Congenital muscular dystrophy and severe central nervous system atrophy in two siblings.

Severe degenerative features of the nervous system of a hitherto unknown kind, associated with a neuromuscular disorder with histopathological features of congenital muscular dystrophy, are reported in two female siblings. The clinical profile was characterized by generalized hypotonia followed by spastic tetraplegia, contractures, polyneuropathy, lack of cognitive development and progressive microcephaly. There as no involvement of the eyes. Neuropathological examination of the brain of one sibling, who died at the age of 30 months, revealed subtotal loss of neurons in the cerebral and cerebe…

Molecular and cytologic studies of Ehlers-Danlos syndrome type VIII.

We present a family with findings of Ehlers-Danlos syndrome type VIII and a presenile appearance due to decreased subcutaneous tissue with drawn skin, defective wound healing, contractures, and thin hair. To investigate this syndrome, we studied collagen production and the growth properties of cultured fibroblasts taken from affected relatives. We could not find evidence of a collagen defect or premature senescence of cultured fibroblasts, although the fibroblasts may have a decreased growth rate. We conclude that this family has findings of EDS VIII and premature aging and propose that this o…

[Transposition of the dorsal fascio-cutaneous flap of foot in treatment of talipes equinovarus].

For the purpose of one stage correction of severe congenital Talipes equinovarus, a new technique was designed to fashion a fascio-cutaneous flap from the redundant tissue on dorsolateral area of the foot, and it is then transposed to cover the defect left on the posteromedial part of the same foot following complete releasing and correcting the contractured soft tissue and lengthening the shortened tendons. This newly designed method was applied to 26 feet of severe congenital talipes equino-varus in 16 children aged 3/4-4 years. The foot deformity has been beautifully corrected and the trans…

[Early morphologic changes in the myocardium after a wound of the heart complicated by hemopericardium (experimental study)].

The myocardia of 20 dogs with penetrating injuries of the right ventricles were studied morphologically and histochemically. Microcirculatory disorders found in the myocardium included overfilled venous and capillary bed that eventually led to capillary stasis, interstitial edema, hemorrhagic foci, arteriolosclerosis. For the first time focal disseminated injuries of cardiomyocytes of a segmental, zonal, and contractural type were detected. In line with these lesions, earlier cardiomyocytic changes were found, such as margination of nuclear chromatin, clarification of karyoplasm, interfibrilla

Scapuloperoneal syndrome with cardiomyopathy: report of a family with autosomal dominant inheritance and unusual features.

Four members of a family with scapuloperoneal syndrome were examined and investigated. The pattern of inheritance was autosomal dominant and the myopathic basis of the muscle atrophy was established by histological studies of muscle and spinal cord. The family illustrates an unusual combination of features which appear to be distinct from those of other families with myopathic scapuloperoneal syndrome and autosomal dominant inheritance. These include early age of onset and rapid progression in most cases; occurrence of early muscle contractures; and a high incidence of severe cardiomyopathy in…

The neurovascular island pedicle flap.

Nine patients with irreparable damage to the nerves of the thumb, treated with a neurovascular island pedicle flap from the third or fourth finger to the defect thumb, have been re-examined 9 months to 8 years after the operation. Eight patients had a 2-point discrimination of less than 15 mm corresponding to the flap and all had a positive Ninhydrin test. Two had complete cortical reorientation and three partial reorientation. Six found the function of the thumb to be good or fair. Three described the result as poor and three had to be reoperated because of contractures of the donor finger an…

Severe progressive respiratory involvement requiring ventilator support in autosomal recessive Bethlem myopathy. A case report.

Bethlem myopathy (BM) was first described in 1976 by Bethlem and van Wijngaarden in patients who presented a myopathy characterized by slowly progressive muscle weakness and typical flexion contractures of the long finger flexors, wrists, elbows, pectoralis muscles and ankles. Patients with Bethlem myopathy usually become symptomatic during the first or second decade of life. The condition is in most cases slowly progressive and more than two thirds of patients over 50 years of age may require aids for ambulation. Inheritance is usually autosomal dominant. However, patients with autosomal rece…

Development of a new patient-reported outcome measure for Dupuytren disease: A study protocol.

Objectives: Dupuytren disease is a common condition that causes progressive finger contractures resulting in impaired hand function and difficulties in performing daily activities. Patient reported outcome measures (PROMs) are commonly used in research and clinical practice to evaluate treatment outcomes. Both general upper extremity PROMs and Dupuytren-specific PROMs are available, typically developed using conventional methodology based on classical test theory. However, most current PROMs have been shown to have low responsiveness and the relevance of included items have been questioned. In…

Spontaneous mutation in the COL6A2 gene causing Ullrich congenital muscular dystrophy type 1 in a Chinese child: A case report.

RATIONALE: Mutations in the gene encoding type VI collagen cause Bethlem myopathy (MIM 158810) and Ullrich congenital muscular dystrophy (MIM 254090); 2 diseases previously recognized as completely independent, and have been increasingly recognized. However, collagen-related myopathy caused by intron variation in the COL6 gene is rarely reported in China. Ullrich congenital muscular dystrophy is an autosomal recessive disorder that leads to severe muscle weakness with early onset. Thus, children may never walk independently, with proximal joint contractures and significant hyperelastic distal …

Central Apneas Due to the CLIFAHDD Syndrome Successfully Treated with Pyridostigmine.

NALCN mutations lead to complex neurodevelopmental syndromes, including infantile hypotonia with psychomotor retardation and characteristic facies (IHPRF) and congenital contractures of limbs and face, hypotonia, and developmental delay (CLIFAHDD), which are recessively and dominantly inherited, respectively. We present a patient in whom congenital myasthenic syndrome (CMS) was suspected due to the occurrence of hypotonia and apnea episodes requiring resuscitation. For this reason, treatment with pyridostigmine was introduced. After starting the treatment, a significant improvement was observe…

Dupuytren's disease therapy: targeting the vicious cycle of myofibroblasts?

INTRODUCTION: Dupuytren's disease (DD) is a proliferative fibromatosis of the hand, which causes permanent flexion contracture of the digits and, ultimately, loss of function. The treatment of DD is complex and involves surgical and nonsurgical approaches, with the goal of removing the affected tissue. New biological targets are under investigation in order to develop innovative therapies. AREAS COVERED: The etiology of DD is still unknown. Several authors who focused their studies on the genetics of DD recognized an inherited autosomal dominant pattern. Actually, DD is a multifactorial and c…
