Special distraction osteogenesis before bone grafting for alveolar cleft defects to correct maxillary deformities in patients with bilateral cleft lips and palates: distraction osteogenesis performed separately for each bone segment.

INTRODUCTION: Patients with bilateral cleft lips and palates have premaxillary protrusion and characteristic jaw deformities involving three-dimensional malposition of the premaxilla and bilateral maxillary bone segments. This study examined patients with bilateral cleft lips and palates who had deviation and hypoplasia of the premaxillas and bilateral maxillary segments. Before bone grafting, the patients were treated with special distraction performed separately for each bone segment using a halo-type external device. This report describes this novel treatment method which produced good resu

Multivectorial, external halo-assisted midface distraction in patients with severe hypoplasia.

Osteogenesis distraction is an increasingly used technique for the correction of facial deformities because it offers a faster and more controlled growth of the hypoplasic bone along with the elongation of the soft tissues. In this article, we describe the technique and our experience in midface distraction using an external multivectoral distraction device (BLUE Device; W. Lorenz, Jacksonville, FL) and custom-made midface osteotomies for the correction of severe hypoplasia.

Traumatic cranial injury sustained from a fall on the rigid external distraction device.

Distraction osteogenesis has become a popular treatment of congenital maxillocraniofacial anomalies. Many ingenious internal and external devices have been developed and used. The rigid external distraction system based on systems previously used in correction of maxillary retrusion offers postoperative adjustment in two dimensions. Figueroa and Polley reported the use of this device with minimal morbidity in children as young as 5 years of age. They reported no problems with infection, bleeding, pain, loosening of the intraoral splint, dental injury, or wear problems in a series of 14 consecu

Formate-driven in situ CO2 recycling for efficient succinic acid bioproduction in engineered Actinobacillus succinogenes.

BACKGROUND: Succinic acid (SA) biosynthesis presents a promising sustainable alternative to petrochemical routes, yet industrial potential is often hampered by inefficient CO2 fixation and redox imbalances. RESULTS: To systematically address these bottlenecks, a synergistic metabolic engineering strategy was developed in Actinobacillus succinogenes by co-expressing the endogenous gene of formate dehydrogenase (FdoG) and the heterologous gene of phosphoenolpyruvate carboxylase (PPC). This design established a self-reinforcing metabolic loop where FdoG-mediated formate oxidation generated in si

Interactions between external and internal attention processes during working memory.

BACKGROUND: The interaction between internally and externally driven attention (IDA and EDA) during visual working memory tasks has been primarily investigated separately. There is growing evidence that EDA and IDA interact closely during working memory. However, it seems that previous studies, especially those utilizing EEG, did not modulate EDA and IDA within the same task. Thus, there is a paucity of information on how the two processes interact with each other on the neural level. NEW METHOD: We developed a novel paradigm, the PUMA Task, to investigate the interplay between IDA and EDA. T

Practical and accessible applications of CO2-captured absorbents through electrostatic ion exchange.

To address intensified global warming, it is urgently necessary to suppress CO2 emissions, particularly from fossil fuel industries. This does not imply reducing manufacturing activities, but rather preventing the release of CO2 generated during industrial processes into the atmosphere by utilizing the waste CO2 within a closed CO2 cycle. Numerous methods based on chemical conversion and biological fixation have been developed; however, these technologies have not yet provided effective solutions to the rising atmospheric CO2 concentration and ocean acidification because of limitations in scal

A Gaze-Driven Robotic System for Post-Stroke Active Ankle Rehabilitation Training.

Lower-limb rehabilitation benefits from an input channel that conveys intent cleanly while actuation remains predictable and safety-bounded. We present a clinic-friendly, binocular gaze-driven paradigm that maps quadrant fixations to discrete commands for a two-degree-of-freedom ankle robot (dorsiflexion/plantarflexion and internal/external axial rotation), while inversion/eversion can be left compliant or mechanically constrained as needed. Pupil centers from a near-infrared tracker are mapped to a unit-normalized screen plane using low-order regression with ArUco-guided homography and rapid

[Acute management of pilon fractures].

Pilon fractures are complex injuries involving the weight-bearing articular surface of the distal tibia and are frequently associated with relevant soft tissue damage. The treatment requires a structured strategy in which the condition of the soft tissues plays a central role. These fractures result either from low-energy rotational injuries or high-energy axial compression trauma. While the former typically occur in older patients and are associated with limited soft tissue injury, the latter often lead to complex fracture patterns with severe articular impaction and metaphyseal comminution.

Prediction of pedicle screw fixation strength under craniocaudal cyclic load: comparison of various models trained on quantitative CT based finite element analysis.

PURPOSE: This study aims to predict FEA-derived screw fixation strength (FS-CL) under craniocaudal cyclic load using machine learning and deep learning, and to explore whether FS-CL can serve as a surrogate marker for pedicle screw loosening (PSL) risk. METHOD: A retrospective analysis was conducted on 618 screw trajectories data from preoperative of 112 patients. Various ML and DL models utilizing CT images and screw trajectory, were developed to predict screw FS-CL including multilayer perceptron (MLP) and dual-channel 3D ResNet-18 models. Model performance was evaluated using mean squared

Small mammal activities structure fine-scale soil microbial communities and coordinate rodenticide effects in the typical steppes of China.

Soil microbial community is a diverse combination of microbial species that is influenced by soil moisture, physicochemical properties, biological activities, and other factors. Brandt's vole (Lasiopodomys brandtii) is a small rodent distributed in the typical steppes of China, Mongolia, and Russia, which often causes serious pest to the local areas when its population outbreaks. Yet the impacts of rodenticides on soil microbial diversity and ecosystem functions in Brandt's vole habitats are not well-known. In this study, we compared the microbial community composition and functional genes bet

Gustilo IIIC injury of the thigh, MESS score 11, slow oozing of blood on needling the end of the toe, limb preservation successful: A case report.

RATIONALE: For limb Gustilo type IIIC injuries with a Mangled Extremity Severity Score (MESS) ≥7, amputation is often necessary. However, due to advancements in surgical techniques, limb preservation is also feasible for some of these patients. There is still no consensus on the reliability of existing scoring systems for determining amputation or limb preservation. PATIENT CONCERNS: A patient presented with a Gustilo type IIIC injury of the thigh, absolute indications for amputation (thermal ischemia duration nearly 19 hours, MESS score of 11), but had slow oozing of blood on needling the en

Case Report of Overlapping Pubic Symphysis Dislocation Managed Nonoperatively.

Overlapping pubic symphysis dislocation (OPSD) is a rare form of pelvic injury typically caused by high-energy lateral compression forces, resulting in the entrapment of the pubic body against or behind the contralateral pubic ramus. The majority of reported cases have required open reduction and internal fixation due to the mechanical complexity of the injury. However, less invasive management strategies may be feasible in select cases. We present a case of a 56-year-old male with diabetes who sustained an OPSD associated with a right sacral ala fracture, along with multiple nondisplaced rib

Design and mechanical evaluation of a novel dynamic growth rod to improve the surgical treatment of early-onset scoliosis.

AIMS: Growth rods are the gold standard for treating early-onset scoliosis (EOS), but current treatments often fail to optimally promote spinal growth and require frequent distraction surgeries, leading to complications and significant burdens on patients. METHODS: We designed a novel growth rod (NGR) with unidirectional sliding and external regulation capabilities. Using a 3D model, we simulated the implantation of traditional growth rod (TGR) and NGR, applying a 400 N compressive load and a 1 Nm moment to test stiffness. We assessed spinal joint range of motion and growth rod stress distrib

Evaluation and Management of Knee Dislocations.

Knee dislocations (KDs) are potentially limb-threatening injuries characterized by complete displacement of the tibiofemoral articulation. Historically, most KDs resulted from high-energy trauma and sporting activities. However, KDs occurring in morbidly obese patients from low-energy falls, termed "ultra-low-velocity KDs," are becoming more common. Prompt evaluation and recognition are key to achieving a timely reduction and avoiding potentially devastating complications resulting from popliteal artery injury. A suspected KD should be monitored with serial examinations to assess the vascular
