MR imaging of impingement and rotator cuff disorders. A surgical perspective.

The role of MR imaging in the diagnosis of impingement and rotator cuff disorders is expanding greatly. This article discusses the clinically pertinent history, physical examination, and surgical treatment of rotator cuff problems. A rationale for consideration of open, "mini-open," and arthroscopic surgical intervention is proposed. The utilization of MR imaging by the orthopedic surgeon for nonoperative, preoperative, and postoperative decision-making are presented. A close working relationship between the radiologist and the orthopedist is emphasized.

Magnetic resonance imaging of glenohumeral instability.

Glenohumeral instability refers to subluxation or dislocation of the glenohumeral joint. This painful disorder is particularly common in the athletic population, and the individual with instability may present with a variety of clinical complaints. This paper addresses current theories regarding the pathophysiology of glenohumeral instability and illustrates magnetic resonance findings that can assist in diagnosis and treatment planning.

Posterior superior impingement of the rotator cuff on the glenoid rim as a cause of shoulder pain in the overhead athlete.

A case of impingement of the deep surface of the supraspinatus tendon on the posterior superior rim of the glenoid and mild anterior laxity has been presented. The partial-thickness tear of the undersurface rotator cuff tendon, degenerative tear of the posterior superior labrum, and osteochondral impression fracture of the humeral head have been documented. The purpose of this case report is to present the pathological findings associated with posterior superior glenoid rim impingement and emphasize its role as a cause of shoulder pain in the overhead athlete. In addition it is important to st

Adolescent traumatic dislocations of the shoulder with open epiphyses.

Nine of 212 cases of traumatic anterior shoulder dislocations occurred in children with clearly open epiphyses (4.7% incidence). All cases associated with psychological or physical abnormalities were excluded. A high recurrence rate of 80% (8 of 10) was noted requiring operative intervention. Two of the three remaining unoperated cases had a history of "subluxation" after their initial dislocation.

The throw: biomechanics and acute injury.

The throw and its modifications are integral components of many sports. This study correlates case histories of acute injuries in throwing with a biomechanical analysis of the throwing mechanism. Comparisons are made with a similar analysis of the kick analyzed by the same film technique and computer program. Just prior to ball release, the pitching arm extends through an arc of about 73 degress in 40 msec, beginning with the elbow flexed at 80 degrees. This produces an axial load on the humerus and coincides with a pulse of external torque at the shoulder. This acts as stress protection to th

Frequency, Severity and Ergonomic Awareness of Work-Related Musculoskeletal Disorders Among Australian Optometrists: A Cross-Sectional Survey.

PURPOSE: Work-related musculoskeletal disorders (WMSDs) are highly prevalent among healthcare professionals, including optometrists. This study aimed to assess the frequency, severity and contributing factors of WMSDs among Australian optometrists, as well as evaluate their awareness and implementation of ergonomic practices. METHODS: A cross-sectional online survey was distributed to optometrists in Victoria and South Australia. The survey collected data on demographics, work characteristics, ergonomic awareness, frequency and severity of musculoskeletal discomfort and contributing factors.

Construct validity of the Brazilian QuickDASH for assessing work-related upper-limb musculoskeletal disorders.

BACKGROUND: Work-related musculoskeletal disorders (WRMD) may be associated with various individual, physical, biomechanical, or psychosocial/organisational risk factors. Self-report questionnaires have increasingly been used to assess patients' perceptions of injury impact. QuickDASH is a widely used outcome measurement questionnaire but is seldom applied to WRMD. OBJECTIVE: To evaluate the construct validity of the Brazilian version of the QuickDASH (QD-Br) in patients with WRMD affecting the upper limbs, hypothesising a moderate correlation with fatigue, ergonomic, and functional questionn

Biceps brachii regional shear modulus following elbow flexion exercises at different muscle lengths.

The biceps brachii (BB) muscle, due to its biarticular nature, undergoes substantial length changes during exercise. Muscle elongation, particularly when the shoulder is extended, increases passive tension and may impair force production due to passive insufficiency. The distal BB appears vulnerable to mechanical strain and injury during eccentric contractions. This study investigated regional changes in BB shear modulus (μ) through shear wave elastography before and after elbow flexion exercises performed in three shoulder positions: flexion (45°), neutral (0°), and extension (-45°). Eighteen

Clinical, Kinematic and Muscle Assessment of Bilateral Coordinated Upper-Limb Movements Following Cervical Spinal Cord Injury.

Cervical spinal cord injury (cSCI) often results in bilateral impairment of the arms, leading to difficulties in performing daily activities. However, little is known about the neuromotor alterations that affect the ability of individuals with cSCI to perform coordinated movements with both arms. To address this issue, we developed and tested a functional assessment that integrates clinical, kinematic, and muscle activity measures, including the evaluation of bilateral arm movements. Twelve subjects with a C5-C7 spinal lesion and six unimpaired subjects underwent an evaluation that included th

Management of Patella Fractures Non-amenable to Tension Band Wiring: Series of Nine Cases with Review of Literature.

INTRODUCTION: Isolated coracoid process fracture of scapula is a rare. Usually, it happens along with the rotator cuff tear, dislocation of acromioclavicular joint, or glenohumeral joint. Displaced fracture will cause impairment of upper limb movement. Since there are very low numbers of cases reported so far, the treatment is still controversial. CASE REPORT: We report a case of a 42-year-old male laborer with motor vehicle accident, following which he had pain and restricted movements on his left shoulder. On examination, Glasgow Coma Scale 15/15 and left shoulder had swelling and tendernes

Posterior shoulder dislocation with associated reverse Hill-Sachs lesion: clinical outcome 10 years after joint-preserving surgery.

INTRODUCTION: Posterior shoulder dislocation in association with reverse Hill-Sachs lesion is a rather rare injury. Few studies reporting results after joint-preserving surgery in these cases are available. This current study presents the clinical outcomes 10 years postoperatively. MATERIALS AND METHODS: In a prospective case series, we operatively treated 12 consecutive patients (all males) after posterior shoulder dislocation with associated reverse Hill-Sachs lesion using joint-preserving techniques. Patients received surgery in a single center between January 2008 and December 2011. The j

Effects of focused continuous pulsed electromagnetic field therapy on early tendon-to-bone healing.

AIMS: Rotator cuff (RC) tears are common musculoskeletal injuries which often require surgical intervention. Noninvasive pulsed electromagnetic field (PEMF) devices have been approved for treatment of long-bone fracture nonunions and as an adjunct to lumbar and cervical spine fusion surgery. This study aimed to assess the effect of continuous PEMF on postoperative RC healing in a rat RC repair model. METHODS: A total of 30 Wistar rats underwent acute bilateral supraspinatus tear and repair. A miniaturized electromagnetic device (MED) was implanted at the right shoulder and generated focused P
