Medical imaging and CNS infections.

West Nile virus, Creutzfeldt-Jakob disease and new-variant CJD are emergent infectious diseases that affect the brain. Although incidence of these infections is low compared with other CNS diseases such as viral or bacterial meningitis, the reasons for their emergence are complex. Scientists believe that unique combinations of genetic, environmental and social factors have caused these rare pathogens to become more common within human populations. Because the causes of brain infections are difficult to diagnose and treat, medical imaging is an important screening, diagnostic and monitoring too

S-adenosylmethionine concentrations in diagnosis of Pneumocystis carinii pneumonia.

Pneumocystis carinii is unable to synthesise S-adenosylmethionine and thus scavenges this intermediate. We aimed to test whether measurement of concentrations of this metabolic intermediate in plasma could provide a new method for rapid diagnosis of Pneumocystis carinii pneumonia (PCP). We measured S-adenosylmethionine plasma concentrations in 12 healthy controls, 16 patients with confirmed or suspected PCP, and 36 patients with other infections. Median concentration in healthy controls was 106 nmol/L (range 86-128), but the protein was undetectable in eight patients with histologically proven

Efficacy of environmental measures in reducing potentially infectious bioaerosols during sputum induction.

OBJECTIVE: To evaluate the airborne viable bacterial concentrations generated during sputum induction and their reduction with exhaust ventilation, ultraviolet germicidal irradiation (UVGI), or both. METHODS: Exhaust ventilation, upper air UVGI lights, and a portable UVGI unit were operated independently or in combination while and after sputum induction was performed for 58 patients suspected of having active tuberculosis. Viable airborne bacteria were sampled with volumetric air samplers, grown on blood agar, and identified with standard techniques. RESULTS: During and immediately after sp

Short-course therapy of acute cystitis: a brief review of therapeutic strategies.

Acute cystitis is one of the commonest medical problems encountered by primary care physicians. It affects more women than men (8:1), but the incidence among men is increasing. Uncomplicated cystitis by definition occurs in healthy patients with a normal urinary tract, whereas complicated cystitis implies a predisposing or underlying condition. A narrow range of aetiological agents is responsible for most uncomplicated cystitis in women (Escherichia coli in 80% of cases). Recently, however, pathogens usually associated with sexually transmitted disease have been implicated. In women with typic

Emerging and resurging vector-borne diseases.

Over the last four decades, a number of arthropod-borne infections have been recognized for the first time. Some have become of considerable public health importance, such as dengue hemorrhagic fever (DHF), and others are spreading geographically and their incidence is increasing. There has been an important recrudescence of several long-known vector-borne diseases. Malaria, leishmaniasis, dengue, and plague have resurged in numerous foci, in some cases where they were thought to be under effective control. In most instances, the appearance of new diseases and syndromes and the resurgence of o

Prevention of transmission of tuberculosis in hospitals; a survey of practices in Belgium, 1995.

In July 1995, a questionnaire survey was made of nosocomial tuberculosis (TB) prevention practices in Belgian hospitals. Of 122 respondents (response rate: 64%), 93% had hospitalized at least one TB patient, and 11% at least one multi-resistant TB case, during 1994. Effective prevention measures were not uniformly applied: only 96% isolated contagious TB patients, and only 84% isolated patients suspected of contagious T.B. In six hospitals, TB patients and those with human immunodeficiency virus (HIV) were mixed. Wearing of masks by personnel entering a TB patient's room was routine in 96%, bu

Grepafloxacin.

Grepafloxacin (OPC-17116) is a new once-daily fluoroquinolone antimicrobial agent which appears to have high tissue penetration and the wide spectrum of antimicrobial activity typical of this class of agents, but with improved activity against Gram-positive organisms, notably Streptococcus pneumoniae. The in vitro activity of grepafloxacin was similar to or slightly lower than that of ciprofloxacin against Enterobacteriaceae but better than that of ciprofloxacin against most Gram-positive organisms. In particular, grepafloxacin showed good activity against pathogens implicated in community-acq

An outbreak of Norwegian scabies in a surgical ward.

An outbreak of Norwegian scabies in a surgical ward of a teaching hospital was reported. The source of infestation was an elderly diabetic. The epidemic involved 28 of 32 medical personnel in the ward and 3 family contacts. All infected individuals were given a two-course treatment with hexachlorcyclohexane gel. Control of the outbreak was achieved by strict handwashing after patient contact, application of gowns and gloves for contact with skin lesions and the treatment of infected persons. Norwegian scabies in institutions is on the rise, vigilance for its occurrence is needed.

Paradoxical Th1 activation and CTLA-4 regulation is beneficial during latent cryptococcosis.

UNLABELLED: Cryptococcus neoformans is the predominant causative agent of cryptococcal meningitis in immunocompromised individuals. Conversely in immunocompetent individuals, C. neoformans establishes a latent pulmonary infection characterized by a paucity of clinical symptoms. Using a mouse inhalation model of latent C. neoformans infection, we previously showed that CD4 T-cells are necessary for preventing fungal proliferation in the lungs. In the current study, we performed single cell RNA sequencing (scRNAseq) and found that the CD4 T-cell response was both highly heterogenous and dichotom

Surgical debridement and continuous irrigation for early surgical site infection caused by Staphylococcus aureus with a closed patella fracture surgery: A case report.

INTRODUCTION: Management of postoperative surgical site infection (SSI) is a huge challenge to orthopedic surgeons, and significantly impacts patients and their families due to long treatment cycles and associated discomfort experiences. PRESENTATION OF CASE: A 68-year-old woman without a medical history of any comorbidities, diabetes, hypertension, allergies, or tuberculosis, was admitted to our hospital complaining of right knee pain following a fall. X-ray and CT scans revealed a closed right patella fracture. The patient underwent open reduction and internal fixation with tension band wir

Underascertainment of acute hepatitis C virus infections in the U.S. surveillance system: a case series and chart review.

BACKGROUND: In 2010, the incidence of hepatitis C virus (HCV) infection in the United States was estimated to be 17 000 cases annually, based on 850 acute HCV cases reported to the Centers for Disease Control and Prevention by local public health authorities. Absence of symptomatic disease and lack of a specific laboratory test for acute infection complicates diagnosis and surveillance. OBJECTIVE: To validate estimates of the incidence of acute HCV infection by determining the reporting rate of clinical diagnoses of acute infection to the Massachusetts Department of Public Health (MDPH) and C

Recent advances in developing vaccines against Toxoplasma gondii: an update.

Toxoplasma gondii, a significant public health risk, is able to infect almost all warm-blooded animals including humans, and it results in economic losses in production animals. In the last three years, a large number of vaccination experiments have been performed to control T. gondii infection, with the target of limiting the acute infection and reducing or eliminating tissue cysts in the intermediate hosts. In this paper, we summarize the latest results of the veterinary vaccines against T. gondii infection since 2013. Immunization with live-attenuated whole organisms of non-reverting mutant

Urinary catheter-related infection: an education programme for users.

While infection control has increased in profile both clinically and politically, it has also become embedded in clinical governance frameworks of NHS Trusts. Over the last few years the Department of Health has produced a number of documents in an attempt to highlight the increasing risk of healthcare associated infection. Today, the need for education as a key component in health care has been recognized as well as the need for patients and carers to receive information and be trained in, for example, technique of hand decontamination and catheter management following discharge from hospital…

Intestinal amoebiasis: delayed-type hypersensitivity response in mice.

Delayed-type hypersensitive (DTH) response was evaluated in C3H/HeJ mice intestinally infected with Entamoeba histolytica. Infected and non-infected control mice were challenged with amoebic antigen on day 5, 10, 15, 20, 25, 30, 40, 50, and 60 post-infection. Maximum footpad swelling was observed after 24 hours of the challenge. The E. histolytica-infected mice exhibited a DTH response on day 5, 15, 20, 25, 40 and 60 post-infection. However, on day 10, 30, and 50, such response was similar to that of the non-infected control mice. The mice developed an evident DTH response late in the course o
