We report a case of humero-spinal dysostosis which is only the fourth reported in the literature. The condition is characterized by distal bifurcation of humeri, elbow joint dislocation, spinal malformation, widened iliac bones and talipes equinovarus.

Forty-eight patients who had undergone internal fixation of a displaced fracture of the olecranon by either a tension band technique (34 patients) or screwing (14 patients) were reviewed. The functional results after at least 2 years were equally good with both methods, but there was a considerably higher rate of separation at the fracture after screwing than after inserting a tension band. The technical quality of the fixation was open to criticism in half the cases in each group, and the implications of this are discussed.

We describe a patient who was born with flexion contractures and pterygia at the elbows, clubfeet, torticollis, and several rib fractures. During infancy and childhood, multiple fractures of the lower limbs occurred with minimal trauma and led to disabling deformities. When evaluated at age 19 years, he was normally intelligent, but extremely short, with severe kyphoscoliosis compromising his pulmonary function. Pterygia limited elbow extension to 90 degrees, and severe lower limb deformities prevented ambulation. He did not have blue sclerae, dentinogenesis imperfecta, or hearing loss. X-ray …

Displaced extension type supracondylar fractures of the humerus are associated with neurovascular injuries and malunion (cubitus varus). Reduction is not usually difficult during the first few hours, but it is a problem to maintain. After reduction, the shoulder is placed in abduction external-rotation with the elbow flexed under the C-arm. This position locks the proximal segment of the fracture, and the posteromedially rotated distal segment can then be brought back into an anatomic position under the "medial pillar." In this position, an excellent lateral view is obtained and the "fish tail…

The clinical and radiographic features of chronic posterior subluxation or dislocation of the radial head were studied in thirty-four elbows of twenty-seven patients. Three characteristic radiographic types were noted: Type I, subluxation; Type II, posterior dislocation with minimum displacement; and Type III, posterior dislocation with substantial proximal migration of the radius. Follow-up of eighteen patients (twenty-one elbows) revealed that posterior displacement did not usually cause serious functional impairment except for loss of rotation of the forearm. The least common presentation, …

Early and adequate treatment of haemarthroses, will prevent or lessen future arthropathy. A prospective double blind controlled study of three different dose regimes of Factor VIII in the treatment of haemarthroses of the knees, elbows and ankles has shown that site and severity affect the initial response and should dictate the dose. High risk bleeds can be identified by the degree of movement loss and the presence of pain at rest, and tenderness. Reassessment of bleeds will indicate those not responding. Early recognition of delayed restoration of function requires an analysis of the normal …

The example of the javelin thrower demonstrates that an unphysiological movement sequence at the elbow joint (valgus stress) leads to regular arthrotic alterations by summation of such strains: these are known as "javelin elbow". Since the new javelin introduced in 1986 requires a greater explosive force, a tendency to an increased incidence of these alterations must be reckoned with in the future. Further observations are necessary to clarify whether there will be an increase in degenerative alterations in the lumbar spine by abrupt hyperlordosis and trunk rotation in producing the bow tensio…

Early joint contractures in childhood or adolescence irrespective of muscle weakness are usually found in Emery-Dreifuss muscular dystrophy and collagen-VI related diseases and only rarely in the early stages of other progressive muscular dystrophies. We report a patient presenting severe elbow contractures and a rigid-spine since his early childhood without any evident muscle weakness, who was diagnosed with facioscapulohumeral muscular dystrophy later in life. This case is interesting since there has been no report, to date, of patients with a phenotype resembling facioscapulohumeral muscula…

Radial head and neck fractures are one of the most common elbow fractures, comprising 2% to 5% of all fractures, and 30% of elbow fractures. Although uncomplicated Mason type I fractures can be managed nonsurgically, Mason type II-IV fractures require additional intervention. Mason type II-III fractures with 3 or fewer fragments are typically treated with open reduction and internal fixation using 2 to 3 lag screws. Transverse radial neck involvement or axial instability with screw-only fixation has historically required the additional use of a mini fragment T-plate or locking proximal radius …

The ulnar coronoid process plays a central role in elbow stability due to its unique anatomic characteristics. A fracture of the coronoid, although uncommon, represents a serious injury that can adversely affect functional outcome if not treated appropriately. Several surgical interventions addressing different fracture patterns are being increasingly recognised as effective treatment options even for smaller fragments. A review of the literature was performed in order to evaluate different treatment strategies applied to clearly defined fracture configurations. 14 articles reporting data for …

Olecranon bursitis has a high prevalence and is commonly treated conservatively. However, in case of bacterial infection and open injuries with chronic courses, surgery is indicated. Despite high surgical standards, soft-tissue defects cannot always be avoided in primary surgery for infectious olecranon bursitis. For complicated courses with persisting defects, standardized closing procedures are available. For maintaining adequate elbow function and avoiding long disease progression, definite closure of the defect should be achieved. Various treatment options exist: VAC therapy and local, isl…

Despite advances in surgical techniques, fractures of the radial head are challenging to manage. Most radial head fractures can be managed nonsurgically, with emphasis on early motion to achieve good results. Treatment of more complex radial head fractures, however, especially those associated with elbow instability, remains controversial. The choice for such injury is between open reduction and internal fixation and arthroplasty. Modern implants and techniques have led to improvements in both of these technically demanding procedures. With proper care and understanding of the mechanism of elb…

Isolated fractures of the capitellum are rare injuries and account for 1% of all elbow injuries. The purpose of this study is to evaluate the clinical outcomes of 11 Type I capitellum fractures treated by open reduction and internal fixation using at least two standard Herbert screws between 1998 and 2003. The average age of the patients was 27.5 years. The mean follow-up time was 23.4 months. The final postoperative assessment was made at the 12th month. The results were evaluated according to the Mayo Elbow Performance Index. We obtained excellent result in eight patients and good result in …

The aim of the study was to examine and to compare the level of information and satisfaction among cancer and shoulder-elbow patients in an orthopaedic ward and the development over time. Thirty-eight consecutive patients under suspicion for cancer and 38 consecutive patients with a benign shoulder-elbow disease were included and matched according to age and gender. Three questionnaires were constructed using closed and open-ended questions about level of information and satisfaction in patients' episodes af care. At admission to the ward the patients had a low level of information and satisfa…

Peripheral nerve blockade provides many advantages in the perioperative period for surgery on the shoulder, elbow, wrist, or hand. Aberrant anatomic structures or anomalous courses of nerves may create challenging conditions for use of ultrasound to accurately guide needles for nerve blocks. This paper presents a review of such anomalies affecting the conduct of various peripheral nerve blocks in the upper extremity. A comprehensive literature review was performed, utilizing the PubMed and Embase databases. Variations of anatomy of the brachial plexus and associated structures are described, a…
