Herpetic keratitis with iritis after corneal crosslinking with riboflavin and ultraviolet A for keratoconus.

A 21-year-old woman had crosslinking for keratoconus in the right eye; the left eye was scheduled for penetrating keratoplasty. Five days postoperatively, she presented with geographic epithelial keratitis and iritis. Analysis of tear samples by polymerase chain reaction confirmed the diagnosis. The patient was treated with oral steroids and acyclovir, with significant improvement. Two months postoperatively, the visual acuity was improved and there was no evidence of herpetic disease recurrence. Crosslinking can induce herpetic keratitis with iritis even in patients with no history of herpeti

[Retinopathy in a patient with hepatitis C treated with pegylated interferon and ribavirin: case report].

Interferon is an immunomodulating cytokine used to treat patients with different diseases, such as hepatitis C chronic infection. Pegylated interferon is a new type of interferon, developed to increase the half-life of the drug. Many side effects have been related to its use, including ocular toxicity and retinopathy. The most reported ocular findings are cotton-wool spots and hemorrhages located at the posterior pole and surrounding optic nerve head. We describe one case of pegylated interferon-associated retinopathy with visual loss. The patient had visual acuity improvement four weeks after

[Keratoconus: current surgical options].

The treatment of keratoconus is in part dependant on the severity of the disease process. When contact lenses fail because of patient intolerance or insufficient acuity to meet the patient's needs, penetrating keratoplasty is the usual surgical modality considered for satisfactory visual rehabilitation. Keratoconus is considered to be a contraindication for incisional refractive surgery such as radial keratotomies or LASIK because of the corneal instability conferred by these procedures. When the cornea is transparent, INTACS implantation is a refractive modality that may improve visual functi

Current surgical options for keratoconus.

The treatment of keratoconus is partly dependent on the severity of the disease process. When contact lenses fail because the patient is intolerant or they do not provide sufficient acuity to meet the patient's needs, penetrating keratoplasty is the usual surgical modality for adequate visual rehabilitation. Keratoconus is considered a contraindication to incisional refractive surgery such as radial keratotomy and laser in situ keratomileusis because of the corneal instability conferred by these procedures. When the cornea is transparent, intrastromal corneal ring implantation is a refractive

[Unilateral exophthalmos during the course of cavernous sinus thrombosis: a case report].

Ophthalmopathy, particularly its infiltrative form, is considered a characteristic feature of Graves' disease. However many other also life-treating conditions may be manifested as unilateral exophthalmos and lead to diagnostic problems. A case of a sixty six-year old patient with asymmetric exophthalmos and rapidly decreasing vision acuity is reported. Thyroid eye disease was suspected due to clinical symptoms (diplopy, impaired right eye motion) and thickening of extraocular muscles in ultrasound imaging. Further follow-up and orbits as well as head MRI imaging led to the final diagnosis of

Septooptic dysplasia.

The term septooptic dysplasia was coined in 1956 by de Morsier, who pointed out the association of optic nerve hypoplasia and absence of the septum pellucidum. Patients with this condition may present with clinical features of hypopituitarism, decreased visual acuity and neurodevelopmental disabilities that lead to this diagnosis. The case that is presented here is unusual in that this patient was initially diagnosed as having low tension glaucoma during a routine screening examination and was treated for glaucoma for over a year before he was discovered to have septooptic dysplasia, also know

[Placement of stent for internal carotid artery stenosis in the cervical portion improved ocular ischemic syndrome].

We report a case of ocular ischemic syndrome due to severe stenosis of the internal carotid artery. The patient was a 67-year-old man with a hemispheric transient ischemic attack and an amaurosis fugax. Ocular examination showed multiple soft exudates and dilatation of the retinal veins. Carotid stenting was performed to reconstruct the internal carotid artery. The multiple soft exudates were almost diminished and the visual acuity was improved. These results suggest that carotid stenting for severe stenosis of the internal carotid artery should be one of the less invasive methods to improve t

Pseudoxanthoma elasticum and calcinosis cutis.

A 42-year-old white woman presented with clinical and histologic manifestations of both calcinosis cutis and pseudoxanthoma elasticum: discrete milia-like calcifications at the anterior aspect of the neck, a funduscopic examination with classic eye findings, peripheral vascular disease, and a mottled appearance of the skin at the axillae, groin, and lateral aspects of the neck. A younger sibling had similar skin lesions and deteriorating visual acuity. The patient was normocalcemic and normophosphatemic. This case may represent the coincidental occurrence of two rare entities in the same perso

A sighting of orbital pseudotumor.

A 39-year-old woman developed bilateral proptosis, photophobia, and pain with extraocular movements over the course of 5 days. Her findings initially were ocular pain and photophobia which progressed to periorbital edema and nasal discharge ultimately resulting in proptosis with vertical globe displacement and decreased visual acuity. She was diagnosed with corneal abrasion and sinusitis respectively during two initial emergency department visits. On her third visit to the emergency department within 4 days, she developed acute visual deficits. The patient was subsequently diagnosed with orbit

Ischemic optic neuritis in Churg-Strauss syndrome.

There are few reports of neuro-ophthalmologic involvement in Churg-Strauss syndrome (CSs). We described a case of unilateral optic atrophy in a 46-year-old-white man with CSs. The patient had severe bronchial asthma, allergic rhinitis, hypereosinophilia (8%) and peripheral neuropathy. The visual acuity in his right eye was light perception. At the biomicroscopy there were no corneal and conjunctival lesions. Ophthalmoscopy showed a pale right optic disc and fluorangiography revealed a marked hypofluorescence of the disc at early phase of angiogram. We suggested that the optic atrophy was most

[Tardive congenital glaucoma in the case of brothers].

The case of a family is presented, in which mother had been operated 5 years ago for tardive congenital glaucoma, in a very advanced stage. At that moment, the examination of the eldest son had not revealed modifications of the visual acuity, eye fundus and intraocular pressure. After 5 years, the examination of the 5 children revealed tardive congenital glaucoma in a very advanced stage at the first son and in a manifest stage at the second. The obvious conclusion is to emphasize the importance of the careful periodical examination for all the descendents of a patient with congenital glaucoma

Isolated ocular cryptococcosis in an immunocompetent patient.

A 62-year-old woman without evidence of immunocompromise was evaluated for uveitis and a subretinal lesion in the right eye. Laboratory evaluation, including cerebrospinal fluid analysis, revealed no apparent cause. The diagnosis of subretinal cryptococcosis was established by transscleral needle biopsy of the subretinal mass. Treatment with intravenous amphotericin B and oral 5-flucytosine brought recovery of visual acuity to 20/30-1 and resolution of the inflammation. This patient demonstrates that ocular cryptococcal infection must be suspected, even in the absence of predisposing factors o

A case of Vogt-Koyanagi-Harada syndrome.

A previously healthy 32-year-old female contact lens patient presented with a constant headache of several days duration and a sudden painless loss of vision in the right eye. Examination revealed a serous elevation of the sensory retina with whitish plaque-like lesions and decreased visual acuity. Fluorescein angiography suggested a diagnosis of Vogt-Koyanagi-Harada syndrome (V-K-H). V-K-H syndrome is a chronic bilateral exudative uveitis associated with whitening of the hair and eyelashes and varying signs of meningeal irritation. A discussion of this case with photos and fluorescein angiogr

Sellar granulomatous mass in a pregnant woman with active Crohn's disease.

At the end of her third pregnancy, a woman with a history of chronic active proctocolitis which had been diagnosed as Crohn's disease complained of deterioration of visual acuity. The patient was found to have hypopituitarism and progressive bitemporal hemianopia caused by an intrasellar mass with suprasellar extension. At transsphenoidal surgery an intrasellar granuloma was found without remnants of pituitary tissue. This sellar granuloma could have been an extra-intestinal granuloma of Crohn's disease. However, the possibility that giant cell granulomatous hypophysitis had been present was c

[Toxic damage of the optic nerve in treatment with ethambutol in a patient on chronic dialysis].

The authors give an account of a 57-year-old male patient who was regularly dialyzed for two years and treated because of suspect TB of the spine with antituberculotics, incl. ethambutol. In the course of 8-9 months he was given 17.9 mg/kg ethambutol per day. Because of the practically zero excretion by the damaged kidney the drug cumulated in the serum and reached toxic levels before dialysis. Gradual deterioration of eyesight developed (central acuity and colour sense) which resulted in bilateral atrophy of the optic nerves with concentric narrowing of the visual field.

Subperiosteal abscesses of the orbit due to sinusitis in childhood.

This paper reports 16 cases of subperiosteal abscesses of the orbit due to acute sinusitis in childhood. They comprise 12.4% of 129 patients presenting with an 'acute orbit'. Acute ethmoidal sinusitis was the predominant cause. The typical clinical features, as well as CT scanning of the paranasal sinuses, orbit and brain, should assist in early diagnosis. If possible, the subperiosteal abscess and sinuses should be drained before loss of visual acuity or intracranial complications occur. Streptococcus milleri was the pathogen most commonly cultured. One patient in this series had intracranial

Eleven patients successfully treated with the argon or krypton laser for macular subretinal neovascularization were followed up in order to determine the extent of long-term enlargement of the chorioretinal scars. The mean follow-up time was 51.5 months and the mean increase in choroidal scar size 46.5%. In 10 patients the scars enlarged more toward the fovea than toward the periphery (51% as opposed to 43%). The rate of enlargement per month decreased with time after treatment. Only one patient exhibited decreased visual acuity during the follow-up period, due to invasion of the foveola by th

[Neutron spin tomography of the optic nerve in retrobulbar neuritis and optic nerve atrophy: demonstration of inflammation and demyelination].

Detection of inflammatory lesions of the optic nerve by magnetic resonance imaging requires a special imaging technique. In 3 patients with persisting visual loss due to retrobulbar neuritis we scanned the optic nerve with a short inversion recovery sequence. In each of the 3 patients magnetic resonance imaging revealed inflammatory lesions in the intracanalicular portion of the optic nerve. It is supposed that the unfavorable location of the lesions in the optic canal is responsible for the poor recovery of visual acuity. In another patient with a bilateral optic nerve atrophy demyelination o

Ectopia lentis.

Ectopia lentis was first described more than 200 years ago, but its value as a significant diagnostic clue to the presence of other ocular and systemic disorders has only recently been appreciated. Ectopia lentis may cause a marked reduction in visual acuity, which varies with the type and degree of dislocation and the presence of other ocular abnormalities. An approach to the diagnostic evaluation of the patient with ectopia lentis is outlined. The differential diagnosis is reviewed in detail both clinically and histologically. The complications of ectopia lentis and the appropriate managemen

Ocular quinine toxicity.

A 25-year-old woman had no light perception in either eye 14.5 hours after ingesting 3.7 to 4.7 g of quinine. Fundus photographs showed mild venous distention and retinal haziness, but normal arteries. The electroretinogram, electrooculogram, and visual-evoked potential were all mildly abnormal. Two days after the patient ingested quinine, her visual acuity returned to 6/6 (20/20) with a markedly constricted visual field. The classic late fundus appearance of quinine toxicity, marked vascular narrowing, and disk pallor developed within six months. Visual fields remained constricted to 12 degre
