Subthalamic stimulation elicits hemiballismus in normal monkey.

High frequency stimulation (HFS) of the subthalamic nucleus (STN) reduces parkinsonian symptoms in the 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP)-treated monkey and in human patients. The effects of stimulation on normal waking primates have never been evaluated. While low frequency stimulation has no effect, HFS induces dyskinesias contralateral to the stimulated STN resembling human hemiballismus and those obtained in primates after neurotoxic lesion or pharmacological blockade of the STN. In the normal monkey, HFS appears reversibly to incapacitate the STN and allow the emergence o

Exteroceptive suppression of temporalis muscle activity: findings in headache.

Exteroceptive suppression of temporalis muscle activity was proposed by Schoenen and co-workers in 1987 as a tool in headache diagnosis and research. Their finding of a decreased or abolished second silent period (ES2) in chronic tension-type headache sufferers has been confirmed by several independent laboratories during the last five years. Temporalis silent periods have also been studied in various other types of headaches. Their modulation by neuropsychological factors and pharmacological agents has also been investigated as well as their retest reliability. The pathophysiological concept

Altered mechanical responses of malignant hyperthermic skeletal muscle during repetitive stimulation.

This investigation examined the mechanical responses of malignant hyperthermic (MH) and normal porcine skeletal muscle to repetitive stimulation. Twitch and maximal tetanic tensions were not significantly different between muscle types. Tensions produced during stimulation at 20-80 Hz were significantly less in MH muscle than in normal muscle. In addition, MH muscle showed significantly greater force decline (tetanic fade) at the end of contractions evoked by 20-80 Hz stimulation. When stimulated to fatigue, both normal and MH muscle exhibited similar rates of tension decline during the initia

Dorsal selective rhizotomy through a limited exposure of the cauda equina at L-1. Technical note.

The technique of the dorsal selective rhizotomy as originally developed by Professor Fasano and by the author is described. The rhizotomy is performed through a one-level laminectomy at L-1. Exposure of the conus medullaris and the cauda equina at this level is adequate to thoroughly assess the reflex electrical response to stimulation of the lumbosacral roots and to section the selected roots/rootlets. This approach, based on over 100 surgical procedures, obviates the need for an extensive laminectomy in a growing child without compromising the clinical results.

Management of spasmodic torticollis.

Based on a review of the literature, the management of spasmodic torticollis may begin with the conservative measures of pharmacotherapy, sensory feedback, or percutaneous dorsal column stimulation. Approximately 50% of patients will benefit from an adequate trial of these modalities. With particularly resistant and disabling torticollis, the ablative procedures of microsurgical cervical rhizotomy or stereotaxic thalamotomy have offered significant relief in about 74% and 56%, respectively, of properly selected cases. Nevertheless, the variability in the duration of response to all forms of th
