Effect of scaffold material, construct length and mechanical stimulation on the in vitro stiffness of the engineered tendon construct.

Introducing mesenchymal stem cell (MSC)-seeded collagen constructs into load-protected wound sites in the rabbit patellar and Achilles tendons significantly improves their repair outcome compared to natural healing of the unfilled defect. However, these constructs would not be acceptable alternatives for repairing complete tendon ruptures because they lack the initial stiffness at the time of surgery to resist the expected peak in vivo forces thereafter. Since the stiffness of these constructs has also been shown to positively correlate with the stiffness of the subsequent repairs, improving i…

Lipoma arborescens of the suprapatellar bursa and extensor digitorum longus tendon sheath: report of 2 cases.

OBJECTIVE: Lipoma arborescens (diffuse articular lipomatosis) is a rare intra-articular lesion consisting of subsynovial villous proliferation of mature fat cells. The usual clinical presentation is painless swelling. The aim of this series was to emphasize the importance of gray scale and color Doppler sonography through the investigation of chronic joint swelling. METHODS: Lipoma arborescens of the knee and extensor digitorum longus tendon sheath at the ankle region are described. The latter is extremely rare. We examined 2 patients with sonography before using cross-sectional modalities.

The results of 163 Achilles tendon ruptures treated by a minimally invasive surgical technique and functional aftertreatment.

BACKGROUND: There is still controversy regarding the optimal surgical technique and post-operative treatment of acute Achilles tendon ruptures. We evaluated a treatment protocol for Achilles tendon ruptures consisting of a minimally invasive Achilles tendon repair combined with early full weight bearing. METHODS: A consecutive group of 163 patients was prospectively followed during a 6 year period (1998-2004) in one university hospital and five teaching hospitals. Data were collected during the outpatient department visits at 1, 3, 5, and 7 weeks, 4 months and 12 months after the intervention

Sonography findings in tears of the extensor pollicis longus tendon and correlation with CT, MRI and surgical findings.

We present our experience in the diagnosis of extensor pollicis longus tendon tears using different imaging methods. In the past 2 years, 12 patients (7 males, 5 females) with extension deficit of distal phalanx of thumb were diagnosed with extensor pollicis longus tendon (EPL) rupture by means of different imaging methods. The ultrasound pattern consisted of a gap between tendon stumps occupied by a continuous (eight cases) or discontinuous (four cases) attenuated hypoechoic string. In nine cases, the tendon ends were identified as a thickened stump-like structure. In the other three cases, t

Flexor carpi radialis tendon rupture as a complication of a closed distal radius fracture: a case report.

Although extensor pollicis longus tendon ruptures have been noted as a complication of distal radius fractures, flexor tendon ruptures in association with acute fractures of the distal radius are rare. We report a rupture of the flexor carpi radialis tendon as a complication of an acute distal radius fracture that was discovered during operative management of the fracture.

Open versus percutaneous repair in the treatment of acute Achilles tendon rupture: a randomized prospective study.

There is no agreement on the ideal type of surgical management for Achilles tendon rupture. The present randomized prospective study was performed to compare outcome data of open and percutaneous repair in the treatment of Achilles tendon rupture. Forty consecutive patients with acute rupture of Achilles tendon were recruited. Patients were randomized to receive open (group A) or percutaneous repair with Tenolig (group B). All patients followed the same rehabilitation protocol except for slight differences in the duration of immobilization. Follow-up included objective evaluation (at 4 and 12

Repair of distal biceps tendon ruptures using suture anchors through a single anterior incision.

PURPOSE: The purpose of this study was to review the results of distal biceps tendon repair via suture anchors through a single anterior incision. METHODS: This is a retrospective review of 17 patients (18 repairs) treated for complete distal biceps tendon rupture between 1998 and 2005 by use of G4 Superanchors (DePuy Mitek, Raynham, MA) in our unit. The length of follow-up was 14 to 70 months (mean, 45 months). RESULTS: There was a mean loss of 5.3 degrees (range, 0 degrees to 50 degrees ; SD, 14.12) of extension when compared with the uninjured side. Of the 17 patients, 6 achieved full ext

An in vitro biomechanical study comparing the spiral linking technique against the pulvertaft weave for tendon repair.

A new spiral linking technique for tendon repair in which one end of the tendon is spiralled around the other end has been developed. Using pig trotter extensor tendons, the Pulvertaft weave technique was compared with this new technique. Twenty-five repairs using each technique were tested by tensile loading with an Instron testing machine. The spiral linking technique matched the strength of Pulvertaft method: the mean peak loads were 102 and 105 N, respectively. The Pulvertaft weave was stiffer than the spiral linking technique: mean stiffness of 11.1 and 6.7 N/mm, respectively. The spiral …

Posterolateral knee reconstruction with an anatomical bone-patellar tendon-bone reconstruction of the fibular collateral ligament.

BACKGROUND: The authors have long advocated a graft reconstruction of the fibular collateral ligament, believing that direct suture repair or augmentation procedures do not provide a stable construct. PURPOSE: To describe an operative technique and determine the clinical outcome of a bone-patellar tendon-bone graft anatomical replacement of the fibular collateral ligament in a consecutive series of knees. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: A consecutive group of knees undergoing anatomical posterolateral reconstruction that included a fibular collateral ligament recon

Simultaneous patellar tendon avulsion fracture from both patella and tibial tuberosity: a case report.

We present a simultaneous patella tendon avulsion fracture from both sides of its attachment in an ectopic ossified patellar tendon. Medially two thirds of the patellar tendon was detached from the inferior patella, and laterally one third of that from anterior tibial tuberosity with the patellar tendon split. Osteosynthesis was used to restore the extensor mechanism of the knee. The result at 46 months was good, even though there were some remaining ectopic ossifications in the patellar tendon.

Results of primary extensor tendon repair in relation to the zone of injury and pre-operative outcome estimation.

INTRODUCTION: The outcome of primary extensor repair in hand surgery has been widely explored, but little systematic effort has been made to investigate the influence of the anatomical zone of tendon injury. Therefore, the aim of our study was to assess the outcome of primary extensor tendon repair with a special focus on the pre-operative state and Verdan's anatomical zones. Our hypothesis being tested was that the outcome after primary extensor repair depends on the complexity of trauma and the site of lesion. MATERIALS AND METHODS: One hundred and seventy seven patients with 203 extensor t

Bilateral quadriceps tendon ruptures in a healthy, active duty soldier: case report and review of the literature.

Unilateral quadriceps tendon ruptures are not uncommon. These injuries have been reported to occur spontaneously and after seemingly trivial trauma in elderly individuals, patients undergoing renal dialysis, and patients with metabolic derangements such as hyperparathyroidism. In young patients, unilateral quadriceps tendon ruptures have been reported as complications of burns, anabolic steroid abuse, and elective orthopedic surgery. Bilateral quadriceps tendon ruptures in young healthy patients are rare injuries. We present the case of a young, healthy, active duty soldier who sustained bilat

Single-stage reconstruction of the flexor mechanism of the fingers with a free vascularized tendon flap: case report.

The reconstruction of the flexor tendon in zones I and II of a ring finger in a single stage, using a free vascularized tendon graft, is reported in a single clinical case. The flexor digitorum superficialis (FDS) tendon of the same finger was freely transferred based on a vascular branch off the ulnar vessels. The ulnar artery was reconstructed with an end-to-end anastomosis, and the free flap was connected to the superficial palmar arch. The functional result was satisfactory.

The use of dynamic EMG in predicting the outcome of split posterior tibial tendon transfers in spastic hemiplegia.

The purpose of this study was to assess the outcome of split posterior tibial tendon transfers in patients with spastic cerebral palsy to determine whether the use of preoperative dynamic electromyography was predictive of surgical success. The study is a consecutive case series of 25 children with spastic hemiplegia who underwent unilateral split posterior tibial tendon transfers for varus foot deformities. Three patients were considered failures because of residual varus, which required further surgery. An additional 5 patients had mild residual varus. Preoperative dynamic electromyographic

Segmental facial anhidrosis and tonic pupils with preserved deep tendon reflexes: a novel autonomic neuropathy.

A 31-year-old woman had exertional right-sided hemifacial flushing and sweating. Examination demonstrated slightly dilated pupils with absent constriction to light and a tonic near response and redilatation, features consistent with Adie syndrome. Neurological examination was otherwise normal, including preservation of deep tendon reflexes. Magnetic resonance imaging of brain and spine were normal. The combination of unilateral loss of sudomotor and vasomotor activity without loss of ocular sympathetic innervation fulfills the diagnosis of Harlequin syndrome. The combination of Harlequin and A

Tendon-bone interface motion in transosseous suture and suture anchor rotator cuff repair techniques.

BACKGROUND: Although many studies involving rotator cuff repair fixation have focused on ultimate fixation strength and ability to restore the tendon's native footprint, no studies have characterized the stability of the repair with regard to motion between the tendon and repair site footprint. HYPOTHESIS: Suture anchor fixation for rotator cuff repair has greater interface motion between tendon and bone than does transosseous suture fixation. STUDY DESIGN: Controlled laboratory study. METHODS: Twelve fresh-frozen human cadaveric shoulders were tested in a custom device to position the shou
