[Relationship between auditory hallucination and regional homogeneity of functional magnetic resonance imaging in first-episode childhood and adolescence-onset schizophrenia].

Objective: To investigate the characteristics of regional homogeneity (ReHo) of the resting-state magnetic resonance imaging (fMRI) in first-episode childhood and adolescence-onset schizophrenia (CAOS) and to analyze the relationship between the severity of auditory hallucinations and ReHo. Methods: Seventy-nine cases of first-episode CAOS patients (case group) aged 10 to 16 were collected from October 2017 to December 2019 in the Second Affiliated Hospital of Xinxiang Medical University. There were 28 males and 51 females with a mean age of (14.1±1.3) years. And meantime, 32 healthy children

Primary intracranial myxopapillary ependymoma: two case reports and literature review.

Myxopapillary ependymoma (MPE), a histological variant of ependymoma, is characterized by a radial arrangement of spindled or epithelioid tumor cells surrounding blood vessels, accompanied by perivascular myxoid changes and microcyst formation. MPE typically occurs in young adults, predominantly in the conus medullaris, cauda equina, and filum terminale. However, primary intracranial MPEs, especially those arising in the cerebral parenchyma, are exceptionally rare. Here, we present two cases of isolated MPEs located in the frontal lobe to underscore their extraordinary rarity. We provide a com…

Did Joseph Conrad have juvenile myoclonic epilepsy?

Joseph Conrad's epilepsy is well documented but has received little attention as he had convulsive seizures only in childhood and adolescence. The type of epilepsy has never been discussed. His biography reveals that his condition was decidedly neuropsychiatric with depression, a suicidal attempt, and prominent signs of frontal lobe dysfunction, as is seen typically in juvenile myoclonic epilepsy. This diagnosis is supported by a congruent family history and probable lifelong myoclonic seizures including reflex myocloni that were misunderstood as nervosity. It is impressive to see how he disci…

Unforeseen hazards: cranial penetration with a metallic chopstick in a suicide attempt.

A 71-year-old man attempted suicide by self-inflicting a cranial injury with a metallic chopstick following a family dispute. CT imaging showed penetration through the frontal sinus and lobe, yet the patient experienced no significant neurological deficits. Mini-craniotomy revealed a dural deficit without active bleeding. The favorable outcome is attributed to anatomical factors and the chopstick's limited penetrative ability. Successful recovery was achieved through minimal debridement and appropriate antibiotic therapy. This case highlights both the dangers of common objects and the importan

A Case of High-Grade Glioma in an Eloquent Area Treated with Awake Craniotomy in an 85-year-old Patient.

An 85-year-old woman presented with aphasia due to an occupying lesion in the left frontal lobe near the language area. Complete resection of the contrast-enhancing lesion was performed under awake conditions. The pathological diagnosis was anaplastic astrocytoma, and postoperative radiochemotherapy was administered. Awake surgery is a useful technique to reduce postoperative neurological sequelae and to maximize surgical resection. Although the patient was elderly, which is generally considered high risk, she did not have any severe neurological deficits and had a good outcome. Even in the ex…

Rhinocerebral Mucormycosis With Brain Abscess Presenting as Status Epileptucus in a COVID-19-Infected Male: A Calamitous Complication.

With the evolution of COVID-19 disease, the emergence of more complications associated with COVID-19 is taking place. Mucormycosis is the most common opportunistic fungal infection encountered after COVID-19. In this case report, we describe a case of a 61-year-old male type 2 diabetic with sino-orbital-cerebral mucormycosis infection who was treated with conventional guidelines for a recent COVID-19 infection and further presented with generalized tonic-clonic status seizures. Neuroimaging revealed sino-orbital mucormycosis with right frontal lobe abscess. He was treated with anti-epileptics,…

Discovery of the First Orally Available, Selective KNa1.1 Inhibitor: In Vitro and In Vivo Activity of an Oxadiazole Series.

The gene KCNT1 encodes the sodium-activated potassium channel KNa1.1 (Slack, Slo2.2). Variants in the KCNT1 gene induce a gain-of-function (GoF) phenotype in ionic currents and cause a spectrum of intractable neurological disorders in infants and children, including epilepsy of infancy with migrating focal seizures (EIMFS) and autosomal dominant nocturnal frontal lobe epilepsy (ADNFLE). Effective treatment options for KCNT1-related disease are absent, and novel therapies are urgently required. We describe the development of a novel class of oxadiazole KNa1.1 inhibitors, leading to the discover…

Supratentorial ependymoma with YAP1:FAM118B fusion: A case report.

We report a case of a 26-year-old Chinese man who had experienced three grand mal seizures in the past two months. Magnetic resonance imaging revealed a relatively well-circumscribed lesion in the left frontal lobe. A craniotomy with total excision of the tumor was performed. Histopathological investigations confirmed a grade 2 ependymoma according to the World Health Organization classification. Genetic analysis revealed a tumor harboring FAM118B fusion to YAP1, and no other genetic alterations or methylation of the O6 -methylguanine-DNA methyltransferase gene promoter were detected. This is …

Intraoperative Rupture of Unruptured Cerebral Aneurysm during Craniotomy: A Case Report.

An unruptured aneurysm was incidentally found in the right middle cerebral artery in a 67-year-old woman. During an attempt to turn the temporalis muscle for surgical clipping, systolic blood pressure suddenly increased. After opening the dura mater, we found a subarachnoid hemorrhage and severe brain swelling. We promptly expanded the craniotomy area to reach the aneurysm while pulling part of the frontal lobe to apply a clip. We retrospectively analyzed the aneurysm using computational fluid dynamics. Our analysis suggests that the rupture of the aneurysm occurred at a location with very low…

False-positive indium 111 pentetreotide scan for recurrent meningioma due to radiation fibrosis.

A patient with recurrent meningioma in the right frontal lobe, treated with resection and radiation, had a routine magnetic resonance imaging scan that was suspicious for recurrent disease. Follow-up Octreoscan showed moderately increased focal uptake in same region, compatible with recurrence of meningioma; however, the histopathology was consistent with radiation fibrosis. There are reported cases of uptake on Octreoscans at other sites of the body due to chronic inflammation, but only one other case has been reported in the brain. Caution must be taken in interpretation of brain tumors on O…

[Intracranial artifact un magnetic resonance caused by embolization of microscopic metallic fragment].

Magnetic resonance imaging (MRI) artifacts have been described in many articles published in the literature. We present the case of a 72 year old man who was admitted due to left inferior limb weakness. MRI imaging demonstrated an acute right infarction of the right posterior cerebral artery. In the right frontal lobe a metallic artifact was demonstrated as well. The artifact showed a central signal void and it was surrounded by a high signal intensity rim. His aortic valve had been replaced 12 years earlier. This indicates that the artifact is in fact due to a small metallic fragment emboliza…

Hemiplegia caused by inadvertent intra-carotid infusion of total parenteral nutrition.

A 24 year-old woman developed acute hemiplegia and a seizure following accidental catheterization of the right common carotid artery and total parenteral nutrition infusion. Magnetic resonance imaging of the brain showed lesions in the frontal lobe and putamen consistent with an ischemic stroke. Angiography through the central venous catheter confirmed its intra-arterial location. The patient's weakness improved after hyperbaric oxygen treatment. We concluded that stroke or seizures during total parenteral nutrition administration through a central venous catheter should alert one to the possi…

[Marchiafava-Bignami disease: development with flare-up].

Marchiafava-Bignami disease is a complication of chronic ethanol intoxication. The clinical course may be acute, rapidly leading to death, or marked by a progressive dementia. A form with favorable outcome is also described. During the evolution of chronic forms, acute worsenings are possible, with subsequent partial recovery. We report a clinico-radiological case, where a first episode was followed by a good outcome. After a second episode, 3 years later, the patient developed a dementia with predominant frontal lobe signs. This case suggests that, even after a favorable outcome, a long-term …

The bacteriology of brain abscess: a local experience in Malaysia.

31 cases of intracranial abscess seen over a period of 10 years showed a peak incidence in the second and third decades of life with a male preponderance. Tetralogy of Fallot and other congenital cyanotic heart diseases were the predominant associated factors (32%). The commonest site of infection was the frontal lobe. Gram-stained smears of pus proved to be extremely useful. The majority of the organisms (82%) were either microaerophilic or anaerobic bacteria with Streptococcus milleri being the most frequent isolate. With the exception of Corynebacterium species, all isolates were susceptibl…

Hodgkin's disease presenting with isolated craniospinal involvement.

A 34-year-old man presented with tumor of his cervical spinal meninges and bone and the dura over the right frontal lobe, which was shown on biopsy to be Hodgkin's lymphoma. Extensive noninvasive evaluation failed to reveal any other sites of disease. Central nervous system involvement with Hodgkin's disease is extremely rare and is almost always a late complication in patients who have widely disseminated disease in the usual nodal sites. The current patient is unique in that his initial symptoms were due to involvement of the central nervous system and he had no evidence of Hodgkin's disease…
