Chronic oesophageal fistulation for studying oropharyngeal and gastric influences on ingestive behaviour in the pigeon.

Surgical and postoperative procedures were developed in the pigeon to study the influence of oropharyngeal and postingestional factors in the control of food and water intake. Pigeons were prepared with an oesophageal fistula so that material swallowed emerged from a tube exteriorized at the neck. Nutrient solutions were injected into the lower portion of the oesophagus through a feeding tube inserted in the fistula. A postoperative tube-feeding regimen, using commercially available laboratory chow, was established to maintain the pigeon at its preoperative weight.

[The radiolucent esophageal foreign body in children (author's transl)].

The symptoms of radiolucent esophageal foreign bodies in small children may be predominantly respiratory with stridor and dyspnea. In a 1 3/12 years old girl who suffered from inspiratory stridor and only later from dysphagia as well this diagnosis was delayed for 2.5 months. Air in the upper part of the esophagus was the first striking roentgenologic symptom of the foreign body which was then clearly outlined by a barium swallow examination. The foreign body was a plastic lid of a candy tube.

[A case of Goldenhar syndrome: acute vitamin A intoxication in the mother during pregnancy].

The authors describe the case of a boy aged 2 1/2, who showed at birth a bilateral epibulbar dermoid, preauricular appendices on the left side and other malformations suggesting Goldenhar's syndrome. The history disclosed that in the second month of pregnancy the mother had accidentally swallowed 10 ml of an oily solution of Vitamin A during a laboratory manipulation. In their discussion, the autonrs analyse the teratogenic action of Vitamin A during embryonic development and review the aetiologies found in cases of Goldenhar's syndrome.

Impact of hypercontractile esophagus on clinical and physiologic outcomes after antireflux surgery: a matched-control study.

BACKGROUND: Hypercontractile esophagus is a motility disorder defined by ≥20% of swallows with a distal contractile integral (DCI) > 8000 mmHg·cm·s on high-resolution manometry. Although hypercontractility has been associated with gastroesophageal reflux disease (GERD), its impact on outcomes after antireflux surgery (ARS) and on postoperative esophageal physiology remains unclear. METHODS: Patients undergoing primary ARS from 2014 to 2023 with hypercontractile esophagus were identified from a prospectively maintained database and matched 1:2 to controls without hypercontractility by age, sex…

Genesis of Bolus Pressure During Primary Peristalsis: Key to Understanding "Functional Dysphagia".

BACKGROUND: During swallowing, the bolus passes through the esophagus during the relaxation phase of peristalsis-termed the bolus domain (BD … (FD)" patients. METHODS: We analyzed HRMZ recordings from 30 healthy controls and 30 FD patients. Swallows of 5 mL and 10 mL saline were recorded in the supine and Trendelenburg positions. BP was measured in the BD (from upper esophageal sphincter relaxation to the 20-mmHg isocontour onset of contraction). Automated tools extracted BP, luminal cross-sect …

Type II achalasia with post-POEM recovery of esophageal motility.

54-year-old woman with acromegaly due to pituitary macroadenoma that consulted for dysphagia for solids. In gastroscopy, it is observed difficulty in passing in the esophagogastric junction (EGJ). Under the suspect of Achalasia, a high-resolution esophageal manometry (HRM) was performed, observing a complete absence of motility of the esophageal body, panesophageal pressurization in > 20% of swallows and IRP of 21mmHg, confirming the diagnosis of type II Achalasia. Peroral endoscopic myotomy (POEM) is performed. After the intervention, the patient presented clinical improvement. In the control…

Type II achalasia with focal elevated pressures: A distinct manometric and clinical sub-group.

BACKGROUND: Type II achalasia (Ach2) is distinguished from other achalasia sub-types by the presence of panesophageal pressurization (PEP) of ≥30 mmHg in ≥20% swallows on high-resolution manometry (HRM). Variable manometric features in Ach2 have been observed, characterized by focal elevated pressures (FEPs) (focal/segmental pressures ≥70 mmHg within the PEP band) and/or high compression pressures (PEP ≥70 mmHg). This study aimed to examine clinical and physiologic variables among sub-groups of Ach2. METHODS: This retrospective single center study performed over 3 years (1/2019-1/2022) includ…

In-Office Injection for Retrograde Cricopharyngeal Dysfunction Treatment.

This "How I Do It" article details an in-office, EMG-guided modified lateral transcervical botulinum toxin injection into the cricopharyngeus muscle for retrograde cricopharyngeal dysfunction. It highlights patient positioning, surface landmarking, and EMG confirmation of correct needle placement using swallow and sniff maneuvers. The technique provides a rapid office-based alternative to the operating room approach and includes practical counseling on expected side effects and the common need for a planned contralateral injection when initial benefit is incomplete or wanes.

Delayed Diagnosis of Dysphagia: Consider "FOSMN".

An 80-year-old lady had a history of progressive swallowing difficulty over several years with significant weight loss, but prior investigations in several medical departments proved negative. Neurological assessment noted her complaint of impaired feeling for food in her mouth and examination showed impaired corneal reflexes and facial sensory function. Blink reflex electrodiagnostic testing was consistent with a diagnosis of facial onset sensory and motor neuronopathy (FOSMN). This article raises awareness of the diagnosis, investigation and treatment of FOSMN.

Duodenal obstruction after non-endoscopic intragastric balloon placement: a rare but severe complication.

We report the case of a mid-30s woman with no prior medical history who presented to the emergency department with intense upper abdominal pain lasting 12 hours. She had undergone the placement of a non-endoscopic, swallowed intragastric balloon the day before. Physical examination revealed significant tenderness in the upper abdomen without signs of peritoneal irritation. Abdominal computed tomography showed a 12-cm balloon lodged in the duodenum with peritoneal effusion but no evidence of free perforation.

Perforation of the sigmoid colon with formation of a sigmoido-ureteral fistula caused by an ingested denture: a case report.

Accidentally ingested objects such as dentures can have serious health consequences with damage to the gastrointestinal tract. We report the case of a patient who ingested her dental plate unnoticed, leading to a sigmoid perforation and formation of a sigmoido-ureteral fistula. The patient presented to our emergency department with symptoms 3 months after ingestion. After unsuccessful endoscopic retrieval, a low anterior rectal resection with retrieval of the swallowed denture and resection of the left ureter with subsequent reconstruction surgery was performed.

Contained Rupture of a Thoracic Aortic Aneurysm: A Rare and Life-Threatening Cause of Dysphagia.

Dysphagia is commonly seen in the elderly and has both benign and malignant causes. A difficulty in swallowing due to the esophageal compression by the aorta, or dysphagia aortica, is a rare entity, little described in literature. However, diagnostic error or diagnostic and treatment delays of aortic dysphagia can be fatal. Herein, we report a case of dysphagia aortica caused by the contained rupture of a descending aneurysmatic thoracic aorta, presenting at the emergency department with acute dysphagia and diffuse chest pain, successfully treated by thoracic endovascular aneurysm repair.
