models that directly parameterize immune metrics can be a powerful approach for exploring the role of time-varying immunity on disease. Field data can be used to determine how well a parameterized model can reproduce trends and differences observed amo

Environmental VOC exposure and mental health.

Volatile organic compounds (VOCs) are major atmospheric pollutants with potential neurotoxic effects, yet their impacts on population-level mental health remain understudied. Here, we develop an integrative framework that combines the Community Multiscale Air Quality (CMAQ) model, epidemiological analysis, and network toxicology to assess the mental health risks of ambient VOC exposure across China. We simulated the spatiotemporal distribution of 33 VOC species at a 36 km × 36 km resolution using CMAQ, driven by the Weather Research and Forecasting (WRF) model. Ground-based observations and sa

Risk of second primary malignancies in prostate cancer patients: A Surveillance, Epidemiology, and End Results population-based study.

OBJECTIVE: Second primary malignancy (SPM) is becoming a challenge in clinical practice. However, limited studies have focused on this issue in patients with prostate cancer (PCa). We sought to explore the potential risk of SPMs in patients with a prior diagnosis of PCa and construct a model to predict the risk of developing SPMs. METHODS: We retrospectively extracted data on PCa patients from the Surveillance, Epidemiology, and End Results database between 2000 and 2018. A competing-risk model was established to explore the risk factors for developing SPMs and to predict the probability of d

Estimating the value of face coverings during the COVID-19 epidemic: a dynamic causal modelling study.

INTRODUCTION: The value of face coverings in the pandemic has been disputed. Dynamic causal modelling (DCM) allows one to test hypotheses about the nature of viral spread and effectiveness of interventions. Our aim was to quantify the impact of face coverings on viral transmission and deaths associated with COVID-19 between 2020 and 2023 in the UK. METHODS: A DCM of the COVID-19 outbreak in the UK was used. It combines conventional (ie, Susceptible Exposed Infectious Recovered (SEIR)) epidemiological and sociobehavioural models to simultaneously fit measures of prevalence, clinical data, vacc

Testosterone modulates respiratory control but does not exert neuroprotective effects in a Parkinson's disease model.

Parkinson's disease (PD) is a neurodegenerative disorder characterized by the loss of dopaminergic neurons in the substantia nigra (SN). Patients with PD exhibit classic motor symptoms such as tremors and bradykinesia, as well as non-classical symptoms, including respiratory dysfunctions, which significantly contribute to increased mortality. Epidemiological studies suggest that men are more frequently affected by PD, although the role of testosterone remains unclear. Therefore, this study aimed to investigate the role of testosterone in modulating respiratory activity and respiratory nuclei u

Impact of detection rate and preventive treatment of latent tuberculosis infection on the future burden of tuberculosis among students in shanghai: a predictive study using Markov modeling.

BACKGROUND: Students represent a key demographic for tuberculosis (TB) control in China. This study utilizes a dynamic Markov model to forecast the epidemiological trends of active tuberculosis (ATB) among students in Shanghai through 2035 and evaluate the effectiveness of different control strategies. METHODS: A Markov model incorporating six distinct states of TB transmission was utilized to simulate disease dynamics within a hypothetical cohort of 100,000 individuals. The model assessed the prevalence of latent tuberculosis infection (LTBI) and ATB, categorizing the cases based on whether t

Assessment of Vaccination Impact in PPR-Control Program Implemented in Southern States of India: A System Dynamics Model Approach.

Mass vaccination against peste des petits ruminants (PPR) in two southern states of India, namely Andhra Pradesh and Karnataka, has reduced disease outbreaks significantly. The sporadic outbreaks reported now can be attributed in part to the recurring movement of sheep and goats between these contiguous states. This study assessed the present level of economic burden and impact of vaccination on the local system (one state), considering the exposure from the external system (neighboring state) using a system dynamic (SD) model. The SD model relies on interdependence, interaction, information f

Dengue dynamics in Nepal: A Caputo fractional model with optimal control strategies.

An infectious disease called dengue is a significant health concern nowadays. The dengue outbreak occurred with a single serotype all over Nepal in 2023. In the tropical and subtropical regions, dengue fever is a leading cause of sickness and death. Currently, there is no specified treatment for dengue fever. Avoiding mosquito bites is strongly advised to reduce the likelihood of controlling this disease. In underdeveloped countries like Nepal, the implementation of appropriate control measures is the most important factor in preventing and controlling the spread of dengue illness. The Caputo
