The Fixion expansion nail in the surgical treatment of diaphyseal fractures of the humerus and tibia. Our experience.

The authors present their experience in the application of a Fixion expansion intramedullary nail for the treatment of diaphyseal fractures of the humerus and tibia, in a total of 40 surgeries. The features of the nail are: stability and flexibility of the instrumentation, speed of surgery, minimal exposure to radiation. The results obtained up to now are encouraging; there are no significant problems either during or after surgery, and mean consolidation time is 3 months for fractures of the humerus, and 4 months for those of the tibia. The Fixion nail is a versatile type of instrumentation t

Treatment of low-energy tibial shaft fractures: plaster cast compared with intramedullary nailing.

We analyzed data from 87 patients who had displaced closed or open grade I simple or spiral wedge tibial shaft fractures caused by low-energy impact. Fifty-four patients were treated with plaster cast and 33 with intramedullary locking nail (IMLN). Delayed union only occurred in 8 patients after plaster cast treatment. Forty-two patients in the IMLN group and one in the plaster cast group suffered from anterior knee pain. Final treatment outcome, healing time, hospitalization time and duration of sick leave were assessed on the basis of 25 matched pairs of patients. Mean healing time, hospital

Anterior gonalgic syndrome after intramedullary nailing: ultrasound and radiologic study.

A total of 33 patients submitted to tibial intramedullary osteosynthesis for fracture (27 cases) and non-union were assessed by ultrasound and x-rays an average of 10.9 months after surgery. The route of access was patellar transtendineal in each case; the means of synthesis used was the Marchetti Vicenzi nail. In 19 patients (57.6%) there was anterior gonalgia. Radiologic assessment evaluated prominence of the nail, while nail-tendon impingement was examined by ultrasound. Nail-tendon impingement was frequently observed (24 cases, 72.7%); in cases such as these anterior pain in the knee was p

Subtrochanteric fractures of the femur treated with the Zickel nail.

In a retrospective study of 41 subtrochanteric fractures treated with Zickel intramedullary nails (with a follow-up period of at least 1 year) there was a union rate of 100 per cent (39/39). Union occurred between 4 and 7 months (average healing time 5.2 months). There were no implant failures. Four patients have had their Zickel nails successfully removed. In 48 per cent of the cases we had some difficulties with the stability of the fracture, and in 14 per cent, all of them long oblique comminuted fractures, skeletal traction was needed for 6 weeks. In nine cases there was a shortening of 1.

Locked intramedullary nailing for recent lower limb fractures.

This paper reports the results in a prospective series of recent fractures of both the femur and tibia treated by the Grosse and Kempf locking system of intramedullary nailing. Although the technique requires some attention to detail, it has been possible to treat a wide range of relatively severe injuries with a short hospital stay and rapid rehabilitation, with benefit to both the patient and hospital services.

The displaced femoral neck fracture. Internal fixation versus bipolar endoprosthesis. Results of a prospective, randomized comparison.

The displaced femoral neck fracture poses difficult decision-making issues for the orthopedic surgeon. Young patients frequently require a rapid open reduction and rigid internal fixation in the face of multiple associated injuries. Elderly patients present the typical decision dilemma of internal fixation versus arthroplasty. Consecutive, randomized, prospective series of cases for evaluation of alternatives in the treatment of this difficult fracture are lacking. Between 1982 and 1984, 34 elderly patients with displaced femoral neck fractures were randomized to open reduction or hemiarthropl

Operative treatment of humeral shaft fractures.

The results of the operative treatment of 27 humeral shaft fractures treated at the University of Louisville during a 2-year period were reviewed. The aim of this study was to analyze 1) the indications and results of surgical treatment, 2) the indications for nailing versus plating, and 3) the failures and their treatment (especially surgical nonunions). Indications for surgery were polytrauma patients (including open fractures, associated neurovascular injuries, associated ipsilateral forearm injuries) and isolated unstable fractures in which closed reduction failed. Plate and screw osteosyn

Pediatric supracondylar humeral fractures: Does pin and splint removal 3 weeks after surgery increase the risk of secondary displacement?

PURPOSE: Supracondylar fractures of the distal humerus are among the most common pediatric fractures. For unstable fractures, reduction with pin fixation and postoperative immobilization is standard, but the optimal duration of pin fixation and splinting remains uncertain. This study evaluated whether pin and splint removal approximately 3 weeks after surgery was associated with secondary fracture displacement in children treated for displaced supracondylar humeral fractures. METHODS: We retrospectively reviewed pediatric patients surgically treated for displaced supracondylar humeral fractur

Radial entry for ESIN fixation of pediatric radial fractures: a 10-year cohort study with focus on neurological complications.

BACKGROUND: Elastic stable intramedullary nailing (ESIN) is an established treatment for pediatric radius fractures. However, the optimal surgical entry point remains debated due to differing complication profiles. The radial approach carries a risk of superficial branch of the radial nerve (SBRN) irritation, whereas dorsal approaches have been associated with extensor tendon injuries. This study evaluates the neurological complication profile of the radial approach and provides clinically relevant risk factors. METHODS: A retrospective single-center study was performed including 285 pediatri

Iatrogenic injury to the descending branch of the lateral circumflex femoral artery during intertrochanteric fracture fixation: a case report of guide pin-related vascular complication.

Vascular injuries associated with femoral intertrochanteric fractures are rare but can result in serious complications. A case of iatrogenic injury to the descending branch of the lateral circumflex femoral artery in a patient who underwent surgical fixation of an intertro-chanteric femoral fracture is presented. The injury possibly occurred during the creation of the entry point for nail insertion using a guide pin. Clinical symptoms emerged 10 days postoperatively and were successfully managed with percutaneous embolization, without major complications. This case highlights that pseudoaneury
