Once daily compared with twice daily administration of slow-release diltiazem as monotherapy for hypertension.

We compared the efficacy of the antihypertensive drug diltiazem in a slow release formulation administered once daily with its twice daily administration as monotherapy in 34 patients with mild to moderate essential hypertension. All subjects received placebo for three weeks before the randomised, double blind, crossover study, and their supine diastolic blood pressure (BP) ranged from 95 mmHg to 115 mmHg. After the patients had received the placebo for three weeks diltiazem was titrated in the open label treatment to either 120 mg or 180 mg twice daily until the target BP level was achieved.

Postnatal development of brain alpha 1-adrenergic receptors: in vitro autoradiography with [125I]HEAT in normal rats and rats treated with alpha-difluoromethylornithine, a specific, irreversible inhibitor of ornithine decarboxylase.

The postnatal development of brain alpha 1-adrenergic receptors was studied in the rat brain using in vitro autoradiography. In some regions, such as the globus pallidus, receptor-binding sites were present at birth and increased during the first week but then decreased to very low levels by adulthood. In contrast, other regions such as the olfactory bulb and cerebral cortex exhibited little binding at birth, with a subsequent increase in receptors during the second week of life that persisted into the mature stage. Several regions had an intermediate pattern with significant labelling at birt

Results of a safety initiative for patients on concomitant amiodarone and simvastatin therapy in a Veterans Affairs medical center.

BACKGROUND: The FDA revised the labels of amiodarone and simvastatin in 2002 to warn of increased risk of rhabdomyolysis, the most severe form of myopathy, when the 2 drugs are taken concomitantly in doses greater than 20 milligrams (mg) per day of simvastatin. The FDA reissued the warning in 2008 after receiving reports of 52 cases of rhabdomyolysis in the Adverse Event Reporting System (AERS) after the label changes in 2002 and suggested use of an alternative statin for patients receiving amiodarone who require more than 20 mg simvastatin to attain lipid goals. OBJECTIVES: To (a) assess the

Chronic effects of exposure to a pharmaceutical mixture and municipal wastewater in zebrafish.

Pharmaceuticals and personal care products (PPCPs) are discharged in municipal wastewater. Effects in aquatic organisms exposed to individual pharmaceuticals in the laboratory have raised concerns regarding the environmental impacts of PPCPs, yet environmental exposures are always to complex mixtures. In this study, adult zebrafish (Danio rerio) showed significantly decreased embryo production after a 6 week exposure to a pharmaceutical mixture (MIX; 0.5 and 10μgL(-1)) of acetaminophen, carbamazepine, gemfibrozil and venlafaxine and to diluted wastewater effluent (WWE; 5% and 25%). Stabilised 111In-labelled DTPA- and DOTA-conjugated neurotensin analogues for imaging and therapy of exocrine pancreatic cancer.

Neurotensin (NT) receptors are overexpressed in exocrine pancreatic cancer and Ewing's sarcoma. The potential utility of native NT in cancer diagnosis and therapy is, however, limited by its rapid degradation in vivo. Therefore, NT analogues were synthesised with modified lysine and arginine derivatives to enhance stability and coupled either to DTPA, to enable high specific activity labelling with indium-111 for imaging, or to DOTA, to enable high specific activity labelling with beta-emitting radionuclides, such as lutetium-177 and yttrium-90. Administration off label of recombinant factor-VIIa (rFVIIa) to patients with blunt or penetrating brain injury without coagulopathy.

Traumatic brain contusions may increase in size over time or may develop at a delay after injury. This may lead to neurological deterioration, long term morbidity or even death. Coagulation disorders after injury can contribute to progression of haemorrhage. Recombinant activated factor VII (rFVIIa) was used in 12 patients with a severe head injury who had no systemic coagulopathy but who were considered to be at risk of progression of their intracranial lesion. Twelve consecutive patients suffering from life-threatening acute head injuries from blunt (3 cases) and penetrating mechanisms were

Bacterial lipopolysaccharide exposure augments aflatoxin B(1)-induced liver injury.

Bacterial endotoxin (lipopolysaccharide; LPS) given to animals in large doses results in pronounced, midzonal liver injury. Exposure to smaller, non-injurious doses of LPS augments the toxicity of certain hepatotoxicants. This study was conducted to delineate the development of injury in a rat model of augmentation of aflatoxin B(1) (AFB(1)) hepatotoxicity by LPS. At large doses (i.e., > 1 mg/kg, ip), AFB(1) administration resulted in pronounced injury to the periportal regions of the liver. Male, Sprague-Dawley rats (250-350 g) were treated with 1 mg AFB(1)/kg, ip or its vehicle (0.5% DMSO/sa

Principles of drug dosing for obese patients in the intensive care unit.

PURPOSE: The increasing prevalence of severe obesity in intensive care unit (ICU) patients has led to several challenges with medication dosing. Doses listed in product labels or tertiary references typically do not account for the pharmacokinetic or pharmacodynamic variability encountered with obesity, leading to the potential for treatment failure or an adverse effect. The purpose of this review is to provide guidance for ICU clinicians when crafting pharmacotherapy regimens in critically ill patients with extreme obesity. METHODS: A literature search was performed to identify relevant arti

Comparative Analysis of Potential Side Effects of Targeted Oral Hypoglycemic Agents Using Spontaneous Adverse Event Reports and Social Media Data.

AIM: Adverse events (AEs) detected in tweets and in the FDA Adverse Event Reporting System (FAERS) provide valuable insights into patient experiences with oral hypoglycemic agents including sodium-glucose transporter 2 (SGLT2) and dipeptidyl peptidase-4 (DPP4) inhibitors. This study compared the side effects identified from tweets with those in the FAERS to identify the AEs associated with each drug. MATERIALS AND METHODS: We collected AE data through tweet and the FAERS during 2017-2021. Relevant sentences in the tweets were annotated and manually labeled to identify AE terms. The data obtai

Dabigatran Prevents Lipopolysaccharide-Mediated Apoptosis in Zebrafish Through a Thrombin-Independent Mechanism.

Endotoxemia is a feature of sepsis pathogenesis and has also been found to mediate the pathophysiology of multiple inflammatory conditions. In this work, we use a lipopolysaccharide (LPS) induced endotoxemia model in zebrafish to identify novel mediators of LPS toxicity. We performed transcriptomic studies on LPS-treated larvae, followed by in silico analysis, which revealed associations between the signatures of LPS-treated embryos and those of drugs involving diverse pathways. In parallel, we performed an in vivo screen using >1,500 Food and Drug Administration-approved compounds and identif

Unravelling the triad of penicillin-binding proteins, β-lactamase activity, and mRNA dynamics in Pseudomonas aeruginosa AmpC induction.

OBJECTIVES: The main objective of the present work was to dissect the interplay between penicillin-binding protein (PBP) occupancy, ampC transcriptional dynamics, and β-lactamase activity in Pseudomonas aeruginosa. METHODS: Using wild-type PAO1 and isogenic LMW-PBP knockout mutants (PAOΔdacB, PAOΔdacBdacC, and PAOΔdacBdacCpbpG), we assessed ampC induction following exposure to 18 β-lactams and β-lactamase inhibitors. PBP binding (IC50) was quantified via Bocillin-FL labelling. AmpC mRNA levels were measured by qRT-PCR, and β-lactamase activity was determined in crude extracts, periplasmic and

Effectiveness of a pharmacist-led protocol to manage cystitis in women, in France: the PharmaCyst' open-label, multicenter, randomized, controlled, cluster study.

BACKGROUND: During their lifetime, more than half of all women will have a urinary tract infection. In many countries, due to difficulty accessing a general practitioner, community pharmacists can prescribe drugs for uncomplicated cystitis. OBJECTIVE: This study evaluated the effectiveness of a pharmacist-led protocol compared to usual care (over-the-counter drugs, lifestyle, referral to a physician) for managing uncomplicated cystitis in women presenting to community pharmacies. METHODS: The PharmaCyst' open-label, multicenter, controlled, cluster-randomized study was conducted in 24 commun

Microfluidic nano-in-microcapsules for ph-responsive colonic delivery of 5-ASA: A moving-boundary kinetic approach.

The performance of targeted oral delivery systems for hydrophobic drugs like 5-aminosalicylic acid is limited by variable release kinetics and premature release in the upper gastrointestinal tract. To address this challenge, monodisperse 5-aminosalicylic acid nanocrystal-in-hydroxypropyl methylcellulose acetate succinate (5-ASA NCs@HPMCAS) composite microcapsules were prepared via microfluidic interfacial assembly and antisolvent precipitation. Droplet-templated solidification yielded microcapsules with an average diameter of 28.33 μm, a coefficient of variation < 3%, a drug loading of 20.4 wt

Evidence Supporting Utility of the Oral Cholate Challenge Test of Liver Function and Physiology to Aid Clinical Management: A Review of Analytical and Clinical Validation Studies.

Current standard liver assessments rely on static laboratory values that reflect advanced disease or liver injury rather than overall hepatic health. This limitation highlights the need for dynamic measures of liver function to support accurate risk stratification, therapeutic monitoring, and clinical trial design. Quantitative liver function tests address this gap by providing physiologically based measures of hepatic clearance. Among these, cholate clearance is uniquely suited to assess hepatocellular function and portal-systemic shunting-key determinants of disease severity and prognosis. T

The European bifurcation club randomized trial of stepwise provisional stenting vs drug-coated balloon therapy for nonleft main true coronary bifurcations: The EBC DCB trial.

BACKGROUND: Drug-eluting stents (DES) are associated with a 2% annual failure rate and have worse outcomes at coronary bifurcations. Recent randomized control data have shown the noninferiority of drug-coated balloons (DCB) vs stents at 1 year. Possible benefits of DCBs include the lack of a metal prosthesis, absence of stent malapposition, lack of obligate antiplatelet treatment, and maintenance of vessel geometry and function. HYPOTHESIS: The use of DCBs in the treatment of nonleft main bifurcation disease is noninferior to a DES strategy for Target Bifurcation Failure at 1 year. STRATEGY

Chitosan nanogels for the improvement of benznidazole activity.

Chagas disease, caused by Trypanosoma cruzi, remains a major neglected disease with limited therapeutic options. Benznidazole (Bzn) is the first-line treatment, despite low aqueous solubility, poor bioavailability and significant side effects, which reduce its efficacy, particularly during the chronic phase of infection. Herein, chitosan (CS) nanogels (NGs) obtained by ionic gelation with tripolyphosphate (TPP) and citrate were developed as nanocarriers for Bzn, aiming to improve its antiparasitic performance. CS obtained from renewable crustacean shell waste was purified, characterized, and u
