Neurohormonal consequences of diuretics in different cardiovascular syndromes.

Diuretics have long been used to lower blood pressure in hypertensive patients or to control body fluid and electrolyte homeostasis in diseases such as congestive heart failure, chronic renal failure or cirrhosis. The initial response to diuretics is a negative sodium and fluid balance. The diuretic-induced loss of salt and water activates several hormonal systems such as vasopressin, the renin-angiotensin-aldosterone system or the sympathetic nervous system which tend to compensate for the changes in sodium and water balance. This neurohormonal response may have important clinical implication…

Sympathetic-like-integrated engineered heart tissue models AGEs-induced adverse remodeling.

BACKGROUND: Cardiovascular metabolic diseases (CMDs) are a major contributor to global mortality and disability, yet their pathogenesis remains incompletely understood, partly because existing in vitro models fail to capture disease complexity. Conventional engineered heart tissues (EHT), which typically contain only a limited set of cell types and lack neural components, cannot replicate the intricate neuro-cardiac interactions involved in CMDs. OBJECTIVE: This study aimed to develop a neuron-like-Integrated Engineered Heart Tissue for investigating neuro-cardiac interactions under both phys

Arterial wave reflections increase during voluntary apnea and the cold pressor test, increasing aortic systolic blood pressure.

Acute sympatho-excitation increases peripheral blood pressure (BP) through sympathetically mediated vasoconstriction. Aortic BP and pulsatile hemodynamics are stronger predictors of cardiovascular risk and outcomes compared with brachial BP. How pulsatile hemodynamics respond to sympatho-excitation and the impact on aortic BP are incompletely understood. The purpose of this study was to assess the effects of maximal voluntary apnea and the cold pressor test (CPT) as sympatho-excitatory perturbations on aortic BP and measures of pulsatile hemodynamics. Radial artery pressure waves were acquired

Progressive baroreflex dysfunction and hypotension preceding VVS: a vicious cycle?

PURPOSE: We aimed to clarify the mechanism for presyncope, defined as the gradual onset of hypotension, starting some minutes before vasovagal syncope. Although there is a fall in cardiac output and usually vasodilatation, the control of sympathetic activity during presyncope is uncertain. METHODS: We retrospectively compared haemodynamics and muscle sympathetic nerve activity levels from positive tilt tests (without provocation) in patients with known vasovagal syncope (age 41 ± 3 years, 13 female, n = 27) to controls (age 39 ± 3 years, 8 female, n = 13). We used sequence methods to measure

Effect of acute nitrate supplementation on the superficial conduit venous vascular response at rest and during sympathoexcitation.

The effects of increased nitric oxide (NO) activity and/or NO precursor levels following nitrate (NO3-) supplementation on venous vascular control remain poorly understood. We investigated the effect of acute NO3- supplementation on the venous vascular response of a single conduit vein at rest and during sympathoexcitation (e.g., static exercise and muscle metaboreflex). Participants were 15 healthy young adults who consumed either beetroot juice (BRJ, 140 mL; ∼8 mmol NO3-) or a control beverage (prune juice; CON, 166 mL; <0.01 mmol NO3-) following a random crossover study design. Two hours af

Sympathetic reactivity to physiological stress is associated with expanded cardiac extracellular volume in humans.

BACKGROUND: Expanded extracellular volume (ECV) is an early marker of myocardial interstitial fibrosis in patients with hypertension. Animal studies suggest that surges in sympathetic nerve activity (SNA) might contribute more to the development of interstitial fibrosis than the resting level of SNA. The aim of this study was to investigate whether resting SNA or greater SNA reactivity to a stressor may be associated with expanded ECV in humans across a range of blood pressures. METHODS: This was a cross-sectional study in 19 individuals with varying levels of ambulatory systolic blood pressu

Habitual sleep duration is associated with heightened muscle sympathetic nerve activation and blood pressure reactivity in response to the cold pressor test.

The link between short sleep duration and cardiovascular disease may be related to exaggerated neurocardiovascular reactivity. We examined the association of actigraphy-measured habitual sleep duration (HSD) with muscle sympathetic nerve activity (MSNA) and blood pressure (BP) at rest and in response to a cold pressor test (CPT). Thirteen adults (5 males/8 females; 30 ± 9 yr; 26.9 ± 5.8 kg/m2, 117 ± 13/76 ± 6 mmHg) were instrumented for measurement of beat-to-beat BP, forearm vascular resistance, and MSNA at rest and during a 2-min CPT. HSD was measured using wrist actigraphy across 6-7 days.

Skin sympathetic nerve activity in symptomatic and asymptomatic paroxysmal atrial fibrillation.

BACKGROUND: The causes of symptoms in patients with paroxysmal atrial fibrillation (AF) remains unclear. OBJECTIVE: The purpose of this study was to correlate the magnitudes of skin sympathetic nerve activity (SKNA) with symptoms in patients with AF. METHODS: We prospectively enrolled patients with symptomatic paroxysmal AF for ambulatory electrocardiography and SKNA recording. Heart rhythms at the time of symptoms were categorized as AF or normal sinus rhythm (NSR). Maximal and average skin sympathetic nerve activity (aSKNA) and heart rate (HR) were compared between symptomatic and asymptom

New evidence for baroreflex and respiratory chemoreflex-mediated cerebral sympathetic activation in humans.

The brain is highly innervated by sympathetic nerve fibers; however, their physiological purpose is poorly understood. We hypothesized that unilateral cerebral norepinephrine (NE) spillover, an index of cerebral sympathetic nerve activity (SNA), would be elevated when engaging the baroreflex [via lower-body negative pressure (LBNP; -20 and -40 Torr)] and respiratory chemoreflexes [via carbon dioxide (CO2) administration (+8 Torr)], independently and in combination. Twelve young and healthy participants (five females) underwent simultaneous blood sampling from the right radial artery and intern

IL-1R Mediated Activation of Renal Sensory Nerves in DOCA-Salt Hypertension.

BACKGROUND: Clinical trials of renal denervation for the treatment of hypertension have shown a variety of off-target improvements in conditions associated with sympathetic overactivity. This may be due to the ablation of sympathoexcitatory afferent renal nerves, which are overactive under conditions of renal inflammation. Renal IL (interleukin)-1β is elevated in the deoxycorticosterone acetate-salt model of hypertension, and its activity may be responsible for the elevation in afferent renal nerve activity and arterial pressure. METHODS: Continuous blood pressure recording of deoxycorticoste

Divergent splanchnic sympathetic efferent nerve pathways regulate interleukin-10 and tumour necrosis factor-α responses to endotoxaemia.

The efferent branches of the splanchnic sympathetic nerves that enhance interleukin-10 (IL-10) and suppress tumour necrosis factor-α (TNF) levels in the reflex response to systemic immune challenge were investigated in anaesthetized, ventilated rats. Plasma levels of TNF and IL-10 were measured 90 min after intravenous lipopolysaccharide (LPS, 60 µg/kg). Splanchnic nerve section, ganglionic blockade with pentolinium tartrate or β2 adrenoreceptor antagonism with ICI 118551 all blocked IL-10 responses. Restoring plasma adrenaline after splanchnic denervation rescued IL-10 responses. TNF response

Enhanced Expression and Function of Renal SGLT2 (Sodium-Glucose Cotransporter 2) in Heart Failure: Role of Renal Nerves.

BACKGROUND: Recent clinical studies demonstrate that SGLT2 (sodium-glucose cotransporter 2) inhibitors ameliorate heart failure (HF). The present study was conducted to assess the expression and function of renal SGLT2 and the influence of enhanced renal sympathetic tone in HF. METHODS: Four weeks after coronary artery ligation surgery to induce HF, surgical bilateral renal denervation (RDN) was performed in rats. Four groups of rats (Sham-operated control [Sham], Sham+RDN, HF and HF+RDN; n=6/group) were used. Immunohistochemistry and Western blot analysis were performed to evaluate the renal

Silencing Epidermal Growth Factor Receptor in Hypothalamic Paraventricular Nucleus Reduces Extracellular Signal-regulated Kinase 1 and 2 Signaling and Sympathetic Excitation in Heart Failure Rats.

Activation of extracellular signal-regulated kinase 1 and 2 (ERK1/2) signaling in cardiovascular regulatory regions of the brain contributes to sympathetic excitation in myocardial infarction (MI)-induced heart failure (HF) by increasing brain renin-angiotensin system (RAS) activity, neuroinflammation, and endoplasmic reticulum (ER) stress. The mechanisms eliciting brain ERK1/2 signaling in HF are still poorly understood. We tested the involvement of the epidermal growth factor receptor (EGFR) which, upon activation, stimulates ERK1/2 activity. Adult male Sprague-Dawley rats received bilateral

Systemic administration of pentoxifylline attenuates the development of hypertension in renovascular hypertensive rats.

There is evidence to suggest that hypertension involves a chronic low-grade systemic inflammatory response; however, the underlying mechanisms are unclear. To further understand the role of inflammation in hypertension, we used a rat renovascular model of hypertension in which we administered the TNF-α synthesis inhibitor pentoxifylline (PTX, 30 mg/kg/day) in the drinking water for 60 days. In conscious rats, PTX administration significantly attenuated the development of hypertension (systolic blood pressure, PTX: 145 ± 8 vs. vehicle (Veh): 235 ± 11 mmHg, after 38 days of treatment, P < 0.05,

Role of the Carotid Body in an Ovine Model of Renovascular Hypertension.

The carotid body is implicated as an important mediator and potential treatment target for hypertension. The mechanisms driving increased carotid body tonicity in hypertension are incompletely understood. Using a large preclinical animal model, which is crucial for translation, we hypothesized that carotid sinus nerve denervation would chronically decrease blood pressure in a renovascular ovine model of hypertension in which hypertonicity of the carotid body is associated with reduced common carotid artery blood flow. Adult ewes underwent either unilateral renal artery clipping or sham surgery
