Ultrasound-guided minimally invasive cubital tunnel thread release: An experimental study.

INTRODUCTION: Cubital Tunnel Syndrome is characterized by the compression of the ulnar nerve in the cubital tunnel and can cause restrictions to the activities of daily living. Traditional treatment includes conservative methods and, in more severe cases, different types of surgery. In these cases, common side effects of surgery may limit the clinical success. A new alternative could be a minimally invasive Ultrasound-guided (US-guided) thread release of cubital tunnel retinaculum also known as the Osborne's ligament. The aim of this study was to assess the efficacy and safety of ultrasound-gu

EFFECTIVENESS IN INDIRECT DECOMPRESSION USING MINIMALLY INVASIVE SURGERY - TRANSFORAMINAL LUMBAR INTERBODY FUSION IN SINGLE-LEVEL LUMBOSACRAL SPONDYLOLISTHESIS.

BACKGROUND: The lateral indirect decompression shows many advantages over the posterior approach for patients with low-grade spondylolisthesis or mild canal stenosis. While the minimally invasive surgery - transforaminal lumbar interbody fusion (MIS TLIF) was considered a favourable approach for most surgeons, it can also archive nerve release without exposure, especially with the use of intraoperative neuromonitoring which provides safer and fewer complications. MATERIALS AND METHODS: From 2022 to March 2024, 20 single-level lumbosacral spondylolisthesis was performed MIS TLIF technique, in

An investigation of factors associated with the development of postoperative bone flap infection following decompressive craniectomy and subsequent cranioplasty.

OBJECTIVE: After a decompressive craniectomy (DC), a cranioplasty (CP) is often performed in order to improve neurosurgical outcome and cerebral blood circulation. But even though the performance of a CP subsequent to a DC has become routine medical practice, patients can in fact develop many complications from the surgery that could prolong hospitalization and lead to unfavorable prognoses. This study investigates one of the most frequent complications, bone flap infection, in order to identify prognostic factors of its development. PATIENTS AND METHODS: In this single-center study, we have

Timing of Operative Intervention in Traumatic Spine Injuries Without Neurological Deficit.

BACKGROUND: Numerous studies have demonstrated the benefits of early decompression and stabilization in unstable spine injuries with incomplete neurological deficits. However, a clear consensus on timing to operative intervention still does not exist in those with a normal neurological exam and unstable spine. OBJECTIVE: To determine the optimal timing of operative intervention in traumatic spine injuries without neurological deficit. METHODS: Retrospective chart review at a single institution was performed including patients with traumatic spine injuries without neurological deficit admitte

Central Retinal Vein Occlusion Resolving After Orbital Decompression in Thyroid Eye Disease.

A 49-year-old male presented with proptosis and was found to have optic nerve edema with peripapillary hemorrhages. Diagnostic testing showed a suppressed thyroid-stimulating hormone. CT orbits showed homogenous tendon-sparing enlargement of the medial and inferior rectus muscles, characteristic of thyroid eye disease. Intravenous methylprednisolone was administered given the concern for compressive optic neuropathy. He initially had improvement of his symptoms, so orbital decompression was deferred. Subsequently he presented with worsening diplopia and right proptosis, a new afferent pupillar

Open vascular treatment of median arcuate ligament syndrome.

BACKGROUND: Median arcuate ligament syndrome is a rare condition with abdominal symptoms. Accepted treatment options are open release of median arcuate ligament, laparoscopic release of edian arcuate ligament, robot-assisted release of median arcuate ligament and open vascular treatment. Here we aimed to evaluate the central priority of open vascular therapy in the treatment of median arcuate ligament syndrome. METHODS: We conducted a monocentric retrospective study between January 1996 and June 2016. Thirty-one patients with median arcuate ligament syndrome underwent open vascular surgery, i

Minimal invasive transcaruncular optic canal decompression for traumatic optic neuropathy.

Traumatic optic neuropathy is a cause of loss of vision associated with head injuries. Treatment options include observation, steroids and decompression of the optic canal. We report a case where the optic canal decompression was performed using a transcaruncular approach under a regional block. The incision was made through the caruncle and the dissection was carried down to the periosteum down to the orbital apex where the optic nerve was seen exiting through the optic canal posterior to the posterior ethmoidal artery. The optic nerve was decompressed with good visualization. Hemostasis and

Hidden blood loss during posterior spine fusion surgery.

BACKGROUND CONTEXT: Posterior spine fusion is associated with significant intra- and postoperative blood losses. When referring to the total blood loss during spine surgery, the standard is to measure the intraoperative bleeding plus the postoperative drainage. This ignores the "hidden" blood loss that was found to be significant in other fields of surgery. PURPOSE: The purpose of this study was to examine whether posterior spine fusion carries a substantial hidden blood loss. STUDY DESIGN/SETTING: A prospective study. PATIENT SAMPLE: We prospectively studied 114 patients undergoing instrum

Outcome of deep lateral wall rim-sparing orbital decompression in thyroid-associated orbitopathy: a new technique and results of a case series.

PURPOSE: To describe a new technique for deep lateral (single) wall orbital decompression surgery, developed by Mr. Geoffrey Rose, for proptosis in patients with thyroid-associated orbitopathy and to analyse the results achieved in our series. METHODS: The study is an interventional, retrospective, non-comparative case series. Twenty-one eyes of seventeen patients underwent the described technique of deep lateral wall orbital decompression for thyroid-associated orbitopathy. All patients had controlled thyroid functions and underwent surgery for cosmetic rehabilitation, with analysis of the r

Minimally invasive corpectomy and posterior stabilization for lumbar burst fracture.

BACKGROUND CONTEXT: Surgical indications for lumbar burst fracture remain controversial. Potential indications for surgery include 50% canal compromise, 50% loss of vertebral height, 30° of kyphosis, and posterior element fracture or disruption of the posterior ligamentous complex. Different surgical approaches are available depending on fracture characteristics. It is possible that a minimally invasive approach could allow for a safe and effective treatment with fewer comorbidities than the traditional open technique. PURPOSE: This is a report of an L3 burst fracture treated with a minimally

Minimally invasive techniques for the treatment of osteoporotic vertebral fractures.

Osteoporotic vertebral compression fractures are a leading cause of disability and morbidity in the elderly. The consequences of these fractures include pain, progressive vertebral collapse with resultant spinal kyphosis, and systemic manifestations. Nonsurgical measures have proved unsuccessful in a portion of this population and for this group, minimally invasive vertebral augmentation can be beneficial. Vertebroplasty is designed to address vertebral fracture pain. It involves percutaneous injection of polymethylmethacrylate (PMMA) directly into a fractured vertebral body with the goals of

Carpal tunnel syndrome, the search for a cost-effective surgical intervention: a randomised controlled trial.

OBJECTIVE: There is insufficient evidence regarding the clinical and cost-effectiveness of surgical interventions for carpal tunnel syndrome. This study evaluates the cost, effectiveness and cost-effectiveness of minimally invasive surgery compared with conventional open surgery. PATIENTS AND METHODS: 194 sufferers (208 hands) of carpal tunnel syndrome were randomly assigned to each treatment option. A self-administered questionnaire assessed the severity of patients' symptoms and functional status pre- and postoperatively. Treatment costs were estimated from resource use and hospital financi

Modified orbital decompression for dysthyroid orbitopathy.

PURPOSE: The transantral approach to orbital decompression remains useful for the management of exophthalmos associated with dysthyroid orbitopathy. However, the risk of postoperative diplopia is a concern. Preservation of the anterior periorbita may help support the orbital contents and decrease the incidence of diplopia. METHODS: The medical records were reviewed of 15 consecutive patients who underwent 30 transantral orbital decompressions for proptosis associated with dysthyroid orbitopathy. The procedures were completed in standard fashion, including removal of the inferomedial bony stru

Median Nerve Compression by the Ligament of Struthers: Clinical Image.

In case of weakness of finger flexors and/or persistence of symptoms after carpal tunnel release, a more proximal compression site of the median nerve should be considered. In this report we present a case of compression by a ligament of Struthers. It was detected with ultrasound, which showed a clear thickening of the nerve at the site of a bony spur on the humerus, which was confirmed with magnetic resonance imaging. Surgical decompression resulted in partial recovery of weakness. This case shows the added value of ultrasound in the detection of more proximal median nerve compression such as
