[Reconstruction of segmental bone defects using massive osseous and osteocartilaginous allograft].

The reconstruction of large segmental defects after the resection of malignant bone tumors is usually done with modular or custom-made endoprostheses, so far, they appear to work well. From the experience with other indications for endoprostheses it must be admitted that failures will be a matter of time only. With the improved prognosis for patients with primary malignant bone tumors with regard to relapse-free survival and increased chances for permanent cure, the trend for reconstruction procedures should be directed to more 'biological' techniques. The reconstruction of osseous and osteoca

Diaphyseal bone lengthening in children using Wagner device: long-term results.

In a series of 32 patients, the tibia was lengthened nine times and the femur 26 times. The lengthening was 4.1 cm in the tibia and 4.9 cm in the femur. The callotasis principle and atraumatic handling of the periosteum were considered important. The leg inequality was corrected appropriately, except in cases where the leg-length inequality was greater than 12 cm as a result of a progressive congenital deformity. Late femoral fractures occurred in six patients, but no tibial fractures were seen. One hip dislocation was seen after femoral lengthening, one talus deformation developed after the t

Evaluation of an interfragmentary compression system for the repair of equine femoral capital physeal fractures.

Femoral neck and proximal epiphyseal lengths were measured in 37 femurs from 19 cadaver foals that were 1 day to 12 months old to determine the applicability of a human interfragmentary compression system to equine femoral capital physeal fractures. Because components of the implant system are available only in fixed sizes, its use was possible in foals older than 5 weeks of age, but not in younger foals. The 135 degree angle plate conformed best to the equine femur. Femoral capital physeal fractures were created surgically and repaired with the implant system in three foals. Fracture stabilit

[Pars interarticularis reconstruction of spondylolysis using plates and screws with grafting without arthrodesis. Apropos of 78 cases].

Between 1978 and 1985, 78 reconstructions of the pars interarticularis were performed for painful spondylolysis in which there had been failure to respond to conservative methods and the subjacent disc was not degenerate. Sixty-five patients were reviewed with a follow-up greater than 2 years and a mean of 4.4 years. The concept of pars interarticularis reconstruction can be attributed to Kimura (1968), Buck (1970) and Nicol and Scott (1986). It consists of a posterior approach with freshening of the margins of the defect, the insertion of cancellous chips taken from the spinous processes, res

External skeletal fixation. Secondary indications.

In addition to open tibial fractures, external fixation has many other applications to the leg. By keeping the method in mind, the surgeon will find the occasional application in which external fixation may serve as a convenient, safe, and comfortable alternative method of treatment.

Open reduction of central compression fractures of the tibial plateau. Preliminary report of a new method and device arrangement.

A new method of reduction and fixation of a uniformly depressed fracture of the tibial condyle is described. A curved steel probe, introduced through a cortical window in the uninjured condyle, is used to reduce the fracture fragments as well as to compact the fracture region (TV-monitored x-ray projectioning is desired). The instruments needed for the operative procedure are as follows: a probe for reduction and compaction, a tube and a plunger for deposition of bone transplants, and a forked plate. Five cases in which the curved probe reduction method has been used are reported.

First Human Leucocyte Antigen (HLA) Response and Safety Evaluation of Fibrous Demineralized Bone Matrix in a Critical Size Femoral Defect Model of the Sprague-Dawley Rat.

Treatment of large bone defects is one of the great challenges in contemporary orthopedic and traumatic surgery. Grafts are necessary to support bone healing. A well-established allograft is demineralized bone matrix (DBM) prepared from donated human bone tissue. In this study, a fibrous demineralized bone matrix (f-DBM) with a high surface-to-volume ratio has been analyzed for toxicity and immunogenicity. f-DBM was transplanted to a 5-mm, plate-stabilized, femoral critical-size-bone-defect in Sprague-Dawley (SD)-rats. Healthy animals were used as controls. After two months histology, hematolo

Chondrogenesis and osteogenesis are one continuous developmental and lineage defined biological process.

Although chondrogenesis and osteogenesis are considered as two separate processes during endochondral bone formation after birth, recent studies have demonstrated the direct cell transformation from chondrocytes into bone cells in postnatal bone growth. Here we use cell lineage tracing and multiple in vivo approaches to study the role of Bmpr1a in endochondrogenesis. Our data showed profound changes in skeletal shape, size and structure when Bmpr1a was deleted using Aggrecan-Cre ERT2 in early cartilage cells with a one-time tamoxifen injection. We observed the absence of lineage progression of

Maxillary Corticotomies With Bone-to-Bone Retraction and Mandibular Segmental Osteotomy for Correcting an Anterior Double Protrusion.

BACKGROUNDS: This article presents maxillary corticotomies with bone-to-bone retraction and anterior segmental osteotomy (ASO) as an alternative to 2-jaw orthognathics in the bimaxilary protrusion patient with partially anchylosed maxillary anterior tooth. METHODS: The 18-year-old male, complaining of anterior protrusion, with a trauma history to the maxillary central incisor, and requesting rapid treatment, was treated with maxillary corticotomies in 2 stages and ASO in the mandible. The mandibular ASO and palatal corticotomy were done under local anesthesia and 2 weeks later, labial cortico

An Essential Physiological Role for MCT8 in Bone in Male Mice.

T3 is an important regulator of skeletal development and adult bone maintenance. Thyroid hormone action requires efficient transport of T4 and T3 into target cells. We hypothesized that monocarboxylate transporter (MCT) 8, encoded by Mct8 on the X-chromosome, is an essential thyroid hormone transporter in bone. To test this hypothesis, we determined the juvenile and adult skeletal phenotypes of male Mct8 knockout mice (Mct8KO) and Mct8D1D2KO compound mutants, which additionally lack the ability to convert the prohormone T4 to the active hormone T3. Prenatal skeletal development was normal in b

External and internal bone micro-architecture in normal and Kienböck's lunates: a whole-bone micro-computed tomography study.

Kienböck's disease is idiopathic osteonecrosis of the lunate, leading to its fracture and collapse. This study compares internal and external bone micro-architecture of normal and fractured lunates (Kienböck's), by using high-resolution three-dimensional (3D) micro-computed tomography (micro-CT) on the whole bone of the two lunate types, and histology. Fractured Kienböck-diseased lunates were obtained from patients undergoing proximal-row-carpectomy, while normal cadaveric lunates served as controls. 3D-micro-CT-imaging of control lunates revealed an encircling cortex surrounding trabecular bo

Increased Col10a1 expression is not causative for the phenotype of Phex-deficient Hyp mice.

X-linked hypophosphatemic rickets (XLHR) is a severe disorder of phosphate homeostasis and skeletal mineralization caused by mutations of PHEX, encoding a bone-specific endopeptidase. Phex-deficient Hyp mice have been extensively studied to understand the molecular bases of XLHR, and here it was found that Fgf23, encoding a major phosphaturic hormone, was transcriptionally activated in bone-forming osteoblasts. We and others could additionally show that Col10a1 expression is increased in Hyp osteoblasts and bones, thereby raising the possibility that ectopic production of type X collagen could

Socket grafting with the use of autologous bone: an experimental study in the dog.

BACKGROUND: studies in humans and animals have shown that following tooth removal (loss), the alveolar ridge becomes markedly reduced. Attempts made to counteract such ridge diminution by installing implants in the fresh extraction sockets were not successful, while socket grafting with anorganic bovine bone mineral prevented ridge contraction. AIM: to examine whether grafting of the alveolar socket with the use of chips of autologous bone may allow ridge preservation following tooth extraction. METHODS: in five beagle dogs, the distal roots of the third and fourth mandibular premolars were

Characteristic features of trabecular bone in edentulous maxillae.

Following tooth loss, the maxillary alveolar ridge is affected by extensive resorption and its cancellous bone substance undergoes intense remodeling processes. This is particularly important for endosseous implant surgery as the primary stability and thus the prognosis of endosseous implants depends on the cancellous bone density and structure of the alveolar ridge. To analyze the structure of alveolar trabecular bone, 156 sections were obtained from 52 edentulous maxillae (29 female, 23 male; mean age: 72.5 years) from the lateral incisor, first premolar, and first molar regions. The structu

Comprehensive repair of the alveolar cleft using cortical and cancellous bone layers: A retrospective study.

To retrospectively review the clinical effect of comprehensive treatment of alveolar cleft (CTAC) using the mandible as the bone source. Patients with alveolar clefts who met the inclusion criteria were subjected to a CTAC protocol that included the following: (1) preoperative orthodontic treatment for creating good soft-tissue conditions; (2) 'area-like grafting' with subperiosteal osteogenic chin bone instead of cartilaginous osteogenic iliac bone; (3) simulation of normal bone anatomy via a sandwich-like bone graft consisting of 'cortical bone + cancellous bone + cortical bone'; and (4) str

Is transphyseal intramedullary fixation of the distal radius in pediatric fractures a safe procedure? An MRI study.

BACKGROUND: Radius and ulna fractures are very common in the pediatric population. Despite the use of pinning through the growth plate, which was proposed in the past and is still being used to treat these fractures, an instrumental validation to define this procedure as safe has not yet been done. Because of this, in the absence of reliable data regarding the passage of fixation devices through the growth plate, most surgical techniques used for treating radius and ulna fractures are based on absolute respect for the growth cartilage. We conducted an MRI pilot study to evaluate the presence o

Outcomes of intramedullary nailing versus plate fixation for humeral shaft fractures: a retrospective cohort study.

BACKGROUND: Humeral shaft fractures account for 1-3% of all bone fractures. Conservative treatment often leads to complications such as non-union and shoulder stiffness. Surgical fixation with a dynamic compression plate (DCP) has been the gold standard treatment. Intramedullary nailing (IMN) has recently gained popularity due to its minimally invasive approach and reduced risk of radial nerve injury. This study aims to compare the outcomes of IMN and plate fixation for humeral shaft fractures. METHODS: This retrospective study included patients with humeral shaft fractures treated with eithe

Early Outcomes of Multilevel ACDF With Segmental Plate Fixation.

STUDY DESIGN: Case series: retrospective review of prospectively collected data. OBJECTIVE: To study the outcomes of segmental plate fixation in multilevel ACDF. SUMMARY OF BACKGROUND DATA: Multilevel ACDF constructs with a single contiguous plate have higher rates of pseudoarthrosis, dysphagia, and mechanical failure compared with single-level fusions. The use of low-profile single-segment plates in series (segmental ACDF) has been proposed as a means of making multilevel anterior fixation more facile while avoiding the biomechanical limitations of "stand-alone" integrated fixation implants

Locking Plates With Computationally Enhanced Screw Trajectories Provide Superior Biomechanical Fixation Stability of Complex Proximal Humerus Fractures.

Joint-preserving surgical treatment of complex unstable proximal humerus fractures remains challenging, with high failure rates even following state-of-the-art locked plating. Enhancement of implants could help improve outcomes. By overcoming limitations of conventional biomechanical testing, finite element (FE) analysis enables design optimization but requires stringent validation. This study aimed to computationally enhance the design of an existing locking plate to provide superior fixation stability and evaluate the benefit experimentally in a matched-pair fashion. Further aims were the ev
