Arthroscopic debridement alone for intercarpal ligament tears.

This study examined the role of arthroscopic debridement alone for complete and incomplete intercarpal ligament tears of the wrist. Forty-three wrists underwent arthroscopic evaluation for persistent wrist pain and were identified as having isolated scapholunate or lunotriquetral ligament tears treated by arthroscopic debridement alone of the torn ligament edges. At follow-up examination at an average of 27 months, 29 (66%) wrists having a complete scapholunate ligament tear and 36 (85%) wrists having a limited scapholunate ligament tear had either complete symptom resolution or improved sympt

Early mobilization of fractures of the metacarpals and phalanges.

Early mobilization of fractures of the metacarpals and phalanges is advocated. It should begin as soon as the stability of the fracture and the condition of the soft tissues permits. It has to be adapted to each case, and often consists of mobilization of the distal phalanges with the wrist and metacarpophalangeal articulations immobilized in a protective position. It is effective in preventing stiffness. Because it is a specific technique, it must be entrusted to specially trained physiotherapists, and requires regular and repeated clinical and radiologic follow up by the surgeon.

Preliminary validation of a web-based MRI scoring system for children with chronic nonbacterial osteomyelitis (ChRonic nonbacterial Osteomyelitis Magnetic Resonance Imaging Scoring: CROMRIS).

BACKGROUND: The ChRonic nonbacterial Osteomyelitis Magnetic Resonance Imaging Scoring (CROMRIS) tool was developed to assess specific characteristics of bone and soft tissue inflammation on MR images of patients with CNO; however, this tool was labor intensive to utilize. We aimed (1) to refine and adapt this scoring method, (2) to assess the usability of this web-based CROMRIS system among radiologists and (3) to evaluate the absolute agreement of the components and summary CROMRIS scores at each body site, and the interrater reliability. METHODS: We used a qualitative, user-centered design

Proximal humeral fractures: the role of calcium sulphate augmentation and extended deltoid splitting approach in internal fixation using locking plates.

UNLABELLED: The aim of our study is to analyse the results of our surgical technique for the treatment of proximal humeral fractures and fracture dislocations using locking plates in conjunction with calcium sulphate bone-substitute augmentation and tuberosity repair using high-strength sutures. We used the extended deltoid-splitting approach for fracture patterns involving displacement of both lesser and greater tuberosities and for fracture-dislocations. Optimal surgical management of proximal humeral fractures remains controversial. Locking plates have become a popular method of fixation. H

Giant cell tumor of bone: a neoplasm or a reactive condition?

Giant cell tumor of bone (GCTB) is a benign but locally aggressive bone tumor of young adults. It typically presents as a large lytic mass at the end of the epiphysis of long bones. Grossly it is comprised of cystic and hemorrhagic areas with little or no periosteal reaction. Microscopically areas of frank hemorrhage, numerous multinucleated giant cells and spindly stromal cells are present. Telomeric fusions, increased telomerase activity and karyotypic aberrations have been advanced as a proof of its neoplastic nature. However such findings are not universal and can be seen in rapidly prolif

Modifications of biochemical markers of bone and collagen turnover during corticosteroid therapy.

Serum bone Gla protein (BGP), carboxyterminal cross-linked telopeptide of type I collagen (ICTP) and aminoterminal propeptide of type III procollagen (PIIINP) levels were determined in 8 patients with autoimmune disorders (2 with systemic lupus erythematosus, 3 with rheumatoid arthritis, 2 with Sjögren's syndrome and 1 with mixed connective tissue disease) before and after 1, 2 and 4 months of treatment with oral prednisone (at a dosage of 1 mg/kg bw/day, p.o. during the first month, then reduced to 0.1-0.2 mg/kg bw/day). Before treatment mean serum BGP (mean +/- SE: 5.3 +/- 0.4 ng/ml) and ICT

Trophic factors in aging. Should older people receive hormonal replacement therapy?

The aging process is associated with significant declines in the levels of many hormones and trophic factors including estrogen, testosterone, growth hormone (somatropin, somatotropin) and insulin-like growth factor-1 (IGF-1, somatomedin-1, somatomedin-C). Since the classic age-related changes resemble the signs and symptoms of endocrine deficiency, it has been hypothesised that some of the negative effects of aging are due to these hormonal deficits. Consequently, the potential role of hormonal replacement in reversing the deleterious effects of aging deserves investigation. In old hypogonada

Long-Term Outcomes of Vascularized Trochlear Flaps for Scaphoid Proximal Pole Reconstruction.

Reconstruction of proximal pole scaphoid nonunion using medial femoral trochlea osteochondral free flap is indicated in young active patients for whom salvage procedures are not ideal. This article reviews the outcomes data available in the literature and provides a brief report of clinical, radiographic, and patient-reported outcomes for the subset of our patients with greater than 3 years postoperative follow-up. The present literature suggests that medial femoral trochlea reconstruction for proximal pole scaphoid nonunion can restore radiocarpal anatomy, prevent progressive carpal collapse,

[Capitate osteotomy in Kienböck's disease in twelve cases. Clinical and radiological results at five years follow-up].

Kienböck's disease could occur pain and reduce wrist's range of motion despite of an early radiologic staging. Usual surgical procedures unload the lunate. Radial shortening is the common procedure in negative ulnar variance. For wrists with neutral or positive ulnar variance, this procedure could produce a distal radio-ulnar discrepancy and an ulnocarpal impingement. We perform, in these cases, a capitate shortening. The goal of this study is to relate the wrist functional outcome and the radiological result in 12 cases. It was a consecutive retrospective study of 12 patients (nine male, thre

Flexor pollicis longus tendon ruptures after palmar plate fixation of fractures of the distal radius.

Palmar plate fixation of distal radial fractures is becoming a standard treatment for this common injury. Ruptures of the extensor pollicis longus tendon have been reported in 8.6% of cases after this procedure. Although palmar plate fixation has also been associated with flexor pollicis longus (FPL) tendon problems, the majority of reported cases pre-date the use of newer anatomically precontoured locking plates. In this paper seven cases of FPL rupture are presented. This complication does not appear to be unique to one type of implant. The possible aetiologies for FPL ruptures are discussed

Treatment of proximal interphalangeal dorsal fracture-dislocation injuries with dynamic external fixation: a pins and rubber band system.

PURPOSE: Unstable, dorsal, intra-articular, fracture-dislocations of the proximal interphalangeal (PIP) joint can be difficult to treat and often lead to long-term pain, stiffness, and functional deficit. We present the outcomes of patients sustaining such injuries that were treated by a novel dynamic external fixator. This fixator uses a system of K-wires and rubber bands that maintains a concentrically reduced PIP joint while allowing for early motion. METHODS: Fourteen patients with unstable, dorsal fracture-dislocation injuries of the PIP joint were treated between September 2001 and Janu

Dynamic scapholunate instability: results of operative treatment with dorsal capsulodesis.

Nineteen patients underwent 20 dorsal capsulodesis for dynamic scapholunate instability. Seventeen patients (18 wrists) were evaluated by a questionnaire and physical examination after a mean postoperative follow-up period of 34 months (range, 12-65). The diagnosis was based on a combination of characteristic symptoms of scapholunate instability and physical findings consisting of dorsal wrist tenderness at the scapholunate interval and a positive scaphoid shift test. Following surgery, a significant decrease was noted in symptoms of pain and clunking. Functional status was improved postoperat
