Evaluation of childbirth self-efficacy and associated factors among pregnant women.

BACKGROUND: Increasing the rate of Normal Vaginal Delivery (NVD) and reducing elective cesarean section is one of the important concerns, especially in Iran. Childbirth self-efficacy can play an important role in managing the fear of childbirth and increasing the rate of NVD. The aim of this study was to determine demographic, fertility, and social factors related to childbirth self-efficacy. MATERIALS AND METHODS: The present study was a single-group, single-stage, multivariate, cross-sectional study that assessed childbirth self-efficacy and associated factors in 358 eligible pregnant women

Borderline We-Space? The Phenomenology of the Background of Safety in Borderline Personality Disorder.

Borderline personality disorder (BPD) is a severe psychiatric condition characterized by instability in identity, relationships, and affect. Individuals, with BPD typically lack a coherent sense of self, are highly sensitive to interpersonal stressors, experience intense fluctuations in mood, and frequently engage in impulsive and self-destructive behaviors. Although both empirical research and development of effective psychotherapy have evidently progressed over the past years, many aspects regarding the structure of experience and the life-world typical for persons with BPD are not yet fully

HIV Pre-Exposure Prophylaxis for Conception Among HIV Serodiscordant Couples in the United States: A Cohort Study.

Practice guidelines on pre-exposure prophylaxis (PrEP) for HIV serodiscordant couples recommend PrEP when the viral load of the partner living with HIV is either detectable or unknown. However, adherence to combination antiretroviral therapy is inconsistent, and research has found that individuals vulnerable to HIV place value on additional protective barriers. We conducted a prospective cohort study to assess the feasibility, perceptions, and adherence associated with periconceptional PrEP use among females without HIV and their male partners living with HIV across four academic medical cente

Dimensions of personality disorders in offenders.

BACKGROUND: Owing to criticisms of current concepts of personality disorders such as high comorbidity, criteria overlap and arbitrary thresholds of categorical diagnoses, a dimensional assessment is proposed that considers interrelations between different personality disorders. Results of previous factor analyses using dimensional personality disorder scores have indicated that one underlying dimension shows strong similarities to the concept of psychopathy and is similarly related to criminal recidivism. AIM: The authors examined the underlying dimensions of ICD-10 personality disorders, to

Dual-Functional Self-Assembled Nanoparticles for Synergistic Photodynamic Therapy and Antimetastatic Treatment of Colorectal Cancer.

Background: Colorectal cancer (CRC) is a major clinical challenge due to high metastasis and therapy resistance. Photodynamic therapy (PDT) offers precise tumor ablation but lacks sustained anti-metastatic activity. Peptidic urokinase-type plasminogen activator (uPA) inhibitors suppress metastasis but suffer from short half-life and poor tumor retention. This study aimed to develop a dual-functional self-assembled nanoplatform integrating PDT and selective uPA inhibition for synergistic CRC treatment. Methods: We designed and synthesized a conjugate by linking pyropheophorbide-a (PPA) with uPA

"Gold standard" Birth: Unmet birth expectations and the ought-maternal-self.

Unmet birth expectations are strongly associated with poorer maternal mental health, yet the processes underlying this association remain underexplored. This study addresses critical theoretical and empirical gaps by being the first qualitative investigation to recruit women specifically on the basis of unmet birth expectations and to theoretically account for how sociocultural discourses translate into psychological harm. Twenty-one semi-structured interviews were conducted with first-time mothers in the U.K. and analysed using reflexive thematic analysis. Three inductive themes were generate

Regulation of response to antigen peptides is independent of peptide distribution in lymph node therapeutics.

Autoimmune disease occurs when immune cells mistakenly identify specific molecules, termed antigens, on healthy cells. There are no cures for these diseases, and existing treatments - including monoclonal antibodies - do not specifically target dysfunctional cells. These challenges have motivated interest in therapies that could achieve antigen-specific immune tolerance. One concept involves co-delivery of self-antigens and regulatory cues to selectively redirect the response to self-antigen. Biomaterials have been investigated in this context owing to the control these technologies offer. We

Educational programs and mental health outcomes in individuals with type 1 diabetes: a scoping review.

BACKGROUND: Mental health is a critical yet often underemphasized dimension in the management of individuals with Type 1 Diabetes Mellitus (T1DM), who are at elevated risk for psychological disorders. Educational interventions, including traditional education, psychoeducational, and psychosocial programs, are increasingly recognized as supporting self-care and promoting psychological well-being. AIM: This scoping review aims to systematically map the existing literature on educational programs for individuals with T1DM, with a specific focus on their impact on mental health outcomes. METHODS:

Paradoxical bio-citizenship: Examining healthy eating from lay and professional perspectives.

In a society saturated with health messages, the concept of an eating disorder based on an obsession with healthy eating involving pathological behaviours and thoughts- "orthorexia nervosa"- presents a paradox, raising questions about extreme healthism and psychiatry's expanding reach into contemporary life. To explore this matter, we re-analysed data from six existing studies, including 56 interviews (individuals preoccupied with healthy eating; bodybuilders practising extreme healthy eating; health practitioners specialising in eating disorders; fitness instructors) - and anonymous eating di

Exploring complex-PTSD comorbidity in trauma-exposed Northern Ireland veterans.

BACKGROUND: Complex posttraumatic stress disorder (CPTSD) describes the results of complex, prolonged, and/or inescapable trauma, and is typified by avoidance, re-experiencing, sense of threat, affect dysregulation, negative self-concept, and interpersonal disturbances. Additionally, CPTSD is highly comorbid with other common psychopathologies. OBJECTIVES: A study was conducted in a trauma-exposed UK Armed Forces Veteran population resident in Northern Ireland (N = 638, NI) to determine the prevalence of CPTSD and comorbid associations. METHODS: Data from the Northern Ireland Veterans Health

The role of incongruence between the perceived functioning by patients and clinicians in the detection of psychological distress among functional and motor digestive disorders.

OBJECTIVES: Previous research on gastrointestinal and other medical conditions has shown the presence of incongruence between self- and clinician-reported functioning and its relation with psychopathology. The main objective of this study was to test whether inconsistencies between clinician- and self-assessed functionality can be used to detect psychopathology among patients diagnosed of motor or functional gastrointestinal disorders. METHODS: One hundred and three patients from a gastroenterology inpatient unit were included in this study. All patients underwent clinical assessment, includi

The concept of temperament in psychoactive substance use among college students.

BACKGROUND: Substance abuse is among the leading causes of preventable diseases and premature death but reasons and conditions leading to substance abuse are complex and multifaceted. Different models of abuse and dependence assume an underlying emotional vulnerability. Individual behavioral and emotional reactivity patterns of personality are considered in the concept of temperament but studies linking different types of temperament with substance use are rare. METHODS: In this study we investigated 1380 inhabitants (59.7% females; 40.3% males) of residential student homes in Austria, using

Design of a mimic of nonamyloidogenic and bioactive human islet amyloid polypeptide (IAPP) as nanomolar affinity inhibitor of IAPP cytotoxic fibrillogenesis.

Protein aggregation into cytotoxic oligomers and fibrils in vivo is linked to cell degeneration and the pathogenesis of >25 uncurable diseases, whereas the high aggregation propensity and insolubility of several bioactive polypeptides and proteins in vitro prevent their therapeutic use. Aggregation of human islet amyloid polypeptide (IAPP) into pancreatic amyloid is strongly associated with the pathogenesis of type II diabetes. IAPP is a 37-residue polypeptide that acts as a neuroendocrine regulator of glucose homeostasis. However, IAPP misfolds and self-associates into cytotoxic aggregates an

The causes of patient dropout from penile self-injection therapy for impotence.

PURPOSE: Penile self-injection therapy, a second line treatment for erectile dysfunction, is the most efficacious means of reestablishing functional erections when first line therapies fail and the patient wants to avoid penile prosthesis implantation. Despite high efficacy rates, injection therapy has high dropout rates. To our knowledge studies to date analyzing patient attrition have reviewed small numbers of patients followed for only short periods. We elucidate the main reasons for patient dropout in a large penile self-injection program with long-term followup. MATERIALS AND METHODS: A

'I know it's not normal but it's normal to me, and that's all that matters': experiences of young adults conceived through egg donation, sperm donation, and surrogacy.

STUDY QUESTION: What are thoughts and feelings of young adults born following egg donation, sperm donation, and surrogacy? SUMMARY ANSWER: Young adults felt either unconcerned or positive about the method of their conception. WHAT IS KNOWN ALREADY: Much of what we know about adults born to heterosexual couples following anonymous donation has come from samples of donor conceived people who had found out about their origins during adulthood. There have been no studies of how young adults born through surrogacy feel about their conception and towards their surrogate. STUDY DESIGN, SIZE, DURAT

First-void urine and cervicovaginal brushes for vaginal microbiome profiling: a proof-of-concept study.

UNLABELLED: The human vaginal microbiome is a cornerstone of female reproductive health, influencing susceptibility to vaginal conditions, pregnancy complications, and urogenital infections. Understanding this microbiome is critical for advancing preventive, diagnostic, and therapeutic interventions, yet its intimate and dynamic nature poses challenges for accurate, representative, and more inclusive sampling methods. This proof-of-concept study evaluated the performance of first-void urine (FVU) samples and cervicovaginal brushes for vaginal microbiome profiling relative to vaginal swabs. FVU
