A 43-year-old Hispanic woman presented to the clinic complaining of fever, chills and cough for 14 days. The patient reported a recent trip to Asia 12 days prior to presenting symptoms. Cardiac tamponade in a patient with severe dengue fever.

A 26-year-old postpartum female presented with symptoms characteristic of dengue fever on the 16th day of puerperium. On the third day of the illness, the patient presented a clinical picture consistent with shock. Tests determined primary infection with dengue virus serotype 2. Cardiac tamponade was confirmed by echocardiography. This rare manifestation is described in a patient without any associated comorbidity.

The increasing migration into Europe may confront clinicians with diseases barely known any more in this part of the world. We present a case of louse-borne relapsing fever in a Somali refugee. Blood smears led to the diagnosis of relapsing fever and DNA sequencing was positive for Borrelia recurrentis. This disease should be considered in all patients with unclear fever and a compatible travel or migration history. Blood smears are employed as the primary diagnostic method. Therapy harbors the danger of a Jarisch-Herxheimer reaction, a complication that may require intensive medical care.

Encephalitis in a traveller with typhoid fever: efficacy of corticosteroids.

Typhoid fever is a bacterial infection caused by Salmonella typhi or S. paratyphi, recognized as a classical cause of fever in returning travellers. However, neuropsychiatric presentations are rarely reported in travellers diagnosed in western countries, whereas they are more commonly described in patients treated in endemic areas. We describe such a case and discuss the pathophysiologic mechanisms of this complication.

Fever in a visitor to Canada: a case of mistaken identity.

We report a case of babesiosis in a traveler from India who was diagnosed with malaria on the basis of blood smears. Pan-Plasmodium PCR was positive, though species-specific assays were negative. Reexamination of blood smears and Babesia-specific PCR confirmed babesiosis. We highlight the overlapping clinical and diagnostic features of malaria and babesiosis and the potential cross-reactivity of Plasmodium primers in cases of babesiosis.
