Blast energy-induced traumas usually result in some type of amputations of lower extremities. It is very hard to determine the amputation level of the feet of these cases at first, and secondary amputation stump revisions by bone shortening are often necessary. Among partial foot amputation levels, Chopart level is the most critical. Four male patients (20 to 24 years old) with modified Chopart amputation due to mine explosion injury have had skin-grafted amputation stumps where troublesome, recurrent unstable wounds had developed. These amputation stumps were electively reconstructed with neu…

In a prospective study of 2,401 newborns, the incidence of foot deformities was 4%, of which three fourths were various forms of adductus anomalies. At age 16 years, the children with a foot deformity and a random sample of those with normal feet as newborns were examined by dynamic foot pressure (EMED) and gait analysis (Vifor). The natural course of all congenital foot deformities, except clubfoot, was favorable. The importance of monitoring the children during the entire growth period is shown by the fact that at age 6 years, 87%, and at 16 years, 95% of the adductus deformities had resolve…

Bilateral weightbearing anteroposterior and lateral radiographs were analyzed in 43 patients with a clinical diagnosis of posterior tibial tendon insufficiency by two independent observers. The radiographs of each patient's asymptomatic foot was considered to serve as the control for the preexistance of pes planus. The radiographs were assessed for the severity of flatfoot deformity, the degree of talonavicular coverage, the talometatarsal angle, the calcaneometatarsal angle, the cuneiform to ground height and the cuneiform to fifth metatarsal height. Interobserver correlations were assessed. …

Sixty-three patients with rigid equinovarus contractures of the foot following ischemic episodes in the lower leg were treated at our institute from 1983 to 1994 by lengthening the Achilles tendon and the tendon of the m.tibialis posterior, release of the tendons of the m.flexor digitorum longus and the m.flexor hallucis longus and release of the dorsal capsule of the ankle joint. Patients with an equinus deformity greater than 20 degrees, with an additional hind foot varus deformity of more than 5 degrees and/or malrotation of the midfoot were not eligible for this procedure. The initial equi…

INTRODUCTION:: Foot pressure measurements furnish information about distribution of pressures, forces, time and contact areas under the foot during standing and walking. Foot pressure measurement has been used in a number of rehabilitation and athletic applications in adults, however, little has been published regarding the clinical usefulness of this technology for children with disabilities. Children with juvenile rheumatoid arthritis (JRA) are reported to have various foot deformities and gait deviations and clinicians report that the children may have foot pain such as metatarsalgia. These…

We have examined the variability in function measurements of three sensory foot nerves in neuropathic diabetic patients and have compared them to measurements from healthy non-diabetic subjects. Sixty-six healthy, non-diabetic subjects (30 (45%) males, mean age 56 years (range, 21-84 years)) and 61 age and sex matched diabetic patients (33 (54%) males, mean age 55 years (range, 34-78 years) Type 1 diabetes mellitus (DM), mean duration of DM 24 years (range, 2-48 years)) were tested. Current perception threshold (CPT) at 250 Hz was employed to test the sensory function of three nerves: superfic…

Ectrodactyly (split hand/split foot malformation, SHSF) is a human limb malformation characterized by absent central digital rays, deep median cleft, and syndactyly of remaining digits. The disorder is genetically heterogeneous, with at least two loci thus far determined: an autosomal locus at 7q21 designated SHFM1 and an X-linked locus at Xq26 designated SHFM2. Cytogenetic analysis of sporadic SHSF patients and linkage studies in extended pedigrees both suggest more than one autosomal locus exists. We report a novel SHSF locus suggested by a stillborn infant with ectrodactyly and other malfor…

Split hand/split foot (SHSF; also known as ectrodactyly) is a human developmental disorder characterized by missing central digits and other distal limb malformations. An association between SHSF and cytogenetically visible rearrangements of chromosome 7 at bands q21-q22 provides compelling evidence for the location of a causative gene at this location, and the locus has been designated SHFD1. In the present study, marker loci were localized to the SHFD1 critical region through the analysis of somatic cell hybrids derived from individuals with SHSF and cytogenetic abnormalities involving the 7…

We reviewed 19 children with 24 congenital club feet at a mean of 11 years after one-stage posteromedial release at the age of five years or older (mean 6.8 years). Thirteen feet had undergone previous surgery. Nineteen feet were functionally excellent or good, three were fair and two had required subtalar arthrodesis. Radiographs showed good alignment of the tarsal bones, although mild adduction or varus deformity remained in several feet. Deformities of the bones were more common in feet which had had previous surgery.

We report two patients with treated pituitary gigantism and peripheral neuropathy, one of whom has chronic foot ulceration. Detailed neurophysiological assessment was performed on both patients. The patient with foot ulceration had clinical and neurophysiological evidence of severe neuropathy, whereas the patient without ulceration had only neurophysiological abnormalities. The sweating response to acetylcholine was markedly impaired in the feet of both patients, suggesting pedal autonomic denervation. Neither patient had evidence of diabetes mellitus and detailed investigation failed to revea…

Some days after vaccination against foot-and-mouth disease a high degree of salivation, trembling of the muscles, colic, hyperemization of the mucosa of the mouth, extreme thirst, and losing of hairs at the end of the tail occurred in cattle. The cause of these symptoms seems not to be the vaccination, but the content of thallium in silage (41 mg/kg) fresh matter, which was bought from abroad. The diagnosis was confirmed by analysis of thallium in several organs (liver, kidney, muscles).

This study examined the effect of conventional plastic bilateral ankle-foot orthoses (AFOs) on the energy expenditure of ambulation in spastic diplegic cerebral palsy patients. Eighteen subjects (aged 8.3 +/- 2.83 years) ambulated five minutes at freely chosen velocities with and without their AFOs. Heart rate (beats per minute) and distance (meters) ambulated were recorded for each minute. A physiologic cost index was calculated by determining the ratio of walking heart rate to velocity [(HRamb-HRrest) divided by meters per minute]. The mean physiologic cost indexes for the last three minutes…

Examinations of 18 patients with mycoses of the foot complicated by allergic processes, have revealed elevated blood serum concentrations of histamine, serotonin, cathepsin D, and acid phosphatase, and a lowered activity of monoamine oxidase, as compared to 22 patients with mycoses not complicated with allergic manifestations. This fact reflects one of the aspects of a pathogenetic difference between these patients and necessitates combined therapy to correct the detected abnormalities.

Diplopodia with the duplicated foot tucked up posteriorly, was reconstructed successfully in an 8-year-old boy. The knee joint, fibula and tibia were uninvolved; he had been walking on the dorsum of the foot wearing an ordinary shoe with the heel forward. The foot had ten toes, nine metatarsals and nine tarsal bones including one talus and one large calcaneus on which two Achilles tendons were inserted into two tuberosities. The lateral supernumerary foot was excised and the medial foot was dorsiflexed by almost 180 degrees by massive release and elongation of tendons. He could walk on his bar…

A case of simultaneous aspergillosis and mucormycosis complicating diabetic foot gangrene is reported. The recognition of this combined opportunistic fungal infection was brought about by an initial finding of birefringent calcium oxalate crystals associated with a background of acute inflammatory exudate, which is considered a strong clue to the diagnosis of infection caused by Aspergillus niger.

There exists considerable confusion in the definition and classification of essential flat foot in the child. In an attempt to put an end to this confusion, the authors define the radiological characteristics and course of the abnormality. The latter is defined radiologically on lateral films of the foot whilst weight bearing by study of the angle of the calcaneum over the horizontal (Angle C) and by the measurement of the angle of talo-calcaneal divergence (Angle T.C.D.). Three groups may be distinguished on this basis: Group I - of minimal gravity; Group II - which may correct itself spontan…
