We report a case of late small intestine perforation by an acetabular cup fixation screw after total hip arthroplasty (THA). A 79-year-old-woman underwent THA for hip osteoarthritis 13 years previously at another hospital. Although the acetabular cup fixation screw protruded into the pelvis, she had no symptoms. She later presented with peritonitis and was admitted to the hospital and prescribed antibiotic therapy. Computed tomography showed that the screw was adjacent to the intestine and was the likely cause of her peritonitis. After the peritonitis healed, she was referred to our hospital f

In this report, the authors described a new minimally invasive technique to repair pars interarticularis defects in adults. The well-established technique using the pedicle screw (PS) and hook-rod system was modified. First, bilateral PSs were inserted percutaneuosly using the Sextant system. Then, through a small skin incision (3-4 cm), an illuminated tubular retractor (Quadrant system; Medtronic Sofamore Danek) was inserted into the pars defect. When this system is used, it is not necessary to detach all the back muscles to access the lytic part. The bursa and fibrocartilaginous mass near th

OBJECTIVE: To determine the correction of flexible pes planus by means of the placement of a conic endorthesis in the subtalar joint. Outcomes were based on measures of the Moreau-Costa-Bartani and talar-navicular angles.
MATERIAL AND METHODS: A series of 22 patients of both genders (7 to 13 years-old), were operated on both feet. The subtalar joint was stabilized by placement of a conic endorthesis. All of them were diagnosed as flexible pes planus, and 44 feet were operated on. All cases had previous conservative treatment without improvement. In these patients the Moreau-Costa-Bartani and

Extraction of the polyethylene liner is necessary if acetabular fixation screws and the metal acetabular shell are to be revised during total hip arthroplasty. In some situations, revision of the polyethylene liner alone may be indicated for excessive wear and osteolysis. A simple method for dissociating the liner from the shell is described in this report. No special instruments are needed with this technique.

A 51-year-old woman with traumatic fracture-subluxation of C6-C7 vertebrae was treated by pedicle screw fixation. Among several methods for surgical treatment of the cervical spine, this technique provides a more rigid anchor to prevent collapsing or instability of the spinal column, particularly at the cervicothoracic junction. However, the risk of injuring the adjacent neurovascular structures cannot be completely eliminated. The characteristics of the cervicothoracic junction, surgical approaches and pedicle screw fixation techniques are emphasised.

Transarticular screw fixation has shown increased stability compared with other posterior stabilization techniques. However, there have been few reports on vertebral artery injury related to the screw insertion. The current study measured the parameters of the pedicle and vertebral artery groove of the axis and clarified the accuracy and safety of the transarticular screw fixation. Direct measurements were taken from 98 dry axis vertebrae. The width and height of the pedicle were measured. The mediolateral and anteroposterior dimensions of the vertebral artery groove also were measured. Forty-

A simple and inexpensive method was developed to obtain a coaxial view of the pedicles to assist with screw insertion. The authors evaluated the accuracy of this device to place transpedicular vertebral screws in a human adult cadaver model. A dual radiation targeting system, a laser targeting system for fluoroscopically guided procedures, was developed to provide an accurate surface entry point and angle of approach to radiographic landmarks. After fluoroscopic cross-hair target localization of the coaxial view of the pedicle, X-ray radiation is turned off and the laser beam allows the surgeo

STUDY DESIGN: Case report of a patient who underwent a new surgical procedure for paraplegia after vertebral collapse due to osteoporosis.
OBJECTIVES: To propose a new approach to posterior spinal fusion surgery for osteoporotic patients.
SUMMARY OF BACKGROUND DATA: Surgical treatment was performed on a paraplegic patient after vertebral collapse due to osteoporosis. However, the surgery was difficult because implants such as hooks and screws often dislodged during the treatment. The poor holding power of these implants to the osteoporotic spine is a challenging problem in this treatment.

Thirty-five fractures of the distal femur were treated by antegrade intramedullary nailing. All of the fractures were treated by a closed technique. In the cases in which there was extension of the fracture to the knee joint, the fracture initially was stabilized with percutaneous lag screws. Four patients died before fracture healing was complete and 11 were lost to followup. There were 14 extraarticular (Type A) and six intraarticular (Type C1 and Type C2) fractures. All of these fractures healed in a mean time of 17.27 weeks. Functional outcome using the Hospital for Special Surgery knee sc

Anatomic reduction, typically obtained by direct visualization through an arthrotomy and internal fixation (open reduction and internal fixation), is the traditional treatment method for displaced intraarticular condylar fractures of the distal femur. We present a case report describing an alternative treatment method, namely, arthroscopic reduction and internal fixation, of a displaced, malrotated intraarticular lateral femoral condyle fracture of the knee. The potential benefits of decreased blood loss, shortened operative time, excellent intraarticular visualization, decreased soft tissue d

The authors report two cases of children treated with growth hormone who had a secondary slipped capital femoral epiphysis. Endocrine disease, radiotherapy are some well-known etiological factors reported in literature, but growth hormone influence is still discussed. This hormone could have an indirect incidence on this pathology because of its effects on growth cartilage. The increased number of treatment with growth hormone is certainly a risk factor for slipped capital femoral epiphysis. Regular checking from "France Hypophyse" allows an early diagnosis of epiphysis and surgical fixation c

To evaluate reduction methods using two of the several types of Cotrel-Dubousset instrumentation (CDI), 39 patients with low-grade (Meyerding grades I and II [12]) lumbar spondylolisthesis were followed clinically and radiologically for a minimum of 6 months. Our surgical methods were classified into three types as follow: reduction by 2-level assembly using double-threaded screws (type I); reduction by single-level assembly using the double-threaded screws (type II); and reduction by single-level assembly using tulip screws (type III). We found that satisfactory reduction could be achieved by

Altogether 28 patients with degenerative (22 cases), isthmic (4 cases), and post-traumatic (2 cases) spondylolisthesis were treated with a reduction fixation system using both angled and pushing-pulling pedicle screws. Preoperatively, the percentages of the sliprage were < or = 25% in 15 cases, < or = 50% in 11, and < or = 75% in 2 cases. All patients had low-back pain and/or leg pain. Postoperatively, 28 cases obtained a nearly anatomical reduction. The average rate of correction was 98%. There is a significant difference (P < 0.01) compared with preoperation. After reduction, all patients ha

The purpose of this study was to develop a noncontacting optical method for the topographical description of surfaces. The method is suitable for all types of surgical implants, including threaded ones. The TopScan 3D system employs a scanning spot and a stationary object at a relatively large working distance to enable proper object alignment, and the depth discrimination property of confocal laser scanning microscopy forms the basis of its surface profiling ability. Scanning is achieved by precision movement of the microscope objective along three orthogonal axes (X, Y, Z). Three-dimensional
