A Sociological Framework for Improving Palliative Pain Care in Low-Resource Nations.

This paper applies institutional theory to palliative pain care in low resource nations (LRNs), suggesting that three interdependent pillars shape access: regulative (laws and rules), normative (professional standards and values), and cultural-cognitive (shared beliefs). Biomedical solutions alone are insufficient. Lasting progress requires coordinated legal reform, professional accountability, and culturally sensitive engagement. RECENT FINDINGS: Restrictive regulations, stigmatizing policy language, and administrative bottlenecks limit access to essential analgesics, while the inverse care l

Educational experience of children living with congenital heart disease: a systematic scoping review.

BACKGROUND: Educational experience of children with CHD is often adversely impacted by factors such as medical burden, social and school functioning challenges. It is, therefore, vitally important that adequate support is provided at an early stage in order to facilitate better educational outcomes for this cohort. The role of the teacher is pivotal in supporting the overall healthy development of a child with CHD. Thus, it is important to understand how we can also support teachers to provide optimal support to this cohort. This systematic scoping review aimed to offer a comprehensive underst

Advancing Telerehabilitation with AI: Predicting Balance Scores from Early Sessions.

Telerehabilitation systems have transformed the management of balance and gait impairments, particularly in older adults with chronic low back pain. This study integrates multimodal balance exercises, cognitive retraining, and augmented reality within the HOLOBALANCE system. Using data from an eight-week rehabilitation program, predictive models based on machine learning (XGBoost and AdaBoost) were developed to estimate final balance outcomes using as few as 3 to 6 sessions. The results indicate that XGBoost performs optimally for shorter prediction horizons (i.e., 3-4 sessions) with RMSE valu

A Unique Interdisciplinary Approach to the Rehabilitation of Two Spouses Involved in Concurrent Trauma: A Case Report.

A married couple presented to our acute inpatient rehabilitation unit after involvement in a motorcycle accident. The 32-yr-old male patient sustained a traumatic spinal cord injury (ie, T5 American Spinal Injury Association impairment scale B paraplegia), and the 29-yr-old female patient suffered a traumatic brain injury resulting in left spastic hemiplegia, increased emotional lability, and cognitive deficits. To facilitate the concurrent rehabilitation of the couple, our spinal cord and brain injury rehabilitation teams worked together to create a joint rehabilitation program for optimal re

Diagnosis of Alzheimer's disease via optimized lightweight convolution-attention and structural MRI.

Alzheimer's disease (AD) poses a substantial public health challenge, demanding accurate screening and diagnosis. Identifying AD in its early stages, including mild cognitive impairment (MCI) and healthy control (HC), is crucial given the global aging population. Structural magnetic resonance imaging (sMRI) is essential for understanding the brain's structural changes due to atrophy. While current deep learning networks overlook voxel long-term dependencies, vision transformers (ViT) excel at recognizing such dependencies in images, making them valuable in AD diagnosis. Our proposed method int

Predicting Progression From Mild Cognitive Impairment to Alzheimer's Dementia With Adversarial Attacks.

Early diagnosis of Alzheimer's disease plays a crucial role in treatment planning that might slow down the disease's progression. This problem is commonly posed as a classification task performed by machine learning and deep learning techniques. Although data-driven techniques set the state-of-the-art in many domains, the scale of the available datasets in Alzheimer's research is not sufficient to learn complex models from patient data. This study proposes a simple yet promising framework to predict the conversion from Mild Cognitive Impairment (MCI) to Alzheimer's Disease (AD). The proposed f

KETAMINE: Neural- and network-level changes.

Ketamine is a widely used clinical drug that has several functional and clinical applications, including its use as an anaesthetic, analgesic, anti-depressive, anti-suicidal agent, among others. Among its diverse behavioral effects, it influences short-term memory and induces psychedelic effects. At the neural level across different brain areas, it modulates neural firing rates, neural tuning, brain oscillations, and modularity, while promoting hypersynchrony and random connectivity between neurons. In our recent studies we demonstrated that topical application of ketamine on the visual cortex

Childhood emotional maltreatment and emerging adults' body dissatisfaction: Self-compassion and body surveillance as explanatory mechanisms.

The association between childhood emotional maltreatment and body dissatisfaction has been widely identified. However, the explanatory mechanisms for this association remain unclear. Guided by the Stress Process Model, the present study tested the potential mediating roles of self-compassion and body surveillance in the associations between emotional maltreatment (i.e., emotional abuse and neglect) and body dissatisfaction. Three-wave, self-report survey data were collected from 668 Chinese young adults (66.8 % female, Mage = 19.96 years old, SD = 1.25) with a 6-month between-wave interval. Re

Distinct neural pathway and its information flow for blind individual's Braille reading.

Natural Braille reading presents significant challenges to the brain networks of late blind individuals, yet its underlying neural mechanisms remain largely unexplored. Using natural Braille texts in behavioral assessments and functional MRI, we sought to pinpoint the neural pathway and information flow crucial for Braille reading performance in late blind individuals. In the resting state, we discovered a unique neural connection between the higher-order 'visual' cortex, the lateral occipital cortex (LOC), and the inferior frontal cortex (IFC) in late blind individuals, but not in sighted con

Cognitive and balance functions of astronauts after spaceflight are comparable to those of individuals with bilateral vestibulopathy.

INTRODUCTION: This study compares the balance control and cognitive responses of subjects with bilateral vestibulopathy (BVP) to those of astronauts immediately after they return from long-duration spaceflight on board the International Space Station. METHODS: Twenty-eight astronauts and thirty subjects with BVP performed five tests using the same procedures: sit-to-stand, walk-and-turn, tandem walk, duration judgment, and reaction time. RESULTS: Compared to the astronauts' preflight responses, the BVP subjects' responses were impaired in all five tests. However, the BVP subjects' performanc

Neuroimaging and neuroendocrine insights into food cravings and appetite interventions in obesity.

This article reviews the previous studies on the distinction between food cravings and appetite, and how they are regulated by hormones and reflected in brain activity. Based on existing research, food cravings are defined as individual preferences influenced by hormones and psychological factors, which differ from appetite, as they are not necessarily related to hunger or nutritional needs. The article also evaluates the neuroimaging findings about food cravings, and interventions to reduce food cravings, such as mindfulness training, alternative sweeteners, non-invasive brain stimulation tec

Dual Semi-Supervised Learning for Classification of Alzheimer's Disease and Mild Cognitive Impairment Based on Neuropsychological Data.

Deep learning has shown impressive diagnostic abilities in Alzheimer's disease (AD) research in recent years. However, although neuropsychological tests play a crucial role in screening AD and mild cognitive impairment (MCI), there is still a lack of deep learning algorithms only using such basic diagnostic methods. This paper proposes a novel semi-supervised method using neuropsychological test scores and scarce labeled data, which introduces difference regularization and consistency regularization with pseudo-labeling. A total of 188 AD, 402 MCI, and 229 normal controls (NC) were enrolled in

Systematic Review: Emotion Dysregulation and Challenging Behavior Interventions for Children andAdolescents with Autism with Graded Key Evidence-Based Strategy Recommendations.

Challenging behavior, such as aggression, is highly prevalent in children and adolescents with autism and can have a devastating impact. Previous reviews of challenging behavior interventions did not include interventions targeting emotion dysregulation, a common cause of challenging behavior. We reviewed emotion dysregulation and challenging behavior interventions for preschoolers to adolescents to determine which evidence-based strategies have the most empirical support for reducing/preventing emotion dysregulation/challenging behavior. We reviewed 95 studies, including 29 group and 66 singl

A preliminary study of the thermal strain experienced by dive support boat personnel.

Anecdotal evidence indicated some British military dive support boat personnel suffer from uncomfortably cold hands and feet, which could impair their performance and be associated with reductions in core body temperature. This study collected preliminary data on thermal stress and cognitive performance of personnel taking part in a cold-weather training exercise. Six men were monitored during four boat transits in a range of climatic conditions (air temperature +1.2°C to -10.5°C, wind chill -4.5°C to 19.3°C). Core body temperature was measured with a radio pill and 12 skin sites (ISO 9886 plu

An olfactory-based Brain-Computer Interface: electroencephalography changes during odor perception and discrimination.

Brain-Computer Interfaces (BCIs) are devices designed for establishing communication between the central nervous system and a computer. The communication can occur through different sensory modalities, and most commonly visual and auditory modalities are used. Here we propose that BCIs can be expanded by the incorporation of olfaction and discuss the potential applications of such olfactory BCIs. To substantiate this idea, we present results from two olfactory tasks: one that required attentive perception of odors without any overt report, and the second one where participants discriminated co
