Patterns of contraceptive use through later reproductive years: A cohort study of Australian women with chronic disease.

BACKGROUND: Pregnancies among women with chronic disease are associated with poor maternal and fetal outcomes. There is a need to understand how women use or don't use contraception across their reproductive years to better inform the development of preconception care strategies to reduce high risk unintended pregnancies, including among women of older reproductive age. However, there is a lack of high-quality longitudinal evidence to inform such strategies. We examined patterns of contraceptive use among a population-based cohort of reproductive aged women and investigated how chronic disease

A clinical practice guideline using percentage of predicted forced vital capacity improves resource allocation for rib fracture patients.

BACKGROUND: Predicting rib fracture patients that will require higher-level care is a challenge during patient triage. Percentage of predicted forced vital capacity (FVC%) incorporates patient-specific factors to customize the measurements to each patient. A single institution transitioned from a clinical practice guideline (CPG) using absolute forced vital capacity (FVC) to one using FVC% to improve triage of rib fracture patients. This study compares the outcomes of patients before and after the CPG change. METHODS: A review of rib fracture patients was performed over a 3-year retrospective

Factors associated with long hospital length of stay in patients receiving warfarin after cardiac surgery.

BACKGROUND: Patients receiving warfarin therapy after coronary artery bypass graft (CABG) or valve surgery have longer length of stay (LOS) than those not receiving warfarin therapy. Longer LOS increases patient costs, postdischarge recovery time, and rehabilitation. It is important to identify variables of longer postoperative LOS in this patient population so that the healthcare team can develop and facilitate interventions to minimize length of hospitalization. METHODS: Using a hospital registry and medical record review of cases completed in 2004, data from cardiac surgery patients having

Aptamer-CRISPR/Cas as a tool in point-of-care testing of infectious diseases.

Aptamers and the CRISPR/Cas system have become the cornerstones of target detections due to their numerous advantages. Aptamers enable direct recognition of non-nucleic acid targets, such as pathogens and antigens, while CRISPR/Cas system drives signal amplification and rapid detection. The combined aptamer-CRISPR/Cas system can overcome the limitations of traditional target detection by realizing field-deployable, highly sensitive, specific, versatile, and rapid assays. Given the increasing number of different aptamer-CRISPR/Cas systems developed recently, particularly for infectious diseases

Antibody-detecting resistive biosensor for protein biomarkers.

Proteins serve as important biomarkers for clinical diagnostics, and a simple detection method for point-of-care applications is highly desirable. We developed an antibody-detecting resistive biosensor that utilizes a conductive network of gold (Au) nanoparticles for the detection of protein biomarkers. The proposed biosensor comprises two electrodes bridged by a functional film comprising Au nanoparticles, a polymer matrix, and target-specific antibodies, and the nanoparticle concentration is selected within an experimentally identified percolation transition region to achieve high sensitivit

Spontaneous Tumor Lysis Syndrome in Refractory Metastatic Esophageal Adenocarcinoma Presenting With Septic Shock.

Spontaneous tumor lysis syndrome (STLS) presents a diagnostic challenge when it occurs in patients with advanced solid malignancies and concurrent critical illness. We present a case of STLS in a 30-year-old man with refractory metastatic esophageal adenocarcinoma who presented with hypotension, anuria, and severe metabolic derangements. Laboratory evaluation demonstrated hyperuricemia (18.1 mg/dL), hyperkalemia (7.6 mmol/L), hyperphosphatemia (7.8 mg/dL), and acute kidney injury. Ionized calcium was also low at 1.03 mmol/L despite hypoalbuminemia, with laboratory and clinical findings consist

Reducing pediatric healthcare plastic pollution: Call to action.

Plastic pollution is a growing global health threat, with healthcare an underrecognized contributor. Hospitals generate large volumes of single-use plastics from clinical care, pharmaceuticals, and sterile supply chains, accounting for nearly one-quarter of hospital waste. These materials, largely derived from fossil fuels, introduce more than 13,000 chemicals into the environment and frequently degrade into microplastics and nanoplastics that contaminate air, water, soil, and human biological systems. Children are uniquely vulnerable to plastic pollution due to developmental physiology and hi

Navigating fairness in artificial intelligence-based prediction models: theoretical constructs and practical applications.

Artificial intelligence (AI)-based prediction models, including risk scoring systems and decision support systems, are being increasingly adopted in health care. Addressing AI fairness is essential to fighting health disparities and ensuring equitable model performance and patient outcomes. However, numerous and conflicting definitions of fairness complicate this effort. In this Viewpoint, we aim to support the transition of AI fairness from theory to practice using appropriate fairness metrics. We assess the relation of 27 fairness definitions identified in the literature to the model's inten

The rise and fall of glucocorticoids and immunosuppressants in idiopathic pulmonary fibrosis: a historical paradigm shift driven by mechanistic understanding.

The therapeutic strategy for Idiopathic Pulmonary Fibrosis (IPF) has undergone a fundamental transformation over recent decades. Combination therapy with glucocorticoids and immunosuppressants, particularly the "triple therapy" regimen including corticosteroids, azathioprine, and an antioxidant, has shifted from being the standard of care to being completely abandoned. This article reviews the key evidence-based milestones in this historical shift and, grounded in a modern understanding of the disease's nature, delves into the pathophysiological basis for the ineffectiveness and even harm of g

The new era of AML therapy: current standards and emerging targets.

Over the past decade, AML therapy has evolved from the uniform "7 + 3" regimen toward a personalized approach. This transition integrates molecular genetic profiles with clinical fitness, driven by the genomic elucidation of AML and the development of selective targeted agents. Key advancements include the integration of FLT3 inhibitors into intensive chemotherapy and the emergence of venetoclax. When combined with hypomethylating agents, venetoclax has redefined the standard of care for older or unfit patients. Furthermore, IDH1/2 inhibitors and menin inhibitors have provided potent options f

EEG for bedside monitoring: the intensivist's point of view.

Electroencephalography (EEG) is a powerful tool that can provide unique and real time insight into cerebral functioning in the context of acute brain injury in the intensive care unit (ICU), ranging from focal deficits to seizures and coma. Despite being a safe, relatively inexpensive, non-invasive and meaningful tool, EEG has not yet transitioned into a true bedside monitoring system in the ICU, as continuous EEG monitoring cannot realistically be provided to all ICU patients, and EEG implementation and interpretation remains heavily dependent on specialized personnel. In order to integrate E

Phylogenomics supports monophyly of marsupial crustaceans: a journey to direct development.

Peracarida (marsupial crustaceans) represent one of the most diverse and ecologically important crustacean groups, yet their evolutionary relationships have long been debated. Here, we present the most comprehensive phylogenomic analysis of Peracarida to date, incorporating the relict taxa Thermosbaenacea, Mictacea, Ingolfiellida, and Spelaeogriphacea in a phylogenomic framework. Our results robustly confirm peracarid monophyly and recover a well-supported clade uniting Mancoida (Isopoda, Tanaidaca, Cumacea, Spelaeogriphacea) and Mictacea. We propose for Mictacea to be classified as part of th
